Is there an epidemic of bipolar disorder in children and adolescents in the US? While good, population-based statistics that document the prevalence of pediatric bipolar disorder are lacking, every child mental health professional confronts the problem on a regular basis. A cursory search readily reveals more than a dozen books in the lay press dealing with early onset bipolar disorder, following the widely read descriptive book, The Bipolar Child. Clearly, in less than a decade what used to be an extremely rare illness in children and adolescents is now being diagnosed and treated with great regularity.
The change in practice is astonishing to those of us whose professional careers have encompassed it. Even in the 1980's, bipolar disorder was not considered a childhood illness any more than was Alzheimer's disease. Our diagnostic Bible, the DSM III, essentially defined away the possibility of bipolar disorder occurring in prepubertal children and no one looked for it; not surprisingly, it wasn't found. A correction was needed, and the pendulum certainly has swung.
Today reasonable estimates place the prevalence of bipolar disorder in adolescents at about 1% and in children, at about 0.3%. The most common age group for the initial diagnosis is between 20 - 30 years, but the second highest incidence occurs between ages 15 - 19 years. Once investigators started systematically asking adults with bipolar disorder when they had the onset of their first symptoms, it became clear that 20 - 30% were symptomatic during childhood or adolescence.

What is Bipolar Illness?
Table Of Contents The bipolar disorders are mood disorders. That means that amongst other things, there is a major change in mood. In bipolar disorders, this change in mood can be down, as in depression, or the opposite, mania. That is, a person can be inappropriately up. Some types of bipolar disorder have a lot of depression and only a little mania. Others have half and half. Still others seem to be both manic and depressed at the same time. Some people with bipolar disorders only have a few cycles of depression and mania. Others have many cycles a year. When bipolar illness is present in children and adolescents, it is more severe and harder to treat than when it occurs in adults. Pediatric Bipolar illness is one of the most severe conditions in pediatrics. In the milder forms, it can be disabling. In the severe forms, it can be lethal. The prognosis for cancers in pediatrics is better than many forms of bipolar illness. All bipolar disorders are a combination of mania with or without depression. So what is mania? It's hard to believe that just about a year ago this site went live on the World Wide Web. We want to thank all of you who've visited and provided important feedback. Your responses to the July newsletter on night-terrors, in particular, indicated that many children are suffering from these parasomnias and helped us assemble invaluable data. You've enriched our knowledge base about children with early-onset bipolar disorder. Your comments on the site also allow us to note emerging trends in prescribing practices. One of the most recent we've spotted is the treatment of bipolar disorder in children and adolescents without mood stabilizers, but rather with one of the newer, "atypical" antipsychotic drugs such as olanzapine (Zyprexa), quetiapine (Seroquel), or risperidone (Risperdal), often as a stand-alone therapy. In other words, the only medication prescribed is one of these three newer antipsychotic drugs, whose properties include antimanic effects, and so-far, unproved mood-stabilization.

    Many children and especially adolescents experience mood swings as a normal part of growing up, but when these feelings persist and begin to interfere with a child’s ability to function in daily life, bipolar disorder could be the cause.  Bipolar disorder, also known as manic-depression, is a type of mood disorder marked by extreme changes in mood, energy levels and behavior.  Symptoms can begin in early childhood but more typically emerge in adolescence or adulthood.  Until recently, young people were rarely diagnosed with this disorder.  Yet up to one-third of the 3.4 million children and adolescents with depression in the United States may actually be experiencing the early onset of bipolar disorder according to the American Academy of Child and Adolescent Psychiatry.  Doctors now recognize and treat the disorder in both children and adolescents, but it is still an under-recognized illness.
    Children with bipolar disorder usually alternate rapidly between extremely high moods (mania) and low moods (depression).  These rapid mood shifts can produce irritability with periods of wellness between episodes, or the young person may feel both extremes at the same time.  Parents who have children with the disorder often describe them as unpredictable, alternating between aggressive or silly and withdrawn.  Children with bipolar disorder are at a greater risk for anxiety disorders and Attention-Deficit Hyperactivity Disorder (ADHD).  These "co-occurring" disorders complicate diagnosis of bipolar disorder and contribute to the lack of recognition of the illness in children.

    Bipolar disorder, also known as manic depression, is an illness involving one or more episodes of serious mania and depression. The illness causes a person’s mood to swing from excessively “high” and/or irritable to sad and hopeless, with periods of a normal mood in between. More than 2 million Americans suffer from bipolar disorder. Bipolar disorder typically begins in adolescence or early adulthood and continues throughout life. It is often not recognized as an illness and people who have it may suffer needlessly for years. Bipolar disorder can be extremely distressing and disruptive for those who have this disease, their spouses, family members, friends and employers. Although there is no known cure, bipolar disorder is treatable, and recovery is possible. Individuals with bipolar disorder have successful relationships and meaningful jobs. A patient information handout on bipolar disorder, written by the authors of this article, is provided on page 1357. This article exemplifies the AAFP 2000 Annual Clinical Focus on mental health. Bipolar disorder most commonly is diagnosed in persons between 18 and 24 years of age. The clinical presentations of this disorder are broad and include mania, hypomania and psychosis. Frequently associated comorbid conditions include substance abuse and anxiety disorders. Patients with acute mania must be evaluated urgently. Effective mood stabilizers include lithium, valproic acid and carbamazepine. A comprehensive management program, including collaboration between the patient's family physician and psychiatrist, should be implemented to optimize medical care. (Am Fam Physician 2000;62:1343-53, 1357-8.) B ipolar disorder is characterized by variations in mood, from elation and/or irritability to depression. This disorder can cause major disruptions in family, social and occupational life. Bipolar I disorder is defined as episodes of full mania alternating with episodes of major depression. Patients with mania often exhibit disregard for danger and engage in high-risk behaviors such as promiscuous sexual activity, increased spending, violence, substance abuse and driving while intoxicated.

Bipolar disorder (previously known as manic-depression) has been referred to by a variety of terms in child/adolescent psychiatry including pediatric, juvenile, early-onset, childhood and prepubescent bipolar disorder. As the American Psychiatric Association's "Diagnostic and Statistical Manual of Mental Disorders " 4 th edition (DSM-IV) describes four types of bipolar diagnosis, we prefer the term early-onset bipolar spectrum disorders (EOBPSD) to refer to this group of disorders. The number of children with EOBPSD is not presently known, however, studies estimate 1-6% (3-18 million) adolescents in the U.S. suffer from symptoms of manic-depressive illness.
