Benign Prostatic Hyperplasia (BPH): Causes, Symptoms and Diagnosis
The Prostate Gland
The prostate is a walnut-sized gland that forms part of the male reproductive system. The gland is made of two lobes, or regions, enclosed by an outer layer of tissue. The prostate is located in front of the rectum and just below the bladder, where urine is stored. The prostate also surrounds the urethra, the canal through which urine passes out of the body. Scientists do not know all the prostate's functions. One of its main roles, though, is to squeeze fluid into the urethra as sperm move through during sexual climax. This fluid, which helps make up semen, energizes the sperm and makes the vaginal canal less acidic.
BPH: A Common Part of Aging
It is common for the prostate gland to become enlarged as a man ages. Doctors call the condition benign prostatic hyperplasia (BPH), or benign prostatic hypertrophy.

Recovering from Benign Prostatic Hyperplasia Surgery
Following surgery for benign prostatic hyperplasia (BPH), you'll probably stay in the hospital from 3 to 10 days depending on the type of surgery you had and how quickly you recover. At the end of surgery, a special catheter is inserted though the opening of the penis to drain urine from the bladder into a collection bag. Called a Foley catheter, this device has a water-filled balloon on the end that is placed in the bladder, which keeps it in place. This catheter is usually left in place for several days. Sometimes, the catheter causes recurring painful bladder spasms the day after surgery. These may be difficult to control, but they will eventually disappear. You may also be given antibiotics while you are in the hospital. Many doctors start giving this medicine before or soon after surgery to prevent infection. However, some recent studies suggest that antibiotics may not be needed in every case, and your doctor may prefer to wait until an infection is present to give them. After BPH surgery, you will probably notice some blood or clots in your urine as the wound starts to heal. If your bladder is being irrigated (flushed with water), you may notice that your urine becomes red once the irrigation is stopped. Some bleeding is normal, and it should clear up by the time you leave the hospital. During your recovery, it is important to drink a lot of water (up to 8 cups a day) to help flush out the bladder and speed healing.

Treatment of Benign Prostatic Hyperplasia at Mayo Clinic
(Synonyms: BPH, prostate gland enlargement) Each year, urologists at Mayo Clinic diagnose and treat more than 6,000 patients who have benign prostatic hyperplasia (BPH). Minimally invasive techniques are among the specialty services offered at Mayo. These procedures typically result in shorter hospital stays, less discomfort and bleeding and a shorter recovery period. Mayo physicians also help advance the field of urology through research.
Diagnosis
Urologists at Mayo Clinic use the latest tests and procedures to diagnose BPH. These assessments include basic tests such as a digital rectal exam, urine analysis or the prostate-specific antigen (PSA) blood test. In some cases more complex evaluations are used to measure urine flow or to visually inspect the prostate. Read more about diagnosis of BPH
Treatment Options
Treatment at Mayo Clinic is tailored to each patient's needs. Experts may recommend behavior changes and medications to help some men with mild symptoms avoid surgery. For moderate cases of BPH, microwave thermal therapy, laser therapies, transurethral needle ablation and other minimally invasive therapies can be used to destroy overgrown tissue. Mayo physicians are also investigating experimental therapies, such as injection of enzymes and other substances to shrink the prostate. Read more about

ABSTRACT What's new in this issue Search abstracts Browse alphabetical list of titles Browse by Review Group A substantive amendment to this systematic review was last made on 31 March 1998. Cochrane reviews are regularly checked and updated if necessary. Background: Benign prostatic hyperplasia (BPH), nonmalignant enlargement of the prostate, can lead to obstructive and irritative lower urinary tract symptoms (LUTS). The pharmacologic use of plants and herbs (phytotherapy) for the treatment of LUTS associated with BPH has been growing steadily. Cernilton, prepared from the rye-grass pollen Secale cereale, is one of the several phytotherapeutic agents available for the treatment of BPH. Objectives: This systematic review aims to assess the effects of Cernilton on urinary symptoms and flow measures in men with benign prostatic hyperplasia (BPH). Search strategy: Trials were searched in computerized general and specialized databases (MEDLINE, EMBASE, Cochrane Library, Phytodok), by checking bibliographies, and by contacting manufacturers and researchers.

Introduction
Hyperplasia is a general medical term referring to excess cell replication. Benign prostatic hyperplasia (BPH) is noncancerous growth of the prostate gland. It is the most common noncancerous form of cell growth in men and usually begins with microscopic nodules in younger men. It should be noted that BPH is not a precancerous condition. Prostate cancer usually occurs in the outer area of the prostate, called the peripheral zone. The prostate gland is an organ that surrounds the urinary urethra in men. It secretes fluid which mixes with sperm to make semen. The urethra carries urine from the bladder and sperm from the testes to the penis.

Symptomatic Benign Prostatic Hyperplasia (BPH) is a common condition in middle aged and elderly men.
BPH can cause bothersome lower urinary tract symptoms (LUTS) related to obstructions of the urethra and gradual loss of bladder function. These symptoms like frequent need to urinate (particularly at night), urgency and leaking or dribbling, or delayed start of voiding with an interrupted weak stream, disrupt the activity and sleep patterns of the sufferers, thus drastically affecting their quality of life.
The incidence of BPH increases with age. Symptomatic BPH in general occurs in approximately 25% of men aged 40 years or over and in one of every three men over 65 years of age. Although surgery can reduce the problems, most patients prefer initial drug therapy, which can delay and maybe even avoid the need for an operation.
Alpha Receptor Blockers have been established as a standard first line therapy of BPH symptoms.

Diagnosis
Benign Prostatic Hyperplasia What is the prostate gland?
The prostate is a small organ about the size of a walnut. It lies below the bladder (where urine is stored) and surrounds the urethra (the tube that carries urine from the bladder). The prostate makes a fluid that becomes part of semen. Semen is the white fluid that contains sperm. Prostate problems are common in men 50 and older. Most can be treated successfully without harming sexual function. A urologist is a specialist in diseases of the urinary system, including diagnosing and treating problems of the prostate gland. How does the doctor detect prostate enlargement?

BPH is an enlargement of the prostate, also known as Benign Prostate Hyperplasia (BPH). The word "benign" means the cells are not cancerous. "Hyperplasia" means an increased number of cells. The prostate gland is a small organ found only in men. It is located in front of the rectum, directly below the bladder. It also surrounds the urethra, the tube through which urine flows. Because of its location, the prostate can have an impact on the ability to urinate. Symptomatic BPH is the term used when BPH produces actual urinary symptoms. In some cases, a man may have an enlarged prostate but not experience the symptoms of BPH.

Enlarged prostate (BPH)
Published by BUPA's Health Information Team
December 2002, revised June 2004 An enlarged prostate – known as benign prostatic hyperplasia or BPH – is caused by an overgrowth of prostate cells. This enlargement constricts the urethra so the flow of urine is reduced, making it increasingly difficult to empty the bladder.
The prostate gland
The prostate is a gland about the size of a walnut that is only present in men. It is located just below the bladder and surrounds the urethra, the tube through which urine flows from the bladder and out through the penis. Please see the diagram, overleaf. One of the main functions of the prostate is to produce an important liquefying component of semen, which allows the sperm to move freely. The gland is divided into three zones, peripheral, transitional and central. With BPH, it is the central part where overgrowth of cells takes place.
