    Since the prostate lies in front of the rectum, your doctor will most likely perform a digital rectal examination (DRE) by inserting a gloved, lubricated finger into the rectum. By doing this, he or she will be able to feel the prostate and determine if it is enlarged or if it has lumps or other abnormalities. Many doctors perform a prostate exam for men over the age of 50, or over the age of 45 for those with risk factors for prostate cancer, such as family history or African-American ethnicity.

    Almost 50 per cent of men over 65 suffer from problems caused by an enlarged prostate gland BPH, which is one of the most common diseases to affect men over the age of 40, can run in families. Almost 50 per cent of men over 65 suffer from problems caused by an enlarged prostate gland. Unfortunately, it's not possible to prevent the prostate gland increasing in size with age. Enlargement of the prostate gland can obstruct the normal flow of urine, which may cause:
    • increased frequency and urgency of need to pass water and having to get up more often at night to go to the toilet difficulty starting to pass water a weak urine stream stopping and starting dribbling at the end of passing urine feeling that the bladder is not completely empty urine incontinence
    If the bladder doesn't empty completely it may enlarge, making the abdomen visibly swollen. Urine infections are more likely to occur, as are bladder stones. In rare cases, urine retention can damage the kidneys. If the flow of urine is blocked completely, painful urinary retention develops needing emergency treatment.

    The prostate gland commonly becomes larger in older men. This is called Benign Prostatic Hyperplasia. It can cause problems with passing urine. Symptoms can usually be improved by medicines or surgery. However, without treatment, symptoms do not always get worse, and may even improve over time.
    This leaflet only deals with benign (non-cancerous) enlargement of the prostate. Cancer of the prostate is a separate condition and is dealt with in another leaflet. What is the prostate gland? The prostate gland (just called 'prostate' from now on) is only found in men. It lies just beneath the bladder (see diagram). It is normally about the size of a chestnut. The urethra (the tube which passes urine from the bladder) runs through the middle of the prostate. The prostate helps to make semen, but most semen is made by the seminal vesicle (another gland nearby). The prostate gets bigger ('enlarges') gradually after the age of about 50. By the age of 70, about 8 in 10 men have an enlarged prostate. So, in later life it is as normal for your prostate to enlarge as it is to have grey hair.     Benign prostatic enlargement, BPH Benign prostatic hypertrophy is an increase in size of the prostate gland that is so common as to be normal with advancing age and there is no malignancy present. Enlargement is by an increase in the number of glandular units, called hyperplasia , as is typical of glandular enlargement, rather than a constant number of units that increase in size as in muscle hypertrophy . It seems likely that the nature of BPH is a failure of apoptosis and that some of the drugs used to treat it may induce that process.
    The term prostatism has fallen from favour to describe the symptoms of BPH and the term lower urinary tract symptoms or LUTS is preferred. BPH, LUTS and bladder outlet obstruction are not entirely synonymous.
    The prostate secretes about 70% of the volume of seminal fluid. It is hormone dependent and BPH does not occur in castrated men.

    OVERVIEW
    Benign prostatic hyperplasia, a noncancerous enlargement of the prostate gland, is the most common benign tumor found in men. As is true for prostate cancer, BPH occurs more often in the West compared to Eastern countries, such as Japan and China, and may be more common among blacks. Not long ago, a study found a possible genetic link for BPH in men younger than age 65 who have a very enlarged prostate: Their male relatives were four times more likely than other men to need BPH surgery at some point in their lives, and their brothers had a six-fold increase in risk.
    BPH produces symptoms by obstructing the flow of urine through the urethra. Symptoms related to BPH are present in about one in four men by age 55, and in half of 75-year-old men. However, treatment is only necessary if symptoms become bothersome. By age 80, some 20 to 30% of men experience BPH symptoms severe enough to require treatment. Surgery was the only option until the recent approval of drugs that can relieve symptoms either by shrinking the prostate or by relaxing the prostate muscle tissue that constricts the urethra.     It is common for the prostate gland to become enlarged as a man ages. Doctors call the condition benign prostatic hyperplasia (BPH), or benign prostatic hypertrophy. Normal urine flow. At around age 25, the gland begins to grow again. This second growth phase often results, years later, in BPH. Though the prostate continues to grow during most of a man's life, the enlargement doesn't usually cause problems until late in life. BPH rarely causes symptoms before age 40, but more than half of men in their sixties and as many as 90 percent in their seventies and eighties have some symptoms of BPH.

