In addition to a complete medical history and physical examination, diagnostic procedures for bone disorders may include the following:
  • laboratory tests for blood, urine, and other body fluids
    x-ray
    - a diagnostic test which uses invisible electromagnetic energy beams to produce images of internal tissues, bones, and organs onto film.
    computed tomography scan (Also called a CT or CAT scan.) - a diagnostic imaging procedure that uses a combination of x-rays and computer technology to produce cross-sectional images (often called slices), both horizontally and vertically, of the body. A CT scan shows detailed images of any part of the body, including the bones, muscles, fat, and organs. CT scans are more detailed than general x-rays. A CT scan provides information about bone, muscle, and fat. It is also used to assist the physician in locating the exact area for a biopsy.

What is bone?
Bone is living tissue that makes up the body's skeleton. There are three types of bone tissue, including the following:
  • compact tissue - the harder, outer tissue of bones. cancellous tissue - the sponge-like tissue inside bones. subchondral tissue - the smooth tissue at the ends of bones, which is covered with another type of tissue called cartilage. Cartilage is the specialized, gristly connective tissue that is present in adults, and the tissue from which most bones develop in children.
Together, compact and cancellous tissues are called the periosteum . Beneath the hard outer shell of the periosteum there are tunnels and canals through which blood and lymphatic vessels run to carry nourishment for the bone. Muscles, ligaments, and tendons may attach to the periosteum. Bones are classified by their shape - as long, short, flat, and irregular. Primarily, they are referred to as long or short. There are 206 bones in the human skeleton, not including teeth and sesamoid bones (small bones found within cartilage):
  • 80 axial bones , which include the head, facial, hyoid, auditory, trunk, ribs, and sternum.

The World Health organization has named this decade the “Bone and Joint Decade”, in anticipation of new methods of prevention and treatment of musculoskeletal disorders.
The past decade has provided us with much new information on calcium homeostasis. Identification of the PTh receptor and its mutations have now been linked to a number of disorders for which there were no previous explanations, for example Jansen Type Metaphyseal Chondrodysplasia. New discoveries about the calcium sensing receptor mutations have led to the development of calcimimetic drugs, which potentiate the activity of calcium and will be available soon. Various genetic studies have emerged in the past decade giving insights into the origin of diseases such as hypophosphatemic rickets and others. An in-depth update on osteoporosis and other bone diseases is also given.

Fibrous Dysplasia
What is fibrous dysplasia?
Fibrous dysplasia is a chronic disorder in which bone expands due to abnormal development of fibrous tissue, often resulting in one, or more, of the following:
  • uneven growth of bones
  • pain
  • brittle bones
  • bone deformity
Any bone can be affected. More than one bone can be affected at any one time, and, when multiple bones are affected, it is not unusual for them to all be on one side of the body. However, fibrous dysplasia does not spread from one bone to another. The most commonly affected bones include the following:
  • femur (thighbone)
  • tibia (shin bone)
  • ribs
  • skull
  • facial bones
  • humerus (the bone of the upper arm)
  • pelvis
  • vertebrae in the spine (less often)
Some people develop hormonal problems and a condition called McCune-Albright syndrome. McCune-Albright syndrome, another form of fibrous dysplasia, includes different symptoms, such as early onset of puberty and skin spots, called cafe-au-lait spots. Fibrous dysplasia usually occurs in children and young adults, and is found equally between males and females.
