A vitamin deficiency disease, caused by a lack of vitamin B1 (thiamin); the most significant manifestations are damage to the heart and nervous system.
Causes, incidence, and risk factors
Beriberi has become almost nonexistent in the United States since the discovery of its cause, thiamin deficiency. Most foods are now vitamin enriched, which means that a normal diet contains adequate amounts of thiamin.
  • Beriberi, although uncommon, can be found among alcoholics. Alcohol-related thiamin deficiency is caused by inadequate intakes of thiamin as well as impaired absorption and storage. Beriberi can also appear, in breast-fed infants when the mother has an inadequate intake of thiamin or if the infant is fed unusual formulas without thiamin supplements. Deficiencies may also be found in people whose diet includes certain types of fish that produce an enzyme that inactivates thiamin. Early symptoms of beriberi are nonspecific and include fatigue, irritability, restlessness, loss of appetite, and vague abdominal discomfort.

This piece will be most useful to medical professionals - doctors and nurses. However, the article may also be useful for non specialists who need to communicate with medical personal about this deficiency disease. In some places medical terms have been simply explained for the non-specialist.
Recently the NGO community in West Africa learned of an outbreak of beriberi in an area with severe nutritional deprivation. The diagnosis had been based upon the presence of epidemic oedema found during a nutritional survey. The survey team included experienced, doctors. The prevalence of beriberi was estimated at 30%. I accompanied an ACF team working in the area to examine these patients. The only clinical feature that these patients had that resembled beriberi was the oedema. The patients had 'famine oedema' which is the same as adult kwashiorkor. In the absence of diagnostic laboratories, the diagnosis of nutritional deficiency diseases depends upon:

University of California Press 2000 A book review by Danny Yee The symptoms of beriberi are weakness and loss of feeling in the feet and legs, swelling of the lower half of the body, and in the worst cases heart failure and death. It used to be a major contributor to mortality, especially in infants a third of the workers in one Malaysian mine in just two years, 10% of all births in Manila around 1915, and so forth. As we now know, beriberi is caused by nutritional deficiency of thiamin (vitamin B1), most commonly associated with reliance on polished white rice. But establishing this, isolating the "vitamin" responsible, and implementing appropriate public health measures was a long and complex process. In Beriberi, White Rice, and Vitamin B Kenneth Carpenter makes of it a fine "medical detective story". Some kind of connection with diet is not hard to discern the Chinese writer Han Yu noted around 800 AD that the disease affected rice-eating people in the south but not wheat-eating ones in the north. But a trace vitamin present in differing quantities in a range of foods posed a complex puzzle and there were a plethora of not implausible explanations: a miasma restricted to particular regions, an infectious agent, toxins or an anti-vitamin in rice, and so forth. An added complication was a lack of communication between scientists and doctors from different national traditions.

In 1893 he discovered that a diet of polished (overkvernet) rice causes beriberi, and was able to produce the disease experimentally in birds. He discovered vitamin B. Together with Sir Frederick Hopkins from Great Britain, he was awarded the 1929 Nobel Prize for Physiology or Medicine, for his discovery of the antineuritic vitamin. Christiaan Eijkman was the seventh child of Johanna Alida Pool and Christiaan Eijkman, a boarding-school proprietor in the small Gelderland town of Nijkerk, situated at the northern border of the Veluwe region. Eijkman had several gifted brothers. One became a chemist and a professor at Tokyo and Groningen; another was a linguist; and a third was one of the first roentgenologists in the Netherlands. A year later, in 1859, the Eijkman family moved to Zaandam, where his father was appointed head of a newly founded school for advanced elementary education. It was here that Christiaan and his brothers received their early education.

General symptoms include loss of appetite and overall lassitude, digestive irregularities, and a feeling of numbness and weakness in the limbs and extremities. (The term beriberi is derived from the Sinhalese word meaning "extreme weakness.") In the form known as dry beriberi, there is a gradual degeneration of the long nerves, first of the legs and then of the arms, with associated atrophy of muscle and loss of reflexes. In wet beriberi, a more acute form, there is edema (overabundance of fluid in the tissues) resulting largely from cardiac failure and poor circulation. In infants breast-fed by mothers who are deficient in thiamine, beriberi may lead to rapidly progressing heart failure. The cardiac symptoms, in both infants and adults, generally respond promptly and dramatically to the administration of thiamine. When neurological involvement is present, response to thiamine therapy is much more gradual; in more severe cases, the structural lesions of the nerve cells may be irreversible. Thiamine normally plays an essential role as a coenzyme in the metabolism of carbohydrates; in its absence, pyruvic acid and lactic acid (products of carbohydrate digestion) accumulate in the tissues, where they are believed to be responsible for most of the neurological and cardiac manifestations.

In Eijkman's time, Indonesia was a Dutch colony and Jakarta was called Batavia. "White rice can be poisonous!" This was the conclusion Christiaan Eijkman declared in 1896 on his return to the Netherlands, after ten years of research in Batavia, Java in the Dutch East Indies (now Jakarta, Indonesia). Christiaan Eijkman was born on August 11, 1858 in the Dutch town of Nijkerk. He was the seventh child of the headmaster at the local school. Robert Koch was Eijkman's teacher in Berlin in 1885. Twenty years later, Koch was to receive the Nobel Prize in Physiology or Medicine for his research on tuberculosis. Who Was Eijkman and Why Did He Travel to Java? Early on Christiaan Eijkman had decided he wanted to become a doctor. When the time came, his family couldn't afford to send him to medical school. Nevertheless, he found the means to get the education he needed. The Dutch colonial army had use for many doctors and they paid for his education in exchange for a few years of medical work in the Dutch East Indies. Christiaan got his medical training, and then went to the colonies to work as an army surgeon. A short time after his arrival he got malaria, and after two years, it became impossible for him to work. He was sent back home to recover.
