Blastomycosis
What is blastomycosis, and how does an animal get this disease? Blastomycosis is a fungal disease caused by Blastomyces dermatitidis . This fungus most commonly infects humans and animals through the respiratory tract. After spores are inhaled, they settle in the small airways and begin to reproduce. Subsequent to this, the organism spreads throughout the body to involve many organs. Infrequently, infection occurs through inoculation of an open wound. Although researchers in human medicine have been mostly unsuccessful in reliably isolating the organism from the environment, it does appear that both humans and animals become infected from particular environmental sources, probably the soil. In the United States, the disease is most prevalent in the warm, moist environment found in the Ohio and Mississippi River valleys. It is very common in the Southeastern United States.

    When you take your hunting dog into the field this autumn, you may be unknowingly exposing it to a deadly fungus that resides in the very ground your pooch is sniffing in order to trail game. Blastomycosis is a fungal disease that can affect many organ systems in your dog. The disease is caused by the fungus, Blastomyces dermatitidis , which lives in the soil. Dogs most commonly become infected by inhaling the organism. This means that dogs that spend a lot of time sniffing the ground in an area where the organism lives are at risk for infection. Geographically, areas with moist soil and standing waters, such as the Mississippi, Missouri, and Ohio River valleys, the Mid-Atlantic States, and the Canadian provinces of Quebec, Manitoba, and Ontario, have a high incidence of Blastomycosis. This includes areas along the Mississippi and Ohio Rivers and the Southeast. Although Blastomycosis can affect any dog of any age, breed, and gender, male dogs are more frequently infected than female dogs, and a greater percentage of female dogs survive treatment.

The treatment of choice for mild to moderate blastomycosis, both pulmonary and extrapulmonary forms, is itraconazole. Severe life-threatening pulmonary or dis-seminated infection should be treated with amphotericin B for a total of 1 to 2 grams or until the patient's condition has stabilized, at which point therapy can be changed to itraconazole. Although present in diverse geographic areas around the world, most cases occur in North America, primarily in the south-central and north-central areas of the United States. Blastomycosis
Definition: Blastomycosis is a rare fungal infection caused by inhaling a fungus ( Blastomyces dermatitidis ), which is found in wood and soil.
Alternative Names: North American blastomycosis; Gilchrist's disease
Causes, incidence, and risk factors: Blastomycosis occurs most often in people living in the south-central and midwestern US and Canada. Exposure to soil is the key risk factor. The disease usually affects people with compromised immune systems, such as people with HIV or organ transplant recipients. Men are more likely to be affected than women. Lung infection may produce no symptoms, but when the infection is widespread, skin lesions or bone lesions may appear and the urogenital system (bladder, kidney, prostate, testes) may be affected. The incidence of blastomycosis is 1-2 out of every 100,000 people in geographic areas where blastomycosis occurs most frequently. It is even less common outside those areas.

Blastomycosis is a rare infectious multisystem disease that is caused by the fungus Blastomyces dermatitidis. The symptoms vary greatly according the affected organ system. It is characterized by fever, chills, cough, and/or difficulty breathing (dyspnea). In the chronic phase of the disease, the lungs and skin are most frequently affected. The genitourinary tract and bones may also be involved.

Blastomycosis is a condition caused by the fungus Blastomyces dermatitidis , which most commonly affects the lungs, but can involve other parts of the body. Although it is an important fungal disease in parts of the midwestern and southern central United States, it is very rare in people with HIV and other immunocompromised people. It can be treated effectively with the anti-fungal drugs amphotericin, itraconazole and ketoconazole. Research
Pappas reviewed reports of blastomycosis in people with HIV in a retrospective survey, identifying only 15 cases. 12/15 had a previous or concomitant AIDS-defining illness; only one had a CD4 count above 200. Two patterns of disease emerged: localised pulmonary involvement (n=7) and disseminated or extrapulmonary blastomycosis (n=8). CNS involvement was common (40%). Among the 9 patients who survived longer than one month, all received amphotericin B as initial therapy and most received subsequent therapy with ketoconazole. References

