Muskoka-Parry Sound Health Unit "Working for a healthier tomorrow" Blastomycosis What is blastomycosis? Blastomycosis is a flu-like illness and is often diagnosed as pneumonia. People who develop blastomycosis in their lungs may experience sudden fever, cough, chest pain, weight loss, shortness of breath and a general feeling of fatigue. Not all of these symptoms have to be present. Generally, blastomycosis starts in the lungs but on some occasions it can spread to the skin. The individual will develop slowly spreading painless red pimples, which can become elevated, crusty or ulcerated. Lesions are usually found on the face and exposed extremities, although mucosal involvement of the nose, mouth or pharynx may occur. On rare occasions, blastomycosis can spread to the bone, prostate, epididymis and other organs. In some cases, the symptoms can go away by themselves. How is it spread? The spore attaches itself to dust and when the dust is stirred up it can be inhaled. Inhalation of this spore produces a yeast infection in the lungs. Direct contact with blastomycosis through an open wound or puncture could produce a skin infection. Transmission of the disease from animal to human is not possible. How is blastomycosis prevented/treated?

Blastomycosis is a fungal disease caused by the organism Blastomycosis dermatitides It is primarily seen infecting dogs, humans and rarely, cats. How does your pet get the infection? Your pet can become infected by inhaling the organism from the soil. It is not known to be transmitted from individual to individual or from animals to humans. Each individual acquires the infection independently and directly from the environment. It is common to see hunting dogs that root and dig in the ground become infected. New house or cottage building or renovations, or soil excavations, may also expose infected soils from deep below the ground which could increase the chance of your pet getting the infection. Where is Blastomycosis found? Blastomycosis can be found in soils extending from the Mississippi-Missouri-Ohio River basin, which reaches up into the Lake of the Woods area. These areas include Sioux Narrow, Dryden, Red Lake, Kenora and surrounding areas. Blastomycosis grows best during warm moist weather and in acidic soils such as occurs in cedar swamps.

Blastomycosis is a rare infectious multisystem disease that is caused by the fungus Blastomyces dermatitidis. The symptoms vary greatly according the affected organ system. It is characterized by fever, chills, cough, and/or difficulty breathing (dyspnea). In the chronic phase of the disease, the lungs and skin are most frequently affected. The genitourinary tract and bones may also be involved.

The remainder of the joint exam was unremarkable. The chest, cardiovascular, and abdominal exams were unremarkable.

Laboratory Studies White blood cell count was 10,590 #/cu mm, with a normal differential; hematocrit 31.3 %; platelets 573,000 . ESR 123 mm/hr, total protein 8.4 gm/dl, albumin 3.6 gm/dl, protein gap 4.8. RPR negative. HIV negative. Urinalysis revealed too numerous to count white blood cells with moderate leukocyte esterase. A PPD skin test was negative, but a Candida control was positive. Bacterial, acid fast bacillus, and fungal cultures were sent of the blood, sputum, and urine. Incision and drainage of several nodules expressed purulent material, which was sent for pathologic examination and cultures, along with biopsies of the nodules. All initial cultures were negative for any organisms. top of page Radiology Studies Radiographs revealed a right upper lung lobe infiltrate (Figure 3) and bony erosions in the left anterior tibia (Figure 4) and distal right second metacarpal.

    Overview Symptoms Treatment Prevention Definition: A skin lesion of blastomycosis is a symptom of an infection with Blastomyces dermatitidis in which the skin becomes infected as the fungus spreads throughout the body. Causes, incidence, and risk factors: Blastomycosis is a rare fungal infection that occurs primarily in the central and southeastern states of the US, Canada, India, Israel, Saudi Arabia, and Africa. It is acquired by inhaling fungal particles found in moist soil, particularly amongst decaying vegetation. People with immune system disorders are at highest risk. The lungs are the portal of entry for this infection and are the main organ involved. The fungus may disseminate (spread) to other areas of the body after initial infection in the lungs and affect the skin, bone/joints, genitourinary tract and other systems. Skin disease is primarily a manifestation of disseminated (widespread blastomycosis) and occurs in up to 80% of individuals. Skin lesions may be wartlike or appear as ulcers and may affect the nose and mouth as well.

    Causes
    Physicians disagree on the cause of this disorder, unless it obviously follows ingestion of caustic material such as lye or strong acids. It may be a congenital defect (present from before birth). Alternatively, it may be related to chronic injury caused by gastroesophageal reflux disease (GERD), in which stomach acid leaks into the esophagus and irritates it. Esophageal rings are known to occur from drinking a caustic substance, causing scar tissue. The condition may produce swallowing difficulties

    Myocardial contusion is a "bruise" of the heart muscle , often caused by blunt trauma to the chest following falls, motor vehicle accidents, or cardio-pulmonary resuscitation (CPR). This can cause abnormal heart movement and "stunning" of the affected area of heart muscle. In non-severe cases, the affected area of heart muscle often regains its function without any permanent scarring.

    Causes
    The most common causes are falls from heights greater than 20 feet, motor vehicle accidents (usually the driver due to contact with the steering wheel), or during cardiopulmonary resuscitation (CPR).
