What is beriberi?
Beriberi is condition which results from a deficiency of the vitamin thiamine.
What causes beriberi?
Beriberi is caused by a deficiency of thiamine. This vitamin is synthesised by a variety of plants and microorganisms but not normally by animals. Small amounts may be synthesised by microorganisms in the gut. About 5mg of thiamine a day can be absorbed by the gut. About 25 - 30mg is stored in the body. Large amounts are present in skeletal muscles (about one half of body stores), heart, liver, kidneys and brain. The recommended daily allowance of thiamine in men is between 1.2 - 1.5mg and in women between 1 - 1.1mg.

Beriberi is a vitamin deficiency disease, caused by inadequate bodily stores of thiamine , (Vitamin B1). It can damage the heart and nervous system.

Causes, incidence, and risk factors: There are two major manifestations of thiamine deficiency: cardiovascular disease (wet beriberi) and nervous system disease ("dry beriberi" and Wernicke-Korsakoff syndrome). Dry beriberi is somewhat of a misnomer because both types are most often caused by excessive alcohol consumption. Symptoms of dry beriberi include pain, tingling, or loss of sensation in hands and feet (peripheral neuropathy), muscle wasting with loss of function or paralysis of the lower extremities, and potentially brain damage and death. Wet beriberi is characterized by swelling (edema), increased heart rate (tachycardia), lung congestion, and enlarged heart related to congestive heart failure.

Almost a decade earlier, Adolphe Vorderman, a Dutch physician working as a government health inspector in the Dutch East Indies, was interested in the problem of beri-beri in prison inmates. He was one of the first to set up a systematic study of beriberi in humans, and although his work was observational, it stands out because of his scrupulous attempts to avoid biases, even the bias of his own memory. Vorderman described how in 1895 he had spoken with Christian Eijkman, just before the latter's departure for Europe due to ill health, who told him about his strange observations that chickens fed polished rice developed a neurological disease that might resemble beriberi, which disappeared when they were fed unpolished rice. This reminded Vorderman of a fleeting idea that he once had: that beriberi was never seen in some prisons but a lot in others. He thought that this might coincide with the type of rice that the inmates received: "whole" or unpolished "red rice"; or "peeled" or polished "white rice".

    Korsakoff's Syndrome is a deficiency of vitamin B-1 (or thiamine) which causes cardiovascular, central and peripheral nervous system disturbances. The disease results from either inadequate dietary intake or from impaired absorption or utilization of vitamin B-1. Eskiden B vitamini olarak adlandýrýlmýþ olan bu suda eriyen maddeye, daha sonra, öbür b grubu vitaminlerinden ayrýlabilmek amacýyla B1 adý verilmiþtir. Bir süre aneurin olarak da adlandýrýlan bu bileþiðin, bugün en yaygýn kullanýlan adý tiamindir. Beriberi hastalýðýnda beyin dokusu, þekerin varlýðý halinde, yavaþ geliþen bir iþlemle oksijenden yararlanmaktadýr. Karbonhidratlarýn oksitlenmesindeki bu aðýrlýðý tiamin hýzlandýrýr. Tiamin bütün canlý hücrelerde bir miktar bulunur. Bu nedenle hemen hemen bütün besinlerde vardýr. Yaðda eriyemediðinden tereyaðýndan hayvansal ya da bitkisel yaðlardan elde edilemez. Deðirmenlerde gerek buðdayýn, gerekse pirincin öðütülmesi tiaminin büyük çapta kaybolmasýna yol açmaktadýr. Beyaz buðday unundan yapýlmýþ olan ekmek yenilmesi, batý ülkelerinde bile zaman zaman beriberi vakalarýnýn görülmesine yol açmaktadýr. Yapay tiamin çok ucuzdur; bazý ülkelerde deðirmenciler beyaz una bu maddeyi katmak zorundadýrlar. Normal bir hayat yaþayan bir eriþkinin günde 1.3 mg. Kadar tiamin almasý gerekir.
