    Beriberi
    Getting Enough Thiamin T he legend goes something like this: A nineteenth-century Dutch shipboard physician, studying the effects of a strange disease in the Far East, calls for his next patient. But instead of seeing someone walking through the door, he hears a faint cry of "Beriberi!" Roughly translated from Sinhalese, a language of the tiny country of Sri Lanka, the response means "I can't, I can't!" The would-be patient literally couldn't muster enough muscle to get up to see the doctor. And his response became the name of the mystery disease. Years later, this disease, which involves a gradual decline in neuromuscular coordination, was linked to a deficiency of thiamin. Although rice and whole grainsdietary staples in that part of the worldnaturally contain thiamin, the process of refining them for consumption removes the nutrient. The result: Those folks who live on the devitalized rice and grains become thiamin-deficient. The deficiency soon leads to symptoms such as leg swelling and numbness, fluid buildup in the heart, severe muscle wasting, irritability and nausea.

New Scientist DESCRIPTION (back to top) In this comprehensive account of the history and treatment of beriberi, Kenneth Carpenter traces the decades of medical and chemical research that solved the puzzle posed by this mysterious disease. Caused by the lack of a minute quantity of the chemical thiamin, or vitamin B1 in the diet, beriberi is characterized by weakness and loss of feeling in the feet and legs, then swelling from fluid retention, and finally heart failure. Western doctors working in Asia after 1870 saw it as the major disease in native armed forces and prisons. It was at first attributed to miasms (poisonous vapors from damp soil) or to bacterial infections. In Java, chickens fed by chance on white rice lost the use of their legs. On brown rice, where the grain still contained its bran and germ, they remained healthy. Studies in Javanese prisons then showed beriberi also occurring where white (rather than brown) rice was the staple food. Birds were used to assay the potency of fractions extracted from rice bran and, after 20 years, highly active crystals were obtained. In another 10 years their structure was determined and "thiamin" was synthesized.

For much of the past century, beriberi played a bit part on the stage of medical history as the disease that initially prompted the identification of vitamins and the elucidation of their role in human well-being.     How Vitamins were Discovered
    H O H NO The Dreaded "Scurvy Infection".
    The sailor's legs were so swollen that the man could not walk. His captain, hoping to stop the spread of the dreaded "scurvy infection," put the man ashore on a desolate Atlantic island. The poor wretch was bound to die, the captain felt, but perhaps the rest of the crew could thus be saved. The story of the sailor who "ate grass, like a beast and lived" was of great interest to a Scottish surgeon Dr. James Lind. Having been with the British fleet, he was aware of the thousands of seamen who died yearly with scurvy. Lind's question was, Did the grass contain something that the man's normal diet did not? Was there a connection between scurvy and diet? Deciding to experiment, Lind became responsible for an important chapter in the story of "how they found your vitamins." Not that Dr. Lind was looking for a vitamin. The word was unheard of before 1911. The discovery of most vitamins was really accidental in that the researchers were attacking specific diseases, not studying foods or nutrition. Further, this story has no one hero, but involves the efforts of men from many countries. These pioneers frequently did not benefit from one another's discoveries, as they lacked the benefits of modern communications. Yet, sometimes, despite the scorn of contemporary doctors and scientists, the efforts of these men constitute a story of courage, perseverance and eventual success.

    Beriberi is an ailment caused by a deficiency of (Any of a group of organic substances essential in small quantities to normal metabolism) vitamin (A B vitamin that prevents beriberi; maintains appetite and growth) thiamine ), the symptoms of which may include weight loss, emotional disturbances, impaired sensory perception, weakness, and periods of irregular heartbeat.
    Beriberi occurs in people whose staple diet consists mainly of polished white rice, which has little or no thiamine. Therefore the disease has been seen traditionally in people in Asian countries and in chronic alcoholics with impaired liver function. If a baby is fed the milk of a mother who suffers from a deficiency in thiamine, the child may develop beriberi.
    There are two forms of the disease: wet beriberi and dry beriberi. Wet beriberi affects the heart; it is sometimes fatal, as it causes a combination of heart failure and weakening of the capillary walls, which causes the peripheral tissues to become waterlogged. Dry beriberi causes wasting and partial paralysis resulting from damage to the peripheral nerves.

    General Discussion Wernicke syndrome and Korsakoff syndrome are related disorders that often occur due to a deficiency of thiamine (vitamin B1). Wernicke’s syndrome, also known as Wernicke encephalopathy, is a neurological disease characterized by the clinical triad of confusion, the inability to coordinate voluntary movement (ataxia), and eye (ocular) abnormalities. Korsakoff’s syndrome is a mental disorder characterized by disproportionate memory loss in relation to other mental aspects. When these two disorders occur together, the term Wernicke-Korsakoff syndrome is used. In the United States, most cases occur in alcoholics. Some researchers believe Wernicke and Korsakoff syndromes are separate yet related disorders; others believe them to be different stages of the same disorder or disease spectrum. Wernicke syndrome is considered the acute phase with a shorter duration and more serious symptoms. Korsakoff syndrome is considered the chronic phase and is a long-lasting condition.
