Management of Bipolar Disorder

A patient information handout on bipolar disorder, written by the authors of this article, is provided on page 1357. This article exemplifies the AAFP 2000 Annual Clinical Focus on mental health. Bipolar disorder most commonly is diagnosed in persons between 18 and 24 years of age. The clinical presentations of this disorder are broad and include mania, hypomania and psychosis. Frequently associated comorbid conditions include substance abuse and anxiety disorders. Patients with acute mania must be evaluated urgently. Effective mood stabilizers include lithium, valproic acid and carbamazepine. A comprehensive management program, including collaboration between the patient's family physician and psychiatrist, should be implemented to optimize medical care. (Am Fam Physician 2000;62:1343-53, 1357-8.) B ipolar disorder is characterized by variations in mood, from elation and/or irritability to depression. This disorder can cause major disruptions in family, social and occupational life. Bipolar I disorder is defined as episodes of full mania alternating with episodes of major depression. Patients with mania often exhibit disregard for danger and engage in high-risk behaviors such as promiscuous sexual activity, increased spending, violence, substance abuse and driving while intoxicated.

What is bipolar disorder?
Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in a person's mood, energy, and ability to function. Different from the normal ups and downs that everyone goes through, the symptoms of bipolar disorder are severe. They can result in damaged relationships, poor job or school performance, and even suicide. But there is good news: bipolar disorder can be treated, and people with this illness can lead full and productive lives. More than 2 million American adults, or about 1 percent of the population age 18 and older in any given year, have bipolar disorder. Bipolar disorder typically develops in late adolescence or early adulthood. However, some people have their first symptoms during childhood, and some develop them late in life. It is often not recognized as an illness, and people may suffer for years before it is properly diagnosed and treated. Like diabetes or heart disease, bipolar disorder is a long-term illness that must be carefully managed throughout a person's life.

Tempering the Mania of Manic Depression

Bipolar Disorder - episodes of mania.
Any discussion of the process of bipolar disorder and mania cannot be entirely divorced from a discussion of the depressive response. Within any episode of mania lurks the possibility, the likelihood of impending depression. There are certain techniques and exercises which I have identified while dealing with my own periods of bipolar disorder mania which help temper the mania and thereby lessen the impact and severity of the subsequent depressive response. It is essential that you have read my article on Emotional Thought Stopping and are familiar with the concept and my suggestions for its use, before you attempt this exercise. I assume that if you are reading this in order to help temper your periods of bipolar disorder mania that this is not your first episode. Look back at your previous situation just prior to those episodes and see if you can identify if there was some event which triggered the episode and begin to identify how you felt at the start of the episode. It is important to be able to recognize these periods as early as possible so that you can begin this tempering exercise before it reaches a crisis stage. This is especially true if you begin your episodes with a euphoric feeling.

Facts Psychologists Know About BIPOLAR DISORDER What is Bipolar Disorder? Bipolar is another name for manic/depression. A bipolar mood disturbance involves a fluctuation of at least two mood states, for a prolonged period of time (at least 4 days). It is important to note that mood changes associated with Bipolar Disorder are more significant than changing moods that are related to sad and happy events that happen in our lives. Bipolar Disorder is also different from an individual that is "sensitive" and may have shifting moods throughout the day. Also, a Bipolar Disorder should never be diagnosed unless the mood shifts are causing significant impairment in an individual's daily life or the mood shifts are extremely apparent to others. There are different types of Bipolar Disorders Bipolar I is the name given to a person's mental illness when they experience a manic episode and depressive episodes (described latter). A manic episode lasts for one week or more and includes a combination of the following components; usually at least four or five of these symptoms are present: grandiose thoughts and/or very high self-esteem (e.g. "nothing can stop me")

Bipolar disorder, or manic-depressive illness, is characterized by moods that swing between two opposite poles:
