Therapy and the Role of Clinical Diagnostic Criteria
In my own face-to-face and online therapy practice , I do not treat clients as manifestations of mental illnesses in need of medical fixing. No one is merely a case of bipolar disorder , an obsessive-compulsive , or a paranoid schizophrenic . In my view, formal diagnostic categories hold some value in terms of contextual information, but they play a much less important role than the specific personal circumstances of a given client. If you are a client, you are first and foremost a specific, unique individual. Nonetheless, in my experience of working with clients who have received formal psychiatric diagnoses from other mental health professionals, I have often found that clients feel they lack adequate information about what these diagnostic labels actually mean. The entries in this section aim to provide the basic facts about each of several diagnostic labels. Please note that the diagnostic information provided here is for educational purposes only and cannot replace the advice of a qualified mental health professional. More information about diagnostic labels is available on the page on the

  • What Is It? Borderline personality disorder is characterized by poor self-image, great difficulty coping with loneliness, and feeling empty. People with this disorder have unstable relationships, highly reactive and intense moods, and impulsive behavior. They are more likely to attempt or commit suicide. Sometimes they harm themselves (for example, cutting or burning) as a form of self-punishment or to combat a numb feeling. At those times, suicide is not the goal.

Everyone has characteristic patterns of perceiving and relating to other people and events (personality traits). Put another way, people tend to cope with stresses in an individual but repetitive style. For example, some people respond to a troubling situation by seeking someone else's help, whereas others prefer to deal with problems on their own. Some people minimize problems; others exaggerate them. Regardless of their usual style, however, mentally healthy people are likely to try an alternative approach if their first response is ineffective.

Personality refers to a distinctive set of traits, behavior styles, and patterns that make up our character or individuality. How we perceive the world, our attitudes, thoughts, and feelings are all part of our personality. People with healthy personalities are able to cope with normal stresses and have no trouble forming relationships with family, friends, and co-workers.

      Recognizing a Personality Disorder
A personality disorder must fulfill several criteria. A deeply ingrained, inflexible pattern of relating, perceiving, and thinking serious enough to cause distress or impaired functioning is a personality disorder. Personality disorders are usually recognizable by adolescence or earlier, continue throughout adulthood, and become less obvious throughout middle age. What Causes a Personality Disorder?
