What is bipolar disorder? Bipolar disorder, which is also known as manic-depressive illness, is a mental illness involving episodes of serious mania and depression. The person's mood usually swings from overly "high" and irritable to sad and hopeless and then back again, with periods of normal mood in between. Bipolar disorder typically begins in adolescence or early adulthood and continues throughout life. It is often not recognized as an illness, and people who have it may suffer needlessly for years or even decades. Effective treatments are available that greatly alleviate the suffering caused by bipolar disorder and can usually prevent its devastating complications. These include marital break-ups, job loss, alcohol and drug abuse, and suicide. ( From the National Institute of Mental Health (NIMH) Almost one-third of 6-to-12-year-old children diagnosed with major depression will develop bipolar disorders within a few years, according to a study of 79 depressed children over 2 to 5 years reported in the May 1994 issue of the Journal of the American Academy of Child and Adolescent Psychiatry . Most became bipolar before the onset of puberty, and 32% became bipolar at an average age of 11 years. The researchers recommend that those caring for depressed children should be on the lookout for symptoms of manic-depressive illness. They also suggested that, because antidepressants may worsen certain forms of manic illness in adults, prescribing antidepressants for children should be done with care. (

Many children and adults with AD/HD also experience other difficulties. It seems that having AD/HD makes it more likely that an individual will also have other difficulties. If a child meets criteria for AD/HD, it may not be enough to prescribe a stimulant get a few checklists and do follow up twice a year. There has been increasing awareness that many adults and children with AD/HD may also meet criteria for one or more other psychiatric diagnoses. (Comorbidity means having two or more diagnosable conditions at the same time) There is some evidence that the incidence of comorbidity is somewhat higher in adults than in children. However, many of the studies looking at the issue of comorbidity are difficult to compare. Studies used different criteria for AD/HD and bipolar disorder, and sometimes got their subjects from different populations. For example, one might expect to see more complex types of AD/HD in specialized hospital clinics than one would see in a door-to-door survey or in a primary care physician’s office. Despite the differing criteria across studies, and the lack of large general population studies of adult AD/HD, there still convincing data that several other psychiatric diagnoses are common among children and adults with AD/HD.

Childhood Schizophrenia and Bipolar Disorder
A child's stage of development must be taken into account when considering a diagnosis of mental illness. Behaviors that are normal at one age, may not be at another. Rarely, a normal young child may report strange experiences—such as hearing voices—that would be considered abnormal at a later age. Clinicians look for a more persistent pattern of such behaviors. Parents may have reason for concern if a child of 7 years or older often hears voices saying derogatory things about him or her, or voices conversing with one another, talks to himself or herself, stares at scary things—snakes, spiders, shadows—that aren't really there, and shows no interest in friendships. Such behaviors could be signs of schizophrenia, a chronic and disabling form of mental illness. What is bipolar disorder?

Bipolar disorder is usually diagnosed after a person has one or more manic episodes. People who have the classic form of bipolar disorder experience alternating periods of depressed moods and periods of manic or excited moods. This condition is sometimes referred to as "mood swings" or manic depressive disorder. Other people with bipolar disorder have episodes of a manic mood without episodes of depression. Still others with bipolar disorder have a mixture of depression and mania, a state of hyperactivity, at the same time.
There are different types of bipolar disorder. The type depends whether the recent mood was elevated, depressed, or mixed.
What is a manic episode?
Some of the characteristics of mania appear as opposites of depression. Rather than a general slowing down of thought and activity, which is very common in depression, the person with mania experiences a speeding up of thought and activity. Also, with a manic episode the person's self-esteem and mood are elevated, which is unlike what happens in depression. A person experiencing a manic episode frequently encounters difficulty with relationships and problems at work, at school, or with the law.
There is a milder form of mania which is called hypomania. The person who is hypomanic experiences speeded up speech, thought, and behaviors, but usually functions normally.

Research findings, clinical experience, and family accounts provide substantial evidence that bipolar disorder, also called manic-depressive illness, can occur in children and adolescents. Bipolar disorder is difficult to recognize and diagnose in youth, however, because it does not fit precisely the symptom criteria established for adults, and because its symptoms can resemble or co-occur with those of other common childhood-onset mental disorders. In addition, symptoms of bipolar disorder may be initially mistaken for normal emotions and behaviors of children and adolescents. But unlike normal mood changes, bipolar disorder significantly impairs functioning in school, with peers, and at home with family. Better understanding of the diagnosis and treatment of bipolar disorder in youth is urgently needed. In pursuit of this goal, the National Institute of Mental Health (NIMH) is conducting and supporting research on child and adolescent bipolar disorder.

Introduction to the Conference
For more than a decade after the introduction of lithium pharmacotherapy, manic-depressive illness was thought of as a problem solved. Recently, however, both naturalistic studies and controlled trials have indicated that we still have much to learn about this puzzling and destructive disorder. In June 1994, the University of Pittsburgh Medical Center's Western Psychiatric Institute and Clinic sponsored the First International Conference on Bipolar Disorder , which attracted more than 400 participants. The conference provided an opportunity for researchers and clinicians to meet and share new information on the neurobiology and pathophysiology of bipolar illness, psychobiology and psychosocial vulnerability factors, and recent findings on epidemiology and clinical course. The Second International Conference on Bipolar Disorder , planned for June 1997, will continue the work begun at the first conference. the second conference will present an update on etiologic factors and treatment methods, including current information on the genetics of bipolar disorder as well as both biological and psychotherapeutic modalities for the acute and maintenance treatment of bipolar disorder. An increasing array of treatment options is becoming available for this disorder, and the roles and significance of these new therapies will be discussed. Top scientists and clinicians from around the world will be available to entertain questions from the audience following presentations and in panels. The conference will also feature " Ask the Experts" question-and answer sessions on June 19 and 20, 1997.
