March 20, 2005 I have not seen India TV’s much-discussed ‘sting’ operation involving Shakti Kapoor, a female journalist and a concealed camera. This is not because I don’t want to see it but because, like most people I know, I don’t get India TV at home and the few friends who do get the channel say they never watch it.
I do not know Shakti Kapoor or Aman Verma personally. I do know Suhaib Ilyasi, the host of the show on which the exposé appeared. In fact, two days before the first telecast, Suhaib phoned, told me about the Shakti Kapoor story and asked if I would comment on the issue.

What kinds of reactions do insect bites cause?
For most people, a minor insect bite or sting may cause a little swelling and itching. The site may be painful for a few hours. Mosquitoes, biting flies, and some spiders usually cause mild reactions. Yellow jackets, honeybees, paper wasps, hornets, and fire ants cause more severe sting reactions. Everyone reacts to the toxic effects of insect venom, for example, from a bee sting. Most people have mild reactions, which may appear in a few minutes or up to 48 hours after the bite or sting. Some people have insect allergies, which may cause mild to severe allergic reactions. Mildly allergic people may have hives with intense itching and pain around the site as well as eye itching. Others may have blisters where they were bitten. A severe allergic reaction to a bite will happen within minutes of the bite. The symptoms may include:

Every year, thousands of calls are made to the Poison Center regarding insect bite information or treatment advice. While all insects can bite or sting, some are more bothersome and dangerous than others. Bites from fleas, mosquitoes and the common horsefly can cause pain, itching and swelling at the site - an unpleasant experience but not necessarily dangerous, unless an infection occurs.
What is the immediate treatment for bites or stings?
If breathing difficulties, difficult swallowing and/or body-wide itching develop, the patient is having a severe allergic reaction. Immediately call 9-1-1 for assistance. Otherwise, wash the bite or sting area well with soap and water to help prevent infection. If stung or bitten on fingers or hand, remove any rings or jewelry in case of swelling. Your local pharmacist can help you select the best over-the-counter medications to help treat insect and spider bites.

Everyone reacts to the toxic effects of insect venom, but some people may develop a very severe allergic reaction. Most bites are not serious. Precautions can be taken to avoid getting stung. Most common sources of bites are: bees, spiders, ticks and mosquitoes. Symptoms of insect bite allergy usually begin a few minutes after the sting or bite. Mildly sensitive people may have hives with intense itching and pain around the site as well as eye itching. Very sensitive people may develop severe hives and life-threatening reactions. The symptoms may include:

This is the eighth article in a series written for Princeton Online on summer hazards. Click here for an archive of other articles Humans are hosts to numerous smaller organisms that may or may not welcome our participation in their environment. Insects, the most successful organism on earth, are often helpful to humans. They help us in agriculture and they keep each other in check. Unfortunately, they also like to check us out. For most of us, the sight of an insect “makes our skin crawl”. Most invasions of our skin are for purposes of feeding. These are often annoying and sometimes dangerous. Some insects sting to protect themselves. Knowing how to protect ourselves from bites and stings, how to treat them once they occur and how to distinguish dangerous from innocuous ones is important to our health. The annoying variety of insectsgnats, flies, mosquitoes, fleasis far more common than the potentially dangerous varietybees, some spiders, ticks. Routine bites of the annoying bugs cause local reactions at the site of the bite. Even a large reaction at the site of the bite is considered a local reaction, not an allergic reaction. This distinction is important because, as a rule, local reactions are not dangerous. A local reactionredness, swelling and itching begins within 1-3 days of a bite. It reach its pinnacle at about 48 hours later and subsides 2 to 3 days after the peak. Treatment is directed at alleviating discomfort, since the lesion resolves on its own with time. Cool compresses, topical 1% hydrocortisone cream, and topical agents such as Rhulli Gel®, Itch-X®, Caladryl®, calamine lotion and Aveeno Anti-itch Cream® may help. For multiple bites or excessive itching, an oral antihistamine, such as diphenhydramine (Benadryl®), hydroxyzine (Atarax®), cetirizine (Zyrtec®), cyproheptadine (Periactin®) or loratidine (Claritin®) may also help. Oral steroids are not used for minor bite reactions. Try to keep your child’s fingernails short and clean to avoid a secondary infection caused by scratching.

How to Protect Children From Insect Bites and Stings in the Wilderness
Young people may be more sensitive to insect repellents, and once bit, tend to scratch more, increasing the risk of infection. Help kids avoid the discomfort of bites and the trauma of stings. Steps: Apply an insect repellent regularly to the child's skin if in an area where bites and stings are a risk. Use a repellent with the lowest possible concentration of DEET or use a natural repellent without DEET. Dress the child in dark clothing thick enough that insects can't bite through it. Teach the child to be respectful of bug's homes: this may mean not turning over rocks and logs (or doing so carefully and only with adult supervision), and not disturbing spider webs. Teach the child to shake out shoes and clothing before putting them on, especially if they have been sitting outdoors. Teach the child to freeze ("play statue") when he or she is being investigated by stinging insects so that the child doesn't frighten the insect into stinging.

