Bipolar Disorder
(Mania)
What is bipolar disorder?
Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in a person's mood, energy, and ability to function. Different from the normal ups and downs that everyone goes through, the symptoms of bipolar disorder are severe. They can result in damaged relationships, poor job or school performance, and even suicide. But there is good news: bipolar disorder can be treated, and people with this illness can lead full and productive lives. More than 2 million American adults, or about 1 percent of the population age 18 and older in any given year, have bipolar disorder. Bipolar disorder typically develops in late adolescence or early adulthood. However, some people have their first symptoms during childhood, and some develop them late in life. It is often not recognized as an illness, and people may suffer for years before it is properly diagnosed and treated. Like diabetes or heart disease, bipolar disorder is a long-term illness that must be carefully managed throughout a person's life.

What is Bipolar Disorder or Manic-Depression?
Bipolar Disorder is a serious mental illness that affects a child's or adolescent’s mood and behavior. Young people with Bipolar Disorder have dramatic changes in their moods, alternating between feeling very depressed and feeling high or manic. They may have more normal moods between these episodes. The periods of depression or mania can last for days, weeks or even months. Because of the depression and mania, Bipolar Disorder is also called Manic-Depression. At this site, we will use the term Bipolar. In other places, you may see the term Manic-Depression used.

Research findings, clinical experience, and family accounts provide substantial evidence that bipolar disorder, also called manic-depressive illness, can occur in children and adolescents. Bipolar disorder is difficult to recognize and diagnose in youth, however, because it does not fit precisely the symptom criteria established for adults, and because its symptoms can resemble or co-occur with those of other common childhood-onset mental disorders. In addition, symptoms of bipolar disorder may be initially mistaken for normal emotions and behaviors of children and adolescents. But unlike normal mood changes, bipolar disorder significantly impairs functioning in school, with peers, and at home with family. Better understanding of the diagnosis and treatment of bipolar disorder in youth is urgently needed. In pursuit of this goal, the National Institute of Mental Health (NIMH) is conducting and supporting research on child and adolescent bipolar disorder.

Childhood severe mental illness Many children and especially adolescents experience mood swings as a normal part of growing up, but when these feelings persist and begin to interfere with a child's ability to function in daily life, bipolar disorder, sometimes called manic depression, could be the cause. Bipolar disorder is a type of mood disorder marked by extreme changes in mood, energy levels and behaviour.
causes of bipolar disorder in children The illness tends to be highly genetic in origin, but there are clearly environmental factors that influence whether the illness will occur in a particular child. Bipolar disorder can skip generations and take different forms in different individuals. For the general population, there is a 1% risk of having bipolar disorder. When one parent has bipolar disorder, the risk to each child is 15-30%. When both parents have bipolar disorder, the risk increased to 50-75%. The risk of siblings and fraternal twins is 15-25%. The risk in identical twins is approximately 70%.
symptoms of bipolar disorder Symptoms can begin in early childhood but more typically emerge in adolescence or adulthood. Research by the American Academy of Child and Adolescent Psychiatry shows that up to 1/3 of 3.4 million children and adolescents with depression in the United States may actually be experiencing the early onset of bipolar disorder.

How to deal with the bipolar conduct disorder in children. What treatments are available; what are the symptoms?
Bipolar disorder, also known as manic-depression, is a disorder of the brain marked by extreme mood swings, rise or fall in activity levels, and erratic behavioral changes. Most children with this disorder will go from a nearly euphoric high mood (manic) to a deeply depressed pessimistic and sometimes even suicidal mood (Depressive). Children have a rougher time dealing with this disorder because they don't understand why they feel the way they do. Sometimes before they are diagnosed, the child may be labeled in school as a troublemaker or the child is so withdrawn that they have lost interest in school activities, and have failed to make friends. With Bipolar disorder, understanding what the child is going through is the most important key to treatment. Bipolar is a serious condition, and although there is no cure, it's treatable with medication. Symptoms can start showing up as early as infancy. Bipolar disorder is different in children than it is in adults. Children usually have continuous mood swings that will go back and forth from mania to depression. These quickly changing moods produce chronic irritability. There may be times where the child will seem normal and display no mood swings or any other symptoms of Bipolar at all, only to have the symptoms come back at another time.

