What is chronic bronchitis?
Smoking is the main cause of more than 8 out of 10 cases of chronic bronchitis and smoking cessation has the most benefit in the long-term outlook. Chronic bronchitis is a chronic inflammatory condition in the lungs that causes the respiratory passages to be swollen and irritated, increases the mucus production and may damage the lungs. The symptoms are coughing and breathlessness, which will get worse over the years.
The definition of chronic bronchitis is chronic cough or mucus reproduction for at least three months in two successive years when other causes have been excluded.

What is Bronchitis?
Bronchitis is basically a less severe case of pneumonia and it is from this infection that pneumonia usually develops from if left unchecked. It is an infection brought about by inflammation of the bronchial tubes, i.e. a respiratory infection. Bronchitis can be acute or chronic. Acute bronchitis is usually severe and usually only lasts a couple of days, whereas chronic bronchitis can last months or even years.
What Causes Bronchitis?
Bronchitis has many causes. A virus, bacteria, heartburn, even smoking, can all lead to a bronchitis infection. It is a very common infection, much like the advanced version of the common cold.

If you have a long-term (chronic) respiratory disease, such as chronic obstructive pulmonary disease (COPD) , acute bronchitis may be treated differently. Bronchitis is inflammation of the tubes that carry air to the lungs (bronchial tubes). When these tubes are inflamed, they swell and produce mucus (sputum). The swollen tubes and increased mucus trigger coughing and may make it more difficult for you to breathe. There are two types of bronchitis:
  • Acute bronchitis usually develops rapidly and lasts 2 to 3 weeks in otherwise healthy people. Most healthy people who develop bronchitis get better without any complications. See an illustration of acute bronchitis Chronic bronchitis recurs and becomes long-term, especially in people who smoke. A cough that produces too much mucus and is present most days during a 3-month period for at least 2 years in a row suggests chronic bronchitis. Chronic bronchitis is a form of chronic obstructive pulmonary disease (COPD).

Cochrane reviews are regularly checked and updated if necessary. Background: Antibiotic treatment of acute bronchitis, which is one of the most common illnesses seen in primary care, is controversial. Most clinicians prescribe antibiotics in spite of expert recommendations against this practice. Objectives: The objective of this review was to assess the effects of antibiotic treatment for patients with a clinical diagnosis of acute bronchitis. Search strategy: In this updated review, we searched the Cochrane Central Register of Controlled trials (CENTRAL) (The Cochrane Library Issue 2, 2004); MEDLINE (January 1966 to March 2004); EMBASE (January 2000 to December 2003); SciSearch from 1989 to 2004; reference lists of articles and the authors' personal collections up to 1996, and also wrote to study authors and drug manufacturers. EMBASE has previously been searched from 1974 to 2000). Selection criteria: Randomised controlled trials comparing any antibiotic therapy with placebo in acute bronchitis or acute productive cough without other obvious cause in patients without underlying pulmonary disease.

ABOUT US Healthcommunities.com Pressroom Testimonials Overview Acute bronchitis is the sudden inflammation of the tracheobronchial tree , which comprises the trachea, or windpipe (tube that leads from the throat to the lungs) and the bronchi (bronchial tubes, air passages of the lungs). It is typically associated with a viral upper respiratory tract infection (URI), such as the common cold, and is usually mild. In patients with chronic lung or heart disease, acute bronchitis is more severe, and can become chronic and progress to pneumonia. Infection causes the mucous membrane of the bronchial tubes to become inflamed and produce thick, sticky mucus. The overproduction of mucus reduces the normal defensive function (called clearing function) of the cilia, the small hairlike structures of the bronchial tubes responsible for moving secretions and debris out of the lungs. Inflammation and accumulated mucus narrow the airways, restrict respiration, and promote bacterial infection. The viruses that cause this condition are often transmitted when they are expelled through coughing, sneezing, and talking. They can also be transmitted through contact with infected drinking glasses and eating utensils.

Bronchitis is inflammation of the large airways that branch off the trachea (bronchi), usually caused by infection but sometimes caused by irritation from a gas or particle. Symptoms lasting up to 90 days are usually classified as acute bronchitis; symptoms lasting longer, sometimes for months or years, are usually classified as chronic bronchitis. When chronic bronchitis also decreases airflow, it is considered a defining characteristic of chronic obstructive pulmonary disease. This chapter discusses acute bronchitis only. Causes
Acute bronchitis can be caused by infection or by exposure to irritants. Infectious bronchitis occurs most often during the winter and is most often caused by viruses. Even after a viral infection has resolved, the irritation it causes can continue to cause symptoms for weeks. Infectious bronchitis may also be caused by bacteria, and it often follows an upper respiratory viral infection. Mycoplasma pneumoniae and Chlamydia pneumoniae often cause bacterial bronchitis in young adults. Among middle-aged and older people, Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis are the most common organisms causing bacterial bronchitis. Viral bronchitis may be caused by a number of common viruses, including the influenza virus. A person often has a combination of bacterial and viral bronchitis.

The value of medical therapy for acute exacerbations of chronic bronchitis generally increases with the severity of the episode. Antibiotics have a definite but limited benefit in severe cases. Systemic corticosteroids are valuable during hospitalization, but not after the patient has been discharged. Once recovery has occurred, pulmonary rehabilitation can improve quality of life.

Case 1 Presentation A 53-year-old man presented with persistent cough and intermittent sputum production. The cough had begun about four weeks earlier, when the patient "caught a cold" from other family members. Self-treatment with over-the-counter cold remedies had been unsuccessful. Because the patient did not have a regular physician, he sought medical attention in the emergency department. The patient worked as a day laborer, primarily in construction. He smoked an average of one pack of cigarettes a day, and had done so for the last 35 years. He became short of breath only on extreme exertion. Other than the cough, he had been in good health.

Definition
Bronchitis is inflammation of the air passages (bronchi) of the lungs. It can make breathing difficult and sometimes painful. Acute Bronchitis “ sudden onset of symptoms. This lasts only a short time and there is full recovery of lung function. Chronic Bronchitis “ long-term inflammation, obstruction, and degeneration of the bronchi. This is often the result of many years of cigarette smoking. This is a serious medical condition, also called chronic obstructive pulmonary disease Asthmatic Bronchitis “ intermittent inflammation of the airways in people with underlying asthma . This is most commonly associated with allergies or environmental irritants. Causes
The inflammation may be caused by:
  • Bacterial and viral infections Smoking (cigarettes or marijuana) Inhalation of certain respiratory irritants (i.e., chemical pollutants, dust), particularly in an occupational setting
