Early-onset bipolar disorder is manic-depression that appears earlyvery earlyin life. For many years it was assumed that children could not suffer the mood swings of mania or depression, but researchers are now reporting that bipolar disorder (or early temperamental features of it) can occur in very young children, and that it is much more common that previously thought. Is bipolar disorder in children the same thing as bipolar disorder in adults? Adults seem to experience abnormally intense moods for weeks or months at a time, but children appear to experience such rapid shifts of mood that they commonly cycle many times within the day. Facts About Childhood-Onset Bipolar Disorder
What is childhood-onset bipolar disorder (COBPD), and how does it differ from bipolar disorder (manic-depression) in adults? All those with bipolar disorder experience mood swings that alternate from periods of severe highs (mania) to severe lows (depression). However, while these abnormally intense moods usually last for weeks or months in adults with the illness, children with bipolar disorder can experience such rapid mood swings that they commonly cycle many times within a day. The most typical pattern of cycling among those with COBPD, called ultra-ultra rapid or ultradian, is most often associated with low arousal states in the mornings followed by increases in energy towards late afternoon or evening. It is not uncommon for the initial episode of COBPD to present itself as major depression. But as clinical investigators have followed the course of the disorder in children, they have observed a significant rate of transition from depression into bipolar mood states.

Bipolar Disorder in Children Appears More Severe Than in Most Adults "Typically adults with bipolar disorder have episodes of either mania or depression that last a few months and have relatively normal functioning between episodes," said Barbara Geller, M.D., the study’s principal investigator. "But in manic children we have found a more severe, chronic course of illness. Many children will be both manic and depressed at the same time, will often stay ill for years without intervening well periods and will frequently have multiple daily cycles of highs and lows. These findings are counterintuitive to the common notion that children would be less ill than their adult counterparts." Geller and colleagues are studying 93 children with bipolar disorder and comparing them to 81 children who have attention deficit hyperactivity disorder (ADHD) and another 94 healthy children from the community. "In particular, we want to distinguish between children with bipolar disorder and ADHD because many parents, teachers and health care providers might confuse the overlapping symptoms of the two problems and think that these are just hyperactive kids," said Geller, who is a professor of child psychiatry at Washington University School of Medicine in St. Louis.

There has been much controversy among professionals about diagnosing Children with bipolar disorder.(1) But the National Institute of Mental Health (NIMH) reports that there is substantial evidence that it can and does occur in children and adolescents.(2) Due to the controversy and the lack of information about children with bipolar disorder, many times children are misdiagnosed.(3) One of the problems with diagnosing children with bipolar disorder is that often the symptoms in children are not exactly the same as symptoms of adults with bipolar disorder. The symptoms of bipolar in children may also appear like the symptoms of other disorders. (2) Many times bipolar disorder may co-occur with other disorders such as attention deficit hyperactivity disorder (ADHD) or conduct disorder.(4) This may really confuse things in making a diagnosis. The treatment plan for bipolar disorder in children is based on the treatment for bipolar in adults.(2) There has not been much research done on the effectiveness of treatment of bipolar in children. There is a great need for research and studies on children with bipolar, especially in the area of treatment and how effective these treatments are. There are however some studies of bipolar in children that is presently going on. (2)
Usually the treatment consists of a combination of medication therapy and psychotherapy.(5) Lithium and/or valproate are the medications most prescribed by doctors for bipolar disorder. It has proven to be effective for treating the symptoms of mania in adults with bipolar disorder. Lithium has also been effective in limiting the chances of a reoccurrence of manic episodes. However there is very limited data on the effectiveness of lithium on children with bipolar disorder and research is ongoing.(6) The National Institute of Mental Health (NIMH) is currently funding multi-site studies to determine the effectiveness of lithium and other mood stabilizers on adolescents.(2)
