Alternate Names : IgA Nephropathy, Berger's Nephropathy Pictures and Images Attribution Berger's disease is generally believed to be an autoimmune disorder that results in kidney damage and may lead to kidney failure. An autoimmune disorder is a condition in which the person produces antibodies to his or her own tissues. What is going on in the body? The job of the kidneys is to make urine by filtering the blood. In Berger's disease, the filtering parts of the kidney, called the glomeruli, become damaged. The damage is due to deposits of proteins called antibodies. Antibodies are normally made by the immune system to fight infections. For some unknown reason, people with Berger's disease have either increased production or reduced clearance of IgA antibodies. The higher level of antibodies clogs up the filtering system of the kidney. What are the causes and risks of the disease?

Treatment of Berger's disease usually focuses on controlling high blood pressure and reducing protein loss in the urine. This is usually done with one of the blood pressure medications called ACE inhibitors. Examples include enalapril and captopril. Fish oil and corticosteroids such as prednisone may help slow down the kidney damage in some cases. Conditions that aggravate kidney failure, such as infections and obstructions, should be treated promptly. Dietary modifications are important as kidney failure progresses. Co-Investigator : William A. Weiss MD, PhD Translations from preclinical to clinical studies have always posed a challenge to extrapolate animal data to the clinical setting. Specifically, subcutaneous human tumor xenograft models, which are most commonly used for glioblastoma studies, have been criticized because of the immunosuppressed and artificial environment in which established high-passage tumor cell lines grow. For these reasons, spontaneous tumor models have gained greater acceptance because in such models, normal cells transform to malignancy within their normal microenvironments, thus better recapitulating the human situation. Our goal is to develop such an endogenous mouse model of GM. First we will establish mice that lack p53 in astrocytes (GFAP-Cre/p53flox), a criterion that has been shown to be one of the earliest events in gliogenesis. Indeed astrocytes, deficient in p53, start proliferating but do not transform. We will then induce subsequential expression of different oncogenes and growth factors (e.g., Ha-ras, K-ras, v-erB, VEGF) in these mice utilizing either a tet-inducible system or the avian RCAS virus system to reiterate a multistep pathway of gliogenesis.

Dr. Bergers studies the functional importance of angiogenesis in tumor development and progression. She has utilized a transgenic mouse model of pancreatic islet cell carcinogenesis to elucidate mechanisms and impact of angiogenesis during tumor formation. She identified that the angiogenic factor VEGF is critical for the angiogenic switch and is regulated posttranscriptionally, via proteolytic release from the extracellular matrix by the metalloproteinase MMP-9 that is expressed by inflammatory cells. This work was published in Nature Cell Biology in 2000. She furthermore translated the studies into experimental therapeutic approaches and designed preclinical trials at three distinct stages of disease progression aimed at preventing the angiogenic switch in premalignant lesions, intervening in the rapid expansion of small tumors, or inducing the regression of large end-stage cancers.
Dr. Bergers was the first to publish that angiogenesis inhibitors have different efficacies depending on the stage of carcinogenesis being targeted (Science 1999). She is currently investigating the underlying mechanisms that lead to stage-specific regulation of angiogenesis. Dr. Bergers' laboratory has expanded its studies to elucidate crucial factors that are implicated in the formation of new blood vessels during gliogenesis and to investigate the functional importance of these factors for the development and progression of glioblastomas utilizing mouse models of glioblastoma. In 2002, Dr. Bergers received the Kimmel Scholar Award, a substantial financial award given to selected promising scientists engaged in cancer research who are at an early stage of their career.

Identifying the Genetic Switches Driving Glioma Initiation and Progression
Glioblastoma (GBM) represents one of the most aggressive and deadly cancers for which there has been little improvement in treatment or basic understanding of disease over the past 25 years of clinical trials and basic science research. Although in its most common form GBM is of highest grade at the time of diagnosis, the ability to study the progression from low-grade astrocytoma to GBM provides a valuable opportunity to identify the key genetic switches governing this transition. Such knowledge of progression genetics would provide both a means of identifying those low-grade patients at increased risk for progression as well as offer novel and more directed drug development opportunities in advanced gliomas including primary and secondary GBM. The support of the Goldhirsh Foundation over the past year has led to the identification of several novel chromosomal regions of gain and loss that are associated with low grade astrocytomas and other regions associated with the transition to secondary GBM. The development of a higher resolution platform for genetic analysis over the past year coupled with the recent implementation of novel bioinformatic tools has been a major advance in our ability to perform these studies and provides even higher confidence that the genes identified will prove to be clinically relevant. The proposed expanded study is designed to ultimately identify those critical genetic elements governing disease progression, grade-specific biology, and therapeutic response.

"EQUALLY LOVED" The great prophet's message of all major religions' could be simplified and understood to be "take care of one another". Our Community at the Shepherds continues to strive not to distinguish between our roles: - client, resident, staff, volunteer, board member, manager, etc., but, to see all as created equal. Understanding each of our roles as better, different or more powerful, only informs us that a lifetime of learning will never be enough to understand the complexities of existence and creation. Events that occur and why they do, can often not be explained or understood, although, science certainly attempts to understand and inform us. Our Creator has created a universe beyond our capacity at this time to understand completely. The message repeatedly heard during the centuries, "if we are hearing", is that there are no differences. We are all of equal value to our Creator. " We are the light!" created equal in our Creator's eyes.
