    Thromboangiitis Obliterans (Buerger’s Disease) By Belinda Sanford BREIF SYNOPSIS Thromboangiitis Obliterans (Buerger’s Disease) is a nonatherosclerotic, segmental, inflammatory, vasoocclusive disease (Hanly, 2002). Which most commonly affects the small and medium sized arteries, veins, and nerves; chiefly in the arms and legs (Olin, 2000). Von Winiwarter first described thromboangiitis obliterans in a client in 1879 as “A strange endarteritis and endophlebitis with gangrene of the feet” then more thanTwenty -five years later Leo Buerger provided a more accurate and detailed clinical description being “presenile spontaneous gangrene” as cited in Hanly , (2002, pp 2). The prevalence of thromboangiitis obliterans has been in decline over the last fifty years. This is hypothesized to be due to the decline in smoking a believe causative of thromboangiitis obliterans in addition to the more stringent diagnostic criteria. However others believe there may be equally valid reasons contributing to this decline such as the change of contents of cigarettes, and the use of other recreational drugs. The decline of the diseases is reflected by the prevalence of the disease in 1947 to now. In 1947 104 cases per 100,000 patients now estimated to be 12.6-20 per 100,000 patients (Hanly, 2002). Thromboangiitis obliterans is believed to have a genetic component with tobacco use triggering the disease. There is also escalating evidence for the suggestion that an alteration in autoimmune response plays a role in thromboangiitis obliterans (Szuba, 1998).

The foot becomes very pale on elevation and red when put on the ground. The pulses in the leg will be felt for: Femoral - in the groin Popliteal - behind the knee Dorsalis pedis - in the foot (this may be missing in 10% of the normal population) Posterior tibial - behind the inner ankle The distribution of missing pulses will give an indication of the part of the arterial tree of the leg that is diseased. To give a more accurate picture the blood pressure at the ankle is measured using a hand-held doppler and a blood pressure cuff: The highest pressure at the ankle is compared to the highest arm pressure to give an index, the Ankle Brachial Pressure Index (ABPI). This is used to give an estimate of disease severity. In some cases there may be areas of gangrene but with a full set of pulses. This indicates disease in the very small blood vessels supplying the skin - these may become blocked by very small blood clots, inflammation in the arteries (vasculitis), Buergers disease or severe spasm (frostbite and Raynaud's syndrome).

    am new on the block - so please excuse if I there is something that I ought to have read which should have avoided this message. I am interested in getting the plaque out of my arteries. I have buergers'disease. What can be done to remove the plaque? I wonder what the chances of reversing heart condition are? As I read it I can expect a 30% improvement in walking distance if I follow general advice and then a slow regression until a heart attack comes to collect me. Is this right?
