Chronic Bronchitis: Primary Care Management

A patient information handout on chronic bronchitis , written by the authors of this article, is provided on page 2376. C hronic bronchitis is one of the principal manifestations of chronic obstructive pulmonary disease (COPD), the fourth leading cause of death in the United States. About 10 million Americans are affected by some degree of COPD; it causes 40,000 deaths annually. Chronic bronchitis is a clinical diagnosis characterized by a cough productive of sputum for over three months' duration during two consecutive years and airflow obstruction. These requisites exclude more transient causes of cough associated with sputum production, such as acute bronchitis. The airflow obstruction in chronic bronchitis is caused by excessive tracheobronchial mucus production and is distinct from the anatomic findings of distal air space distention and alveolar septa destruction, which define emphysema. Cigarette smoking is the most important risk factor for the development of chronic bronchitis. Over 90 percent of patients with chronic bronchitis have a smoking history, although only 15 percent of all cigarette smokers are ultimately diagnosed with some form of obstructive airway disease.

lungs What are the Symptoms? Some of the most common symptoms are: a cough, wheezing, fever , and soreness in the chest. Is Bronchitis contagious? Yes. Bronchitis is contagious. It can be spread by direct or indirect contact. What causes Bronchitis? Bronchitis may be caused by a virus, bacteria, smoking or the inhalation of chemical pollutants or dust. When the cells of the bronchial-lining tissue are irritated beyond a certain point, the tiny hairs (cilia) within them, which normally trap and eliminate pollutants, stop functioning. Consequently, the air passages become clogged by debris and irritation increases. In response, a heavy secretion of mucus develops, which causes the characteristic cough of bronchitis. Types of Bronchitis? There are two main types of bronchitis: Acute and Chronic. Acute bronchitis may follow or accompany the flu, or it may begin without having had an infection. Acute bronchitis usually lasts about 10 days. If you smoke or are around damaging fumes (such as those in certain kinds of factories), you are more likely to get acute bronchitis and to have it longer. This is because your bronchial tree is already damaged.

The disease is a chronic inflammation of bronchi due to infection, physical and chemical irritations and allergic agents. The majority of the patients are of over middle age. It is characterized by chronic course and recurrent episodes. In some chronic cases, the disease may progress to chronic obstructive emphysema and chronic pulmonary heart disease. In TCM, this disease is related to the categories of "ke sou" (cough), "tan yin" (phlegm retention), "xiao chuan" (asthma) and so on. Main Points of Diagnosis 1. Long-term cough, sputum production and dyspnea which attack repeatedly and fluctuate are worse in autumn and winter. There may be fever, exacerbation of cough and mucopurulent sputum production, and dyspnea is more marked if acute concurrent infection is present. 2. Chronic bronchitis usually persists for many years. It can be classified into 3 stages, acute episodic stage, chronic persistent stage and clinically remissive stage. The last stage may last for a period of time and an attack may be induced by infections or catching a cold. Based on the clinical manifestations, the disease can be divided into simple chronic bronchitis and chronic asthmatic bronchitis. In the former, cough and sputum production are predominant. In the latter, in addition to the above symptoms, there are dyspnea and wheezing, indicating that the spasm of the smooth muscles of bronchi and bronchioli is an important pathogenic factor.

A patient information handout on acute bronchitis , written by the authors of this article, is provided on page 1281. Acute bronchitis is a lower respiratory tract infection that causes reversible bronchial inflammation. In up to 95 percent of cases, the cause is viral. While antibiotics are often prescribed for patients with acute bronchitis, little evidence shows that these agents provide significant symptomatic relief or shorten the course of the illness. In a few small studies, bronchodilators such as albuterol have been found to relieve some symptoms of acute bronchitis. Increased attention is being given to the role of Chlamydia species in acute bronchitis and adult-onset asthma. Studies in progress may help to clarify the importance of these organisms in acute bronchitis and to determine whether early treatment can prevent or ameliorate asthma. A cute bronchitis is one of the most common diagnoses made by primary care physicians.

Bronchitis Overview
Bronchitis generally refers to an acute inflammation of the air passages within your lungs. It occurs when your trachea (windpipe) and the large and small bronchi (airways) in your lungs become inflamed because of infection or other causes.
  • The thin mucous lining of these airways can become irritated and swollen. The cells that make up this lining may leak fluids in response to the inflammation. What causes acute bronchitis?
Acute bronchitis is almost always caused by viruses that attack the lining of the bronchial tree and cause infection. As your body fights back against these viruses, more swelling occurs and more mucus is made. It takes time for your body to kill the viruses and heal the damage to your bronchial tubes. In most cases, the same viruses that cause colds cause acute bronchitis. Research has shown that bacterial infection is much less common in bronchitis than we used to think. Very rarely, an infection caused by a fungus can cause acute bronchitis. Return to top