Abstract Blastomycosis is an almost unheard of fungal infection in India. Chronic meningitis is the most common form of Blastomycosis. Presentation of Blastomycosis in the form of extra axial granulomas is very rare. We report the MRI findings in a case of extra axial intra dural blastomycosis involving the central nervous system. T1 and T2 weighted MRI along with contrast enhanced T1 weighted scan was done. Though the diagnosis in the Indian context is difficult, the characteristic appearance though not exclusive to the condition may provide a clue.
Introduction Blastomycosis is an exceedingly uncommon infection in India. It has however been sporadically reported from many parts of the world. Central Nervous system (CNS) involvement is a secondary manifestation of the disease process and is one of the rarest complications of the dimorphic fungus Blastomycosis dermatitidis. In CNS Blastomycosis an intracranial dural-based granulomatous lesion is infrequent in comparison to a finding of chronic meningitis. The MRI features of the lesion in this case were highly suggestive of a meningioma. Such a presentation is rare and to the best of our knowledge there is only a single case of a blastomycotic lesion mimicking a meningioma in literature.[1]

Introduction Endosonographic evaluation of the mediastinum has developed rapidly. EUS is particularly well suited to determine the etiology of masses identified in the posterior mediastinum. The following case report is an unusual presentation of pulmonary Blastomycosis diagnosed by EUS-FNA.
Methods for EUS Capture EUS was performed with the linear array Pentax FG36UX.
Case/Body Figure 1A Figure 1B Figure 2 Figure 3 A 46-year-old woman from northern Wisconsin presented with a non-productive cough and right posterior chest pain. She was initially seen by her primary care provider and was treated for bronchitis with antibiotics. Her symptoms did not improve. She denied weight loss. She denied hemoptysis. She had no upper gastrointestinal symptoms.
Her past medical history was notable for diabetes mellitus and hypothyroidism. Her past surgical history included a cholecystectomy and an appendectomy.

I had blasto 2 years ago and was told at that time that i could not get it again. However i have just been diagnosed again. I was wondering if anyone knew what the chances of a relapse are and why some of the symptoms i have now are different then last time. For example, I cough all the time now and i never did before. The body aches are not as bad and i'm not as fatigued yet. And on both occassions i have gotten them in my lungs. NJC-R.N.-DC
Blastomycosis is an infection caused by a fungus that is found in moist soil, which has been contaminated with animal waste. It is most common in the southeastern and south central United States, extending s up the Mississippi and Ohio River valleys into the north central United States and Canada. The spores of this fungus are light so they easily become airborne. Then they are inhaled into the airsacs of the lungs, causing breathing problems like the cough that you have. Other symptoms may include coughing up blood because of the irritation caused by the infection, fever, night sweats, shortness of breath, soreness of the chest, and fatigue. This fungus can also effect the skin. The fungus is found by looking at some of the sputum or a scraping of effected skin under a microscope. Some people recover without treatment. However relapse is common. Relapse is most likely to happen when there is a deficiency in the immune system. The symptoms may vary with each relapse.

Abstract Blastomycosis is a chronic fun gal disease that primarily affects the lower respiratory tract. The acute inflammatory phase of the primary pulmonary infection is characterized by a lymphohematogenous spread to extrapulmonary sites, especially the skin. The presence of disseminated infection with Blastomyces dermatitidis in the larynx is unusual. In areas of the United States where this fungus is endemic, failure to consider laryngeal involvement might lead to inappropriate therapy and thus worsening inflammation and airway compromise.

Histological Features: cetuaneous or mucosal lesions have a central ulcer
epithlelium shows a pseduoepithleiomatous hyperplasia
connective tissue is granulomatous consisting of histiocytes, lymphocytes, plasma cells and multinucleated giant cells
budding spores are found in areas of inflammation
Diagnosis: microscopic and culture appearances of the organism are important
Treatment:
oral itraconazole if uncomplicated lung infections and without meningeal lesion
