Metabolic bone disease falls between a number of disciplines and there is, in consequence, a real need for a concise account of the subject for the non-specialist. The present volume which describes the management of common metabolic bone disorders fills this gap admirably. Each of the five middle chapters covers one of the common diseases: osteoporosis; osteomalacia; Paget's disease; hypercalcaemia and hypocalcaemia. The first and last chapters give an overview of pathogenesis and investigation and rare conditions respectively. The text is attractively laid out with good topic headings which makes for easy reference. Each chapter follows a fairly standard format of pathogenesis, clinical features, diagnosis and treatment. Management is covered in a somewhat uneven fashion with some chapters covering treatment in depth while others give a more broad brush approach with identification of the main components of management. It is a pity that the drug treatment of some of the less common conditions, for example hypophosphataemic osteomalacia, are not dealt with in more depth because these are the areas with which the generalist will have the most difficulty. In a volume of this length and scope it is inevitable that there will be emphases with which the specialist may disagree. However, the overall aim of providing a readable text to aid diagnosis and established treatment is well achieved.

Arthritis
The term arthritis covers a group of inflammatory and degenerative conditions that cause stiffness, swelling, and pain in the joints. There are several different types of arthritis, each having different characteristics. The most common form is osteoarthritis which most often involves the knees, hips and hands and usually affects middle-aged and older people. Rheumatoid arthritis is a damaging condition that causes inflammation in the joints and in other body tissues, such as the membranous heart covering, lungs, and eyes.
Osteoarthritis
Gradual degeneration of the cartilage covering the bone ends within joints.

Aluminum and bone disorders: with specific reference to aluminum contamination of infant nutrients
W. W. Koo and L. A. Kaplan
Department of Pediatrics, University of Alberta, Edmonton, Canada. Aluminum (Al) impairment of bone matrix formation and mineralization may be mediated by its direct effect on bone cells or indirectly by its effect on parathyroid hormone and calcium metabolism. Its toxic effects are proportional to tissue Al load. Al contamination of nutrients depends on the amount of Al present naturally in chemicals or from the manufacturing process. Intravenous calcium, phosphorus, and albumin solutions have high Al (greater than 500 micrograms/L), whereas crystalline amino acid, sterile

Interrelationship of magnesium and estrogen in cardiovascular and bone disorders, eclampsia, migraine and premenstrual syndrome
M. S. Seelig
Department of Community and Preventive Medicine, New York Medical College, Valhalla. The anticonvulsive and antihypertensive values of magnesium (Mg) in eclampsia, and its antiarrhythmic applications in a variety of cardiac diseases, have caused Mg to be considered only for parenteral administration by many physicians. In contrast, nutritionists have long recognized Mg as an essential nutrient, because severe deficiencies elicit neuromuscular manifestations similar to those justifying its use in eclampsia. More recently, this element has been used to favorably influence latent tetany with and without thrombotic complications, to delay preterm

Background info:
Dwarfism can be PROPORTIONATE - manifesting a symmetric decrease in both truncal and limb length. Dwarfism can be DISPROPORTIONATE - manifesting either a short limb disorder or a short trunk disorder. Short - limbed dwarfism can affect either the proximal, the middle or the distal region of a limb. FORMS OF DYSPLASIA include: Achondroplasia Spondyloepiphyseal Dysplasia Chondrodysplasia Punctalta Kneist Syndrome - AD Metaphyseal chondrodysplasia Multiple Epiphyseal Dysplasia Dysplasia Epiphysiallis Haemimeliea [known as Trevors Disease] Progressive Diaphyseal Dysplasia - AD Mucopolysaccharidosis Diastrophic Dysplasia - AR Cleidocranial Dysplasia - AD Benign Bone Growth Dysplasia
A) Achondroplasia Achondroplasia is the most common form of disproportionate dwarfism. It is an autosomal dominant (AD) condition with an 80% spontaneous mutation. It is caused by abnormal endochondral bone formation [in the proliferative zone] and can be associated with late-in-life childbirth. It is a quantitative [not a qualitative] cartilage defect. The afflicted child will have dwarfed limbs and a normal trunk.

Vitamin D Therapy on Bone Disorders
Clinical Study Title:
Early phase metabolic bone disorders after gastrectomy: influence of active vitamin D treatment. Plain English Summary: This study examined the use of vitamin D therapy on bone disorders that are common after a gastrectomy. A total of 63 participants were given either vitamin D therapy or not given any additional therapy in the early stages post surgery. The vitamin D supplementation helped to increase calcium and phosphate levels, whiled helping to modify the parathyroid hormone. This suggest that the early stages after a gastrectomy can be pivotal in the development of bone disorders, and that vitamin D therapy can help to prevent bone disorders. Authors

What Are Growth Disorders? There are many causes of growth failure in children. Some are constitutional, some are genetic, and some are the result of hormonal disorders. In some cases the growth disorder may be recognized at birth; in others, a parent may be concerned that the child is the shortest in the class, still wearing the same size clothing as last year, and growing less than two inches a year. In these cases, a thorough medical evaluation by a pediatric endocrinologist (or other growth specialist) is necessary to determine if there is a growth disorder affecting the child's development. What is Human Growth Hormone (HGH) Deficiency HGH deficiency may occur at any time during infancy or childhood, with the most obvious sign being a noticeable slowing of growth. The deficiency may be genetic is some families. Although these children may also possess signs of low blood sugar or obesity, they display average body proportions and (average intelligence. Through the use of injections of synthetic HGF over a period of several years, physicians can help these children deficient in HGH to achieve average height. . What is Cushing's Syndrome (Cortisol, Stress, Hormone Excess)?

  • I'm 39 years old and have suffered from a very rare genetic bone disease called Progressive Diaphyseal Dysplasia (Camurati-Engelmann disease) for 20 years. I've tried to research this disease several times with very limited results. Is there any research at all any where being conducted to understand the mechanics behind this disease? And, are there any treatments experimental or otherwise besides morphine and steroids?
The most recent paper I can find on your illness was published in November 2000, but there seems to be researchers in several areas of the world interested in the disease (which is a good sign!)
I read an interesting abstract from a paper published in 1997 in Argentina, about two women who had good results with using calcium supplements and pamidronate. They both had what is called high bone turnover with their illness, so I don't know whether this paper would even apply to your particular case. You might want to print this out and talk with your doctor about it.

Bisphosphonates used for the treatment of bone disorders inhibit squalene synthase and cholesterol biosynthesis

Department of Cardiovascular Biology, Rhone-Poulenc Rorer Central Research, Collegeville, PA 19426-0107. Some bisphosphonates used for the treatment of bone disorders are also potent inhibitors of squalene synthase, a critical enzyme for sterol biosynthesis. Among seven drugs tested, YM 175 (cycloheptylaminomethylene-1,1-bisphosphonic acid) was the most potent inhibitor of rat liver microsomal squalene synthase (Ki = 57 nM) and sterol biosynthesis from [14C]mevalonate in rat liver homogenate (IC50 = 17 nM).
