    Previous Next Treatment of Patients with Body Dysmorphic Disorder BDD has been considered an obsessive-compulsive disorder because some of the clinical features are similar (age of onset, intrusive thoughts and recurring behaviors); thus, the treatment of patients with BDD has involved the use of similar medications. Some preliminary study results suggest a limited response to selective serotonin reuptake inhibitors, but no controlled treatment studies have been published. Other agents, such as lithium, neuroleptics, trazadone and benzodiazepines, have not proved effective. Hollander and colleagues performed the first systematic double-blind, controlled study of the pharmacologic treatment of patients with BDD. They compared clomipramine (a potent serotonin reuptake inhibitor) with the active control desipramine (a selective norepinephrine uptake inhibitor). Patients enrolled were between the ages of 18 and 65 who met the Diagnostic and Statistical Manual of Mental Disorders

    This is in no way an attack on anyone or a matter of disbelief of the viable reason for depression you may be going through. This is a proven concern and the rate of incidence is alarming. Depression can be from medications, anesthesia, mental trauma, pre-existing psychological disorders, unrealistic expectations or post-operative pain, complications, lack of communication, surgeon error or normal after effects that the patient was not aware he or she would experience. I actually did not believe in nor did I agree with disorders such as the newly diagnosed and much thrown around Body Dysmorphic Disorder (BDD) until I came across several patients who in fact DID have it and kept getting surgery to attain a happiness that surgery would never bring them. Obviously the disorder itself has always been around, and seemingly either the diagnoses have increased or the cases, in general, have increased. However, so has the population so one probably cannot make a truly informed opinion without examination of persons with BDD in the past or their case notes. Regardless, it is a real problem and it must be treated with delicacy. No one wants to be told they have BDD. But having friends with BDD, I can say that it was a wonderful day once they realized they did indeed have an issue with their body image and sought help.

    Serotonin, psilocybin and body dysmorphic disorder: A case report

    Body dysmorphic disorder, preoccupation with an imagined defect in appearance, or excessive concern with a slight physical anomaly, has been successfully treated recently with serotonin reuptake inhibitor (SSRI) medication [1,2], which has led to the proposition of a possible serotonergic basis for this disorder [1,2]. Further evidence of this theory comes from reports of exacerbation of body dysmorphic disorder during tryptophan depletion [3] and use of the 5-HT antagonist cyproheptadine [4]. In support of this theory we present a case in which body dysmorphic symptoms were qualitatively altered with the ingestion of psilocybin, a potent serotonergic agent [5] in a patient who later responded to the SSRI fluoxetine.
    Mr A, a 27-year old Caucasian man, presented with symptoms of body dysmorphic disorder, reflecting preoccupation with perceived emaciated and gaunt cheeks, although his appearance seemed unremarkable. Mr A was an unemployed single man, who had never married. He reports that he had never engaged in a significant romantic relationship, due mainly to the fact that he had suffered social avoidance and depressive symptoms as a result of his feeling of ugliness, since the age of sixteen. He had been able to complete two years of tertiary education, but had withdrawn from his studies due to his worsening symptoms. Mr A had a long history of visits to plastic surgeons and dermatologists for treatment of his thin cheeks, and other facial preoccupations (e.g. prominent chin, facial scarring), although his appearance was normal.

    Preoccupation with an imagined defect in appearance. If a slight physical anomaly is present, the person's concern is markedly excessive. The preoccupation causes clinically significant distress or impairment in social, occupational, or other important areas of functioning. The preoccupation is not better accounted for by another mental disorder (e.g., dissatisfaction with body shape and size in Anorexia Nervosa).

I can't speak for your cousin, but psychologists estimate that one to two percent of the population has what is called body dysmorphic disorder. These people obsess over what they perceive as a terrible defect in their physical appearance. In contrast to the case with eating disorders, where people's concern over their physical appearance typically involves their body image as a whole, people with BDD are concerned about specific physical features. The concern often has to do with the face or head, but BDD can involve any body part. A person with the disorder might think her nose is too big or his chest not muscular enough. Sometimes the perceived defect is invisible to anyone else. In other cases, such as is the case with thinning hair, the individual's perception of that body part is extreme, far from what an objective observer sees. D: A lot of people obsess over their appearance in some way or another.
