What was formerly known as “basilar artery migraine” or “basilar migraine” is now termed “basilar-type migraine” and is listed as 1.2.6 in the new IHS classification. A description of this condition is listed in Table 6. Basilar-type attacks are mostly seen in young adults. While originally the terms “basilar artery migraine” or “basilar migraine” were used, the involvement of the basilar artery territory was uncertain, and therefore, it is now preferred to use the term “basilar-type migraine.” Bickerstaff The visual symptoms described included vivid flashes of light throughout the entire visual field, intense enough to obscure vision completely, and sudden bilateral visual loss occurring over seconds and persisting up to 15 minutes, with a gradual return of vision to normal. None of these patients had their symptoms of brainstem ischemia accompanied by the characteristic fortification spectra of classic migraine. Later, Bickerstaff described a group of patients in whom consciousness was impaired during attacks of migraine, and he suggested that the mechanism was transient ischemia of the reticular activating system of the brainstem secondary to vasomotor disturbance in the distribution of the basilar artery. Loss of consciousness in migraine was reviewed by Lees and Watkins, with particular reference to the association of migraine and epilepsy. Basser

Abdominal migraines
Abdominal migraines, which are sometimes diagnosed in children, are unusual because they cause pain in the stomach and lead to nausea and vomiting (sometimes without a headache). A child who complains of recurrent pain in his stomach may be experiencing abdominal migraines. Children who suffer from abdominal migraines are likely to have migraine headaches in adulthood. Abdominal migraines are usually treated with anticonvulsant drugs.
Basilar migraines
The basilar migraine was once known as the basilar artery migraine, or BAM. It was considered a problem that was found primarily in young women and adolescent girls, but it occurs in both sexes and all ages. Basilar migraines are extremely rare, but they have the potential to be a serious health hazard in that they can lead to a transient ischemic attack (TIA) or stroke. Symptoms to watch for are double vision, partial vision loss, terrible vomiting, dizziness, loss of balance, slurred speech, lack of coordination, numbness (on one or both sides of the body), weakness, and confusion. These symptoms typically go away at the onset of the actual headache, although they may last for days after the pain disappears.

I have suffered on and off for the last 5 months with dizziness, disorientation, diarrhea and tiredness.
After an initial diagnosis of labyrinthitis, I saw a consultant today who said I had basal migraine. Unfortunately I cannot see the neurologist until the end of July and know very little about this condition and what I can do to alleviate the awful symptoms.
Please do you have any additional information on this condition? I have been given painkillers but would like to know what else I could do to help myself.
Answer
In migraine attacks, the flow of blood through the arteries of the brain is disturbed. The basilar artery is at the back of the brain, and supplies the brain regions which deal with vision, balance and co-ordination with blood. This is why these become upset during attacks.

Foods Migraines are recurrent headaches separated by symptom-free intervals and accompanied by nausea and light sensitivity. Migraines are often accompanied by visual symptoms and are relieved by sleep; furthermore there is usually a throbbing quality. More often than not there is a family history of migraine. Formal criteria have been defined. In general, there is sensitivity to various types of sensory inputs which may trigger headache light (photophobia), sound (sonophobia), smells (such as perfume), and motion persons with migraine are often prone to become motion-sick. Psychological testing shows unsurprisingly that people with an active migraine headache score more poorly on cognitive tests (Meyer JS et al, 2000). Although migraine headache is usually unilateral, opposite sides of the head are characteristically affected during different attacks. About 20% of Migraine's are preceded by an "aura" (see picture on the right), meaning visual symptoms, dizziness , numbness, or weakness. In fact, about 1% of the time, the aura may dominate the migraine, and there may be minimal or no headache ! Migraine auras should last 5 to 60 min. Basilar migraine headache
Basilar migraines are a less common form of migraine headache. This type of migraine occurs most often in children and adolescents, but it can affect men and women of any age. Symptoms may include:
  • Loss of consciousness. Confusion and disorientation. Decreased alertness (lethargy). Mood changes. Vertigo. Difficulty walking (ataxia). Vision problems in both eyes.

Migraine Types Abdominal Migraine
The attacks are characterized by periodic bouts of abdominal pain lasting for about two hours. Along with the abdominal pain they may have other symptoms such as nausea and vomiting, flushing or pallor. Tests fail to reveal a cause for the pain. Occasionally there may be EEG findings suggestive of epilepsy but this is rarely related to seizures. Medications that are useful for treating migraine work to control these attacks in most children. When migraine affects the circulation in back of the brain or neck, it can cause Basilar Migraine or Bickerstaff's Syndrome. It occurs most frequently in young women. Dizziness, double vision, loss of balance, confusion, slurred speech, fainting and disorientation can occur. During the acute headache, some lose consciousness. Often these patients are mistakenly thought to be intoxicated, under the influence of drugs, or suffering from other conditions. A previous history of migraine is helpful in making the diagnosis.

Complicated Migraine
In all these forms, there seems to be an increased risk of the migraine producing permanent neurological changes which may also correlate with changes on the MRI scan like those seen in stroke. Medications such as the beta-blockers, triptans and ergots are often avoided in these types of headaches for fear of increasing the risk of stroke. Calcium blockers and anti-epileptic drugs seem to be best for prevention. Smoking, the use of hormones in menopause and birth control pills are avoided as well again to decrease the risk of long-term problems.

Basilar migraine: polarity-dependent alteration of brainstem auditory evoked potential

We studied serial brainstem auditory evoked potentials (BAEPs) in a patient who had episodic basilar migraine. During migraine attack, significant alteration of wave IV-V was revealed only by rarefaction clicks. The alteration remained unchanged throughout the headache and reverted to normal when the headache subsided. In contrast, BAEPs with condensation clicks were normal during and after the headache.

To this day, dramatic skies play a dominant roll in the landscapes I produce as an artist. The pain of my Migraines throughout my life has been like being tortured by invisible terrorists. An acute severe Migraine is difficult to explain to a non-suffer. Put it this way; after enduring the so-called 'headaches,' you don't fear other things, such as dentists and bullies.