Behavior/Mental Health - Bipolar Disorder Overview Depressive and manic depressive disorders are two major types of depressive illness, also known as mood disorders or affective disorders. Individuals with bipolar disorder experience mood swings between periods of depression and mania (manic depressive disorder).
Genetic, biochemical, and environmental factors play a role in the onset and progression of depressive and manic depressive disorders. While all of us experience highs and lows from time to time in our lives, individuals with bipolar disorder experience swings in their moods that are characterized by extremes in intensity and duration.
The symptoms and impact of bipolar disorder varies widely between individuals. Some individuals have such a severe case that it can completely disable them. In severe cases, medications alone may not consistently do the trick. Periodically these individuals require hospitalization to control symptoms and be stabilized. They often require regular adjustments in their medication as psychotic symptoms come up during the manic phase.
Individuals with bipolar disorder can be a danger to themselves or others [violent behavior] during manic episodes or during those with psychotic manifestations.

Introduction Bipolar Disorder is characterized by intense, persistent mood swings between the poles of depression and mania. These moods are greatly intensified or clearly different from the youngster's usual personality and are far out of proportion to events in the environment in intensity and/or duration. To meet the criteria for this diagnosis, a person must have experienced the typical signs of depression - helplessness, hopelessness and worthlessness - as well as the signs of mania - grandiosity and exuberance. The disorder may be genetic and caused by a chemical imbalance in the brain. Medication is extremely successful in moderating the symptoms. Back to top.

Description Although rare in young children, bipolar disorder , or manic-depressive illness , can appear in both children and adolescents. Bipolar disorder, which involves unusual shifts in mood, energy, and functioning, may begin with either manic, depressive, or mixed manic and depressive symptoms. It is more likely to affect the children of parents who have the disorder. 20 to 40% of adolescents with major depression develop bipolar disorder within 5 years after depression onset.
Existing evidence indicates that bipolar disorder beginning in childhood or early adolescence may be a different, possibly more severe form of the illness than older adolescent- and adult-onset bipolar disorder. When the illness begins before or soon after puberty, it is often characterised by a continuous, rapid-cycling, irritable, and mixed symptom state that may co-occur with disruptive behaviour disorders, particularly attention deficit hyperactivity disorder (ADHD) or conduct disorder , or may have features of these disorders as initial symptoms.

Similarities
Both disorders share many characteristics: impulsivity, inattention, hyperactivity, physical energy, behavioral and emotional lability (behavior and emotions change frequently), frequent coexistence of conduct disorder and oppositional-defiant disorder, and learning problems. Motor restlessness during sleep may be seen in both (children who are bipolar are physically restless at night when "high or manic",though they may have little physical motion during sleep when "low or depressed"). Family histories in both conditions often include mood disorder. Psychostimulants or antidepressants can help in both disorders (that is, depending on the phase of the bipolar disorder). In view of the similarities, it is not surprising that the disorders are hard to tell apart.
Differences
So what features can help in distinguishing these two disorders? Some distinctions are obvious. 1. Destructiveness may be seen in both disorders but differs in origin. Children who are ADHD often break things carelessly while playing ("non-angry destructiveness"), whereas the major destructiveness of children who are bipolar is not a result of carelessness, but tends to occur in anger. Children who are bipolar may exhibit severe temper tantrums, during which they release manic quantities of physical and emotional energy, sometimes with violence and property destruction. 2. The duration and intensity of angry outbursts and temper tantrums in the two disorders differs. Children who are ADHD usually calm down within 20-30 minutes, whereas children who are bipolar may continue to feel and act angry for over 30 minutes and even for 2-4 hours. The physical energy that a child with ADHD "puts out" during an outburst of anger could be mimicked by an adult who tries to "enact" the tantrum, whereas the energy generated by angry children who are bipolar could not be imitated by most adults without reaching exhaustion within a few minutes.

Bipolar disorders are one of several medical conditions called depressive disorders. Depressive disorders affect the way a person's brain functions. Depressive disorders are widespread. In the United States alone, it's estimated that more than 17.4 million adults have a depressive disorder each year. That works out to about one out of every seven people, so there's a good chance that you or someone you know is dealing with a depressive disorder. Out of the total number of people with depressive disorders, it's thought that about 1% to 2% have bipolar disorder - a relatively small group. What Is Bipolar Disorder?
Bipolar disorder goes by many names: manic depression, manic-depressive disorder, manic-depressive illness, bipolar mood disorder, and bipolar affective disorder are other medical terms for the same condition. Bipolar disorder is classified into four different types. These are Bipolar I, Bipolar II, Cyclothymic Disorder, or Bipolar Disorder Not Otherwise Specified. Mental health experts separate the condition into these four types because the symptoms of bipolar disorder show up differently in different people. When doctors know what type a person has, they can tailor treatment to that person's specific needs. Bipolar disorder affects both men and women. For many people, the first symptoms show up in their early twenties. However, research has shown that the first episode of bipolar disorder is occurring earlier: The condition often shows up in adolescence, and even children can have the disorder.
