ACHING BACKS This article was originally printed in the January, 1997, issue of Tufts University School of Veterinary Medicine’s Your Dog newsletter. Backaches are the bane of many people’s lives. But we’re in good company. As it turns out, our four-legged canine friends are not immune to back pain either. One of the most painful and potentially debilitating causes of back pain in dogs is intervertebral disk disease (IDD). But due to advances in veterinary medicine over the past decade - coupled with increased owner awareness - the majority of dogs with IDD recover at least some normal functioning. Shock Absorbers To understand IDD, it helps to envision the canine vertebral column - a hollow tunnel made up of linked, bony segments (vertebrae) that enclose and protect the delicate, gelatinous spinal cord. The spinal cord is the all-important conduit for nerve impulses between all parts of the dog’s body and its brain. Most of the bony segments of the vertebral column are separated by elastic cushions (intervertebral disks), which absorb compressive forces and allow the dog to move comfortably. Dr. Karl Kraus, associate professor of surgery at Tufts University School of Veterinary Medicine, likens a healthy disk to a "jelly doughnut with a rather fibrous outer covering - the annulus fibrosus - and a springy jelly center that absorbs impact - the nucleus pulposus."

Bulging Disc
Bulging discs are not uncommon, and frequently are displayed on MRIs as an abnormality for both young and older adults. Having a bulging disc is not necessarily a serious concern, and it may not even cause back pain. Bulging most likely happens as the body ages and degeneration of the intervertebral disc occurs. A bulging disk is formed when the soft, spongy center of the disk, the nucleus pulposus, pushes out and places pressure on the outer surrounding fibrous ligament, the annulus fibrosis that contains the center. Unlike a herniated disc, the bulging disc still contains the nucleus material.
A bulging disc can be problematic when the protrusion against the annulus enters into the space of the spinal canal. If bone spurs form on the facet joints behind the bulging disc, the combination of the bulging and spur growth can narrow the spinal canal. This occurrence is referred to as spinal stenosis.

Hello Janis, I am an unfortunate expert of sorts on this matter, having
undergone three spine surgeries in my life thus far to repair herniated
disks and the damaged / impinged nerves resulting from them.
Pardon my most basic example, but it works:... The disks between the
vertebrae are like jelly filled donuts. Sometimes pressure builds up and one of them bulges out a bit, but remains in tact with no jelly leak, and often over time goes back to its normal state. Sometimes an abrupt, sudden pressure can cause one of the donuts to burst and the jelly shoots out. (this is called a herniated disk). If the jelly leaks onto the nerves which attach to the spine, or if a piece of the donut itself breaks off (fragments) and affect the nerves, then you have a problem. Nerve impingement causes great pain, discomfort and all kinds of horrible symptoms down into the legs. Bulging disks do not cause these serious problems, so that's GREAT news for you.

DEGENERATIVE SPINE DISEASE John R. Hesselink, MD, FACR
Degenerative spine disease is a major cause of chronic disability in the adult working population and a common reason for referral to an MR imaging center. Spinal degeneration is a normal part of aging, and neck and back pain are one of life's most common infirmities. There are many potential sources of pain, and finding the specific cause is often a confounding problem for both patient and doctor. Pain can originate from bone, joints, ligaments, muscles, nerves, and intervertebral disks, as well as other paravertebral tissues. The landmark article by Mixter and Barr in 1934 on the ruptured intervertebral disk provided an anatomic basis for selected cases of back pain and neurologic dysfunction. Most neck and back pain responds to conservative therapy, but if the pain is unrelenting, severe, or associated with a radiculopathy or myelopathy, imaging is indicated to look for a treatable cause.
EXAMINATION TECHNIQUE In the evaluation of degenerative spine disease, multiple anatomic sites need to be imaged, including the intervertebral disk, spinal canal, spinal cord, nerve roots, neuroforamina, facet joints, and the soft tissues within and surrounding the spine. Many pulse sequences are available, and specific protocols vary among different MR sites. There is general agreement that the spine needs to be imaged in at least two planes, and surface coils are used almost exclusively. In the cervical and thoracic regions a T2-weighted sequence is mandatory to assess damage to the spinal cord. Thin sections are required to visualize the neuroforamina, and pulse sequences must be tailored to counteract CSF flow and physiologic motion. The imaging requirements for the lumbar spine are less strenuous because the anatomical parts are larger. Most protocols include a T1-weighted sequence and some type of T2-weighted sequence to give a myelographic effect.

Treatments of the SPINE HERNIATED DISC There are many different terms to describe spinal disc pathology, and all are used differently by different healthcare practitioners. Some examples of terms used to describe spinal disc abnormalities include:
  • Herniated disc (or herniated disk)
  • Bulging disc
  • Ruptured disc
  • Slipped disc
  • Disc protrusion
  • Disc degeneration (or degenerative disc disease)
  • Disc disease
There is no agreement in the medical field as to the precise definition of any of these terms. Often the patient hears his or her diagnosis referred to in different terms by different practitioners and is left wondering if there is any consensus on what is wrong. Rather than focus on the terminology referring to spinal anatomy, it’s most helpful for patients to focus on understanding the clinical diagnosis . It should be kept in mind that all the terms – herniated disc, bulging disk, slipped disk, ruptured disc, etc. – refer to radiographic findings seen on a CT scan or MRI scan (x-rays can indicate disc degeneration but cannot actually image the disc itself). While radiographic findings are important, they are not as meaningful in determining the source of the pain (the clinical diagnosis) as the patient’s specific symptoms and the spine specialist’s findings on physical exam.

Running intermittently compresses the spine as the weight of the body above any one segment of the spine comes down upon the lower segments when your feet hit the ground. However, intermittent loading is beneficial in that it stimulates bone mineral deposition, which can be helpful in building bone mass and preventing osteoporosis. In order to keep the spine functioning normally, the structures that support the column must remain strong enough to do the job. Spinal stabilization exercises can help strengthen the muscles that keep the spine stable. Here are a few to repeat daily for muscular endurance.

I have a Ruptured C5-C6 disc and have had several trigger point injections. I found them to be very painful and not very effective. After a long conservative treament plan, I am now scheduled for Cervical Fusion of my c5-c6 discs. I felt that the trigger point injections, even when they did help, were only temporary. Good luck!