How to Protect Adults From Insect Bites and Stings in the Wilderness
Insect bites can be annoying, and some bites and stings can be dangerous. The key to prevention is to be alert when in insect habitats and to minimize contact. Steps: Wear an insect repellent containing DEET on all exposed areas of the body. Wear dark clothing thick enough that bugs can't bite through it. Make sure that your arms and legs are completely covered. Look carefully before picking up stones or logs where insects might be hiding, and exercise caution when reaching under ledges. Shake out shoes and clothing before putting them on, especially if they have been sitting outdoors. Unzip and shake out sleeping bags before climbing into them. Zip tent screens shut, and open screens only when entering and exiting the tent. Wear a hood of mosquito netting if bugs are particularly persistent.

FIRST AID FOR INSECT STINGS
Take immediate action.  If the stinger remains, remove it by scraping with a credit card or thumbnail.  Squeezing the stinger may force more venom into the skin. Some commercial products, such as StingEze®, are very effective if applied quickly after the sting occurs.  A paste of meat tenderizer and water may also provide relief.  Pain and swelling are normal reactions to stings.  Apply ice to relieve pain and swelling.
More serious symptoms, such as shortness of breath, nausea, or difficulty swallowing require immediate medical attention

12.1 Bites by dogs and other large mammals
Bites by dogs are common in all parts of the world. They may cause mechanical damage, including soft tissue injury, avulsion of nerves and tendons, compound fractures, and, rarely, death. They may also be complicated by a range of bacterial infections including tetanus. Some infections are peculiar to animal bites (eg Pasteurella multocida and rabies). Bites may also be inflicted by domestic cats and monkeys, and less commonly by horses, rodents, bats and even large carnivores. Infection may occasionally be introduced through scratches and licks over broken skin.
12.1.1 Treatment
Animal bites should not be ignored. Travellers should be advised to:
  • clean the wound thoroughly as soon as possible with soap/detergent and water (preferably under a running tap) apply an antiseptic such as iodine or 40-70 per cent alcohol (gin, whisky and vodka contain about 40 per cent)

A 6 year old female was playing in the yard when she suddenly felt pain in her left ankle. Her ankle became swollen, red and painful, so her mother took her to the pediatrician's office. Upon close examination there is a central puncture lesion with a small amount of serosanguineous drainage. Her vital signs are normal. No other examination findings are present What are the potential causes for this lesion? What current treatment is needed? What advice should be given to the child and parents? Another health problem associated with insect bites is the potential to transmit disease. An outbreak of dengue fever in Hawaii (2001) by mosquitoes demonstrated the disease carrying ability of insects as vectors. Another noteworthy vector in Hawaii is the flea and its ability to transmit the plague. Fortunately Hawaii does not have ticks and fleas that carry Lyme disease and Rocky Mountain spotted fever. Envenomation occurs from such arthropods such as spiders and centipedes in Hawaii. A centipede's bites cause intense localized pain, swelling, and occasionally infection and local tissue necrosis. A few of the spiders in Hawaii contain venom in their bite that can lead to serious complications. Two of the more dangerous spiders are discussed in greater detail below.

Most bites and stings result in a localised itch and swelling that settles within a few days. Severe allergic reactions are relatively uncommon. When they do occur, however, bees, wasps and the Australian "Jack Jumper ant" are usually responsible. Fortunately, effective treatments are available to treat allergic reactions.
Reactions to bites are only sometimes serious

  • Mosquitoes and "march flies" can cause nasty itchy bites. Serious allergic reactions are very rare, even when the swellings are very large and uncomfortable.
    Caterpillars can cause severe irritation from touching their spines, which are attached to venom sacs underneath the skin.
    Ticks also bite, and occasional life-threatening reactions have been reported.
    Anaphylaxis following snake bites have also been reported, although these are very rare
    Bull ants can occasionally cause anaphylaxis. The major cause of anaphylaxis from ant stings is the Australian "Jack Jumper" ant.
Stinging insects are a common cause of serious allergic reactions
Allergies to venoms from stinging insects are one of the most common causes of serious allergic reactions (anaphylaxis) in Australia. Symptoms include an all-over rash, swelling of tongue or throat, trouble breathing, gut cramps, diarrhoea, vomiting or even a drop in blood pressure (shock). Although these insects are all "hymenoptera" (which means membranous winged insects), their venoms are very different. Allergy to one does not usually increase the risk of reaction to another.

Materia Medica APIS (2*). This remedy is indicated when a Many feel that this remedy is a specific for bee stings as it is made from the honey bee. This remedy has been used for anaphylactic shock from bee and other stings. It is useful in the stings of jelly fish and centipedes. The individual may become drowsy, confused, and apathetic. They are also thirstless. ARSENICUM (2). Indicated when a bite has become infected by dead tissue or animal substances have remained in the wound causing sepsis. The lesion takes on a dark, black, unhealthy look and moves toward gangrene. The patient becomes restless anxious, fussy and is prostrated. It is useful in poisonous spider bites that take of a blackish hue. BEllADONNA (2). This remedy is useful for bites and stings where there is rapid swelling and violent symptoms. There is redness, heat, throbbing, and burning, or redness spreading out in streaks. The face is flushed, eyes glaring, pupils dilated, the body fevered, and an excited mental state. There may even be delirium where the individual becomes furious, rages, and bites and strikes.