    Benign Prostatic Hyperplasia
    Part of the male reproductive system, the prostate gland produces and stores seminal fluids, releasing them into the urethra when semen emission occurs. The gland is located directly below the bladder and surrounds the upper part of the urethra. During adolescence the gland usually matures and reaches a size comparable to that of a walnut. The dimensions of the gland generally remain unchanged for several decades, but in most older men, the prostate begins to enlarge as the size of its cells increases, a process commonly referred to as benign prostatic hyperplasia ( BPH ) or hypertrophy. Researchers do not yet completely understand the cause of this physiological change, but it is widely thought that elevated levels of the female sex hormone estradiol and increased manufacture of dihydrotestosterone , a derivative of the male sex hormone testosterone, contribute to the condition. Many men with benign prostatic hyperplasia experience urinary problems related to the condition. As the prostate enlarges, the gland places increasing pressure on the urethra, often resulting in difficulty beginning or ending urination, an inability to completely empty the bladder, decreased urine flow, and frequent urination. In the most severe cases, complete blockage of the urethra occurs, which may lead to kidney damage. The symptoms of benign prostatic hyperplasia are similar to those of several more serious problems, including prostate cancer, and should, therefore, be quickly brought to the attention of a medical professional. Several tests and procedures, including a digital rectal exam, prostate specific antigen blood test, rectal ultrasound, urine flow analysis, and cystoscopy, can enable the doctor to rule out other possibilities and make a definitive diagnosis of the condition.

    The prostate is a walnut sized gland found beneath the bladder, and in front of the rectum, and surrounds part of the urethra. Benign Prostatic Hyperplasia (BPH) is an enlargement of the prostate gland, and is very common in men over the age of 50. It is not cancerous, and is not a precursor to prostate cancer.
    The prostate can grow in two ways. In one, the cells multiply around the urethra, squeezing it, and in the other, cells grow into the urethra and bladder outlet area. The second type usually requires surgery.
    Symptoms of BPH include:
    - Blood in the urine - Dribbling after voiding - Feeling the bladder has not fully emptied after urination - Hesitant, interrupted or weak urine stream - Pushing or straining to begin urination - Recurrent sudden urge to urinate
    The exact cause of BPH is not known, though it is related to aging and changing hormone levels. Treatments range from observation to medical treatment, to surgery. There are three types of surgery, two of which involve endourological techniques to decrease the size of the prostate, used when the prostate hasn’t become too enlarged. Open surgery is used when the prostate is very enlarged to remove prostate tissue.

          What is benign prostatic hyperplasia (BPH)?
          BPH (also referred to as benign prostatic hypertrophy) is a condition in which the prostate gland becomes very enlarged and may cause problems associated with urination. BPH can raise PSA (prostate-specific antigen) levels two to three times higher than the normal level. An increased PSA level does not indicate cancer, but the higher the PSA level, the higher the chance of having cancer. Some of the signs of BPH and prostate cancer are the same, however, having BPH does not seem to increase the chances of developing prostate cancer. A man who has BPH may also have undetected prostate cancer at the same time or may develop prostate cancer in the future. Therefore, the National Cancer Institute and the American Cancer Society recommend that all men over 50 consult their physicians about having a digital rectal and PSA examination once a year to screen for prostate cancer.

An increased PSA level does not indicate cancer, but the higher the PSA level, the higher the chance of having cancer. Some of the signs of BPH and prostate cancer are the same, however, having BPH does not seem to increase the chances of developing prostate cancer. A man who has BPH may also have undetected prostate cancer at the same time or may develop prostate cancer in the future. Therefore, the National Cancer Institute and the American Cancer Society recommend that all men over 50 consult their physicians about having a digital rectal and PSA examination once a year to screen for prostate cancer.
