Jul. 26, 2005 12:00 AM Spend time outdoors in the summer - in the mountains, at the beach or in your yard - and you raise your risk of insect bites and stings. If you use caution, pests need not keep you indoors, says Phoenix osteopathic physician Art Mollen.
Question: What should people be most concerned about?
Answer: If you've had a serious reaction from bites or stings in the past, you need to be particularly cautious next time. For example, if a bee sting caused your hand to swell, you could have a greater sensitivity if you're stung again. advertisement OAS_AD('BoxAd')
Q: What are the signs of a serious reaction? A: With some people, it's not just swelling and itching at the site. They experience swelling of the lips, tongue, ears, eyelids, mouth, palms of their hands and soles of their feet. They have difficulty breathing. The most severe is an anaphylactic reaction; that's accompanied by hives, nausea, diarrhea and mental confusion. You should get to the emergency room as soon as possible. Q: How can you guard against these reactions?

For most people, a minor insect bite, or sting, may cause a little swelling and itching. The site may be painful for a few hours. Bees, wasps, hornets, fire ants, and yellow jackets are the worst offenders. Everyone reacts to the toxic effects of insect venom. Some people are much more sensitive. They may develop severe allergic reactions. Symptoms of insect bite allergy usually begin a few minutes after the sting or bite. Mildly sensitive people may have hives with intense itching and pain around the site as well as eye itching. Very sensitive people may develop severe hives and life threatening anaphylaxis. This condition causes:
* a tight throat,
* difficult breathing,
* nausea and vomiting,
* severe blood pressure drop,
* confusion, and
* severe anxiety. Seek help right away by calling 911. Immediate treatment with epinephrine is needed. Sometimes cardio-pulmonary resuscitation (CPR) is needed. Later treatment may involve other medications. Reactions that happen within minutes are more severe. Reactions that take up to a few days to develop are less severe. People who know they are sensitive should see a doctor after any sting, regardless of severity. Act fast if bitten by a black widow spider, brown recluse spider, scorpion, or if you have a violent toxic reaction. Many people are sensitive without knowing it. Once an insect bite or sting allergy is confirmed, prevention is the key. If you are sensitive, avoid walking barefoot when outdoors. Do not disturb bee hives or hornet nests. Keep garbage cans tightly covered to avoid attracting insects. Treat mild insect bites for pain with a paste of baking soda. Apply a cold, wet cloth, or ice cubes. Over-the-counter products, such as hydrocortisone cream or calamine lotion, reduce itching and swelling. If you have a severe insect allergy, always keep an Epi-pen or Ana-kit handy. Those who have severe allergies should learn what to do when bitten or stung. It is a good idea to teach co-workers and family members how to respond, as well.

Spiders
Many different types of spiders in Australia bite people. Many bites cause a reaction on the part of the body that is bitten. The only poisonous species known in Australia are the Red Back Spider and the Funnel Web Spider. Red Back Spider A stinging feeling will be followed with swelling and pain.
Basic treatment:
  • wash the area and keep it clean
  • seek medical advice if symptoms occur. Symptoms may include weakness, persistent pain, localised sweating, nausea, vomiting, pale colour.
Funnel Web Spider A bite can be very dangerous, especially to young children. There is usually extreme pain, lots of sweating, nausea and vomiting, difficulty in breathing and muscle twitching.
Basic treatment:
  • apply a very firm bandage around the bite and then bandage the whole limb to slow the flow of venom around the body
  • use a splint to hold the area of the bite still
  • take the person straight to the nearest hospital.

The two greatest risks from most insect stings and bites are allergic reaction (which may occasionally be fatal) and infection (more likely and less serious). What to Do:
Bee, Wasp, Hornet, and Yellow Jacket Stings
  • A bee will leave behind a stinger attached to a venom sac. Try to remove it as quickly as possible. One way is to gently scrape it out with a blunt-edged object, such as a credit card or a dull knife.
  • Wash the area carefully with soap and water. Do this two to three times a day until the skin is healed.
  • Apply a cold pack, an ice pack wrapped in a cloth, or a cold, wet washcloth for a few minutes.
  • Give acetaminophen for pain.
  • For pain and itching, give an over-the-counter oral antihistamine, if your child's doctor says it's OK; follow dosage instructions for your child's age and weight. You could also apply a corticosteroid cream or calamine lotion to the sting area.
  • A sting anywhere in the mouth warrants immediate medical attention. That's because stings in the mucous membranes of the mouth can quickly cause severe swelling that may block airways. You should seek medical care if you note a large skin rash, a large area of swelling around the sting site, or if swelling or pain persists for more than 72 hours. You should seek immediate medical care if you notice any of the following signs, which may indicate a serious or even potentially life-threatening allergic reaction:
    • wheezing or difficulty breathing

Wash well and treat any cut with an antiseptic such as povidone-iodine. Where possible avoid bandages and Band-aids, which can keep wounds wet. Coral cuts are notoriously slow to heal and if they are not adequately cleaned, small pieces of coral can become embedded in the wound. Clean any cut thoroughly with an antiseptic.
Bee and wasp stings are usually painful rather than dangerous. Calamine lotion or Stingose spray will give relief and ice packs will reduce the pain and swelling. There are some spiders with dangerous bites but antivenenes are usually available. Scorpion stings are notoriously painful and in Mexico can actually be fatal. Scorpions often shelter in shoes or clothing. Certain cone shells found in Australia and the Pacific can sting dangerously or even fatally. There are various fish and other sea creatures which can sting or bite dangerously or which are dangerous to eat. Take heed of local advice. Snakes To minimise your chances of being bitten always wear boots, socks and long trousers when walking through undergrowth where snakes may be present. Don't put your hands into holes and crevices, and be careful when collecting firewood. Snake bites do not cause instantaneous death and antivenenes are usually available. Keep the victim calm and still, wrap the bitten limb tightly, as you would for a sprained ankle, and then attach a splint to immobilise it. Then seek medical help, if possible with the dead snake for identification. Don't attempt to catch the snake if there is even a remote possibility of being bitten again. Tourniquets and sucking out the poison are now comprehensively discredited.

Pressure Immobilisation Technique
The pressure-immobilisation first aid technique was developed in the 1970's by Professor Struan Sutherland. Its purpose is to retard the movement of venom from the bite site into the circulation, thus "buying time" for the patient to reach medical care. Research with snake venom has shown that very little venom reaches the blood stream if firm pressure is applied over the bitten area and the limb is immobilised. Pressure-immobilisation was initially developed to treat snakebite, but it is also applicable to bites and stings by some other venomous creatures. It is currently recommended for most life threatening venomous bites and stings in Australia.
Pressure-immobilisation is recommended for:
  • all Australian venomous snakes, including sea snake bites funnel web spiders box jelly fish stings (if possible) bee, wasp and ant stings in allergic individuals

"Sunburn is still the most common and probably the most serious problem a beach-goer faces," says Glenn G. Soppe, M.D., a San Diego physician who lectures on aquatic bites and stings. Still, those murky depths hold more surprises than the contents of Davy Jones's locker. Minor fish bites should be handled with the usual first-aid treatment like any other wound, and they don't present any extra risk of infection. (There's no such thing as a rabid barracuda!) But what about stings from jellyfish and stingrays or cuts from coral, sponge and common seashells? These nautical nuisances may initially seem as frightening as losing your car keys in the sand, but they're usually remedied a lot faster. And here's how.
    When to See the Doctor
    Most marine bites, stings and cuts suffered by the average beach-goer are minor, but you should seek emergency medical care if you experience nausea, vomiting or intense swelling or if you have trouble breathing following your mishap, says Glenn G.
