of the Journal Sentinel staff From the Journal Sentinel Last Updated: Aug. 18, 1999 Green Bay - Green Bay Packers right tackle Earl Dotson doesn't want to think about the alternative if rest, rehabilitation and injections don't alleviate a bulging disk in his lower back. But if the pain and numbness he's been feeling since the team's intrasquad scrimmage Aug. 7 doesn't go away, he will probably have to undergo surgery. "I can't get nervous about it," Dotson said. "I'm a little frustrated because I'm on the sidelines. That's part of the game. There are a lot of guys with a lot worse injuries than this. "I just pray to God what we're doing now will keep me from going under the knife."

In between the vertebra of the spine are discs made soft spongelike material in a somewhat harder shell. A disc bulge occurs when the disc pushes out of its normal space. Some disc bulges go undetected for a long time and do not cause any trouble while others may shift to press against one or more nerves causing extreme pain, numbness and or tingling that sometimes radiates down the leg or even into the feet. Symptoms
Any or all of the following:
  • Pain or muscle spasm in the back, sometimes causing the spine to deviate. Pain into one (or very occasionally) both legs - especially if it seems to run down a line. The pain can be constant or flare suddenly with movement. Tingling or numbness in the buttocks, leg or foot.

49ers' Plummer almost totally back

Overcome by neck soreness, unable to even nod or turn his head, 49ers cornerback Ahmed Plummer shut his eyes and held back tears. ''I couldn't take any more,'' Plummer said. ''It was to the point where it was so painful I could have cried, and I don't cry very easily. To move or to get up to do anything meant excruciating pain.'' Since that agonizing plane ride home after racking up eight tackles in an Oct.17 loss to the New York Jets, Plummer has worked to rehabilitate the bulging disk that sidelined him for the final 10 games of the season. Plummer received medical clearance in March to join the team's non-contact mini-camp after an MRI exam showed the swelling around the disk had dissipated. He has been taking part in full-contact drills on a nearly daily basis since the 49ers opened training camp late last month. ''It was an initial shock to my body, but I've been doing everything, all the tackling, and if it ever gets sore, the coaches understand if I need to sit out,'' said Plummer, a first-round selection out of Ohio State in 2000.

So now that you know what an MRI machine looks like, and how it works, the next question is what can it do. Well here are some interesting pictures of the inside of your body, complete with the report that accompanies them. This is what you can expect to receive after a visit to Enrique's laboratory. This patient had a sudden loss of her motor functions (she wasnt able to move her right arms and legs) 2 months before the study. She went thru a slow recovery with lot physical therapy and drugs. She was recovering some of her movements but suddenly all the improvement stop. We performed an MRI that showed the changes expected for a lesion of that time (2 months old) but also showed and increase in the size of the ventricular system( where the Cerebrospinal fluid or CSF flows) that was causing hydrocephalus. Due to this finding, the patient went thru another surgery and had a shunt valve installed, the last word we had from one of her relatives is that she is again on recovery. The official report included this: T 1 coronal SE (spin echo) sequence that shows an area of infarction in the left parietal lobe. Also enlargement of the ventricular system is observed.

Description: Spinal stenosis may be defined as the narrowing of the spinal canal and lateral recesses as a result of progressive degenerative disease of the disk, bone, and ligament. Problems arise when the spinal canal and cord is constricted. Mostly involves the cervical and lumbar spine.
Etiology: Spinal stenosis may be categorized as either congenital (developmental) or acquired. Congenital spinal stenosis may be due to achondroplasia or anomaly, or may be idiopathic. Acquired central spinal stenosis may result from several manifestations including degenerative disk disease, ligamentum flavum hypertrophy, spondylolisthesis, bulging disk and trauma.
Epidemiology: Most commonly found in the cervical and lumbar spine.
Signs and Symptoms: When the cervical portion of the spine is involved, the patient may present with a radiculopathy, myelopathy, neck or shoulder pain. If the lumbar spine is affected, the patient may present with a limping type of gait (neurogenic or spinal claudication), low back pain, or paresthesia of the lower extremities.
Imaging Characteristics: MRI is the modality of choice for diagnosing spinal stenosis. CT following myelography would be the next best choice for imaging spinal stenosis.

    General description Discs are the relatively soft, gelatinous cushions that surround the spinal cord and act as shock absorbers between the hard, bony vertebrae. A herniated disc is a protrusion of the disk material, usually through small openings in the vertebrae where nerves enter the spinal column. Disks herniate most commonly in the lower back, although they also occur frequently in the lower neck and more uncommonly may occur anywhere.
    Causes A disk may herniate because of sudden trauma, anything from a fall on an icy sidewalk to an athletic injury to simply lifting the wrong bag of groceries in the wrong way at the wrong time. They may also be caused simply by the cumulative long term effects of what doctors like to call poor body mechanics - a lifetime of too much bending and twisting too many awkward positions. Although the majority of lumbar lower back herniations occur in people 40-55 , most cervical neck herniations occur in older people, whose disks loose elasticity and become brittle because of normal aging processes.
    Signs and symptoms Depending on where the herniation occurs, and the degree to which nerves entering the spine, or the spine itself, are affected, a wide range of symptoms are possible. In addition to pain around the site of the herniation, many disk patients also experience significant pain somewhere other than where the disk is. This is because when disks ooze and bulge, they ooze and bulge into spaces occupied by nerves. Because these nerves are carrying impulses from different parts of he body to the spine and then to the brain, the pain is experienced as if it were occurring in the area from where the nerve originates.

    I have had arthroscopic surgery twice on my left knee over the past several years. I was told that I may eventually need a knee replacement. I recently read about a new type of minimally invasive knee replacement procedure called Unicompartmental Knee Replacement. Candidates for this type of surgery are those who, for some reason, could not undergo the full knee replacement surgery. Since I have a recently diagnosed inoperable coronary artery problem, do you think I would be a candidate for this procedure? If so, is this a procedure that you do?

    Specifically, if you have a non-operable cardiac condition the risk is not lessened by unicompartment replacement. Either orthopaedic procedure takes approximately one hour, but cardiac clearance would be a deciding factor to have either surgery.
