RSD Hypothenar Hammer Buergers Disease Raynaud's Disease Buerger's disease (Thromboangiitis Obliterans) is an inflammatory disease of the small and medium sized arteries and veins of the extremities. It is often seen in men and appears to be directly related to smoking. There may also be an autoimmune element as well. Raynaud’s phenomenon, ulcers and pain are typically seen. RSD Hypothenar Hammer Buergers Disease Raynaud's Disease Vascular disorders encompass a broad spectrum of pathology that results in aberrant microvascular perfusion, potentially threatening the viability of the hand or digits. There are numerous causes of vascular disorders of the upper extremity and hand, and to understand them it is valuable to classify vascular disorders into acute or chronic categories.
Acute Vascular Disorders
Acute injury due to trauma is covered extensively in the replantation section of this site and in the free flap section where examples of severe vascular injury are presented. A subcategory of acute injury is iatrogenic disease due to radial artery catheterization or drug injection. Both are treated with the same principles as major vascular injury. Compartment Syndrome is a condition of increased pressure in the hand or forearm that causes acute loss of blood flow and nerve injury. Treatment is directed toward treating the increase pressure which causes loss of blood flow, and often requires fasciotomy.
Chronic Vascular Disease
Chronic vascular disorders tend to develop more slowly and have a tendency, but not always, to be seen in an older population.

Buerger's disease, also called thromboangiitis obliterans, is a rare disease of the blood vessels of the arms and legs. It's characterized by a combination of inflammation and clots in the arteries and veins that obstructs blood flow. Buerger's disease usually affects men between ages 20 and 40. It's more common in the Middle East and the Far East.
The cause of Buerger's disease isn't clear. But it's strongly associated with smoking. Stopping smoking usually improves the problem. Also, some research suggests that arsenic poisoning may play a role in the disease.
The disease restricts or blocks blood flow to the arms and legs, beginning with the hands, fingers and feet. This eventually damages or destroys tissues and makes them more prone to infections and gangrene.
Signs and symptoms of Buerger's disease include:
  • Pain or tenderness in the fingers, hands or forearms
  • Pain in the toes, feet, ankles or legs
  • Skin changes (atrophy) or ulcers in the fingers and toes
  • Fingers and toes turning white or blue in response to cold or strong emotions (Raynaud's phenomenon) — this is due to blood vessel spasms resulting in less blood flow to fingers and toes
  • Visible short segments of enlarged, red and tender surface veins

I must explain something first. I had been offered cigarettes many times before, but thankfully it was drilled into me that they were very dangerous. So up to that point I had been able to say NO.
It all started when I was 15 years old. I was with two of my buddies. We were at the “free pool” park in West Fargo. One of them pulled out a pack of Marlboro Reds and started smoking a cigarette and my other friend lit his cigarette. Almost simantansly they asked me 'do you want one?' I paused and then I said “yes” not knowing what I had just started.
As time went on I found myself hanging around these guys a lot more than I should of, smoking their cigarettes. I had thought to myself 'cigarettes can't hurt me', but boy oh boy was I wrong.
Smoking up to my senior year in High School brought me to a “Reality Check”. Many of you may be confused by my use of the term “reality check”, but to me it makes perfect sense. Let me explain…

Peripheral Artery Disease
Like the blood vessels of the heart (coronary arteries), your peripheral arteries (blood vessels outside your heart) also may develop atherosclerosis, the build-up of fat and cholesterol deposits, called plaque, on the inside walls. Over time, the build-up narrows the artery.  Eventually the narrowed artery causes less blood to flow, and a condition called ischemia can occur.  Ischemia is inadequate blood flow to the body’s tissue.

Buerger's Disease Etiology and Pathophysiology:
Smoking is very closely related to Buerger's disease and smoking history is one of the criterion for diagnosing the disease. In general if the patient absolutely abandons smoking the course of the disease will be invariably benign, but if smoking continues any treatment will ultimately be futile. Though "passive smoking" has adverse effect on cardiovascular system, non smokers should never develop the disease. Active smokers can be indentified by measuring levels of continine, the major metabolite of nicotine in urine. Since all smokers do not develop the disease an immunopathogenesis is considered probable. It has been proposed in Japanese that presence of a gene linked to some HLA antigens might control the susceptibility to the disease. Socioeconomic conditions, work environment may also play in etiology as the disease is seen more in out door and manual workers. Hypercoagulable state has been observed in association with the disease. Hepatitis B Virus and rickettssiosis may contribute to pathogenesis, but this role is uncertain. Buergers disease is an inflammatry occlusive disease which involves all layers of medium sized and small arteries of the extremitiles. Involved superficial veins bear a close resemblance to those in the affected artery. Majority of the patients develop critical limb ischemia with trophic lesions are distal to ankle, the anklebrachial doppler index could be normal in early stage. Toe pressures can be measured and if it is less than 30 mm Hg, the healing of ulcers is unlikely. The disease though commences peripherally, may gradually extend proximately occluding the larger arteries.
