Causes, incidence, and risk factors: Hair grows from its follicle at an average rate of a 1/2 inch per month. Each hair grows for 2 to 6 years, then rests, and then falls out. A new hair soon begins growing in its place. At any one time, about 85% of the hair is growing and 15% is resting.
Baldness
occurs when hair falls out but new hair does not grow in its place. The cause of the failure to grow new hair in female pattern baldness is not well understood, but it is associated with genetic predisposition, aging, and levels of endocrine hormones (particulary androgens, the male sex hormones). Talk about a stressful work environment . . . how about receptionists who never answer the phones till after 11:00 am because they are too busy beautifying themselves; or talking to friends or boyfriends. Many of my clients have refused to return due to the front desk's indifference; lack of communication and inability to schedule early morning appointments. Forget about calling the client to remind them that they may be running late. Except for the fact that I have many foreign clients with whom I share intelligent conversation, I would have been out of there long ago...What a bloody pain in the arse . . . A. While I understand your frustration with unresponsive and preoccupied receptionists, I lay the major responsibility at the feet of indifferent management. This organizational pattern invariably spells danger. When this kind of irresponsible behavior and unsatisfactory job performance is tolerated, it's like creating a climate that ignores or denies a tumor. Predictably, the tumor and it's impact proliferates. Now the organization has malignancy, malingering and malcontents running rampant. Why is this dysfunctionality tolerated? Often it's because supervisors or managers (busy, lazy, short-sighted, etc.) don't want to confront tension or conflict. (Of course, they minimize and insulate themselves from the amount of stress on the workfloor.) They don't want to struggle through the paper trail documentation. Perhaps they have been burned by a grievance procedure. Some manager's deny how disruptive the "up and down" behavior of an employee can be. Alas, I recall a supervisor increasingly urging her branch manager for more support with an unstable employee...to no avail. Until, one day, this disturbed individual pulls a knife on her. Clearly, supervisors and managers need training in quickly recognizing and professionally responding to "stress carriers" and cancer-breeding work environments. Remember, Practice Safe Stress!

This is science, not science fiction. Research has shown that bald males command a larger harem of females than their hairier counterparts...that is, if the bald male is a maneless Tsavo lion from Kenya. In humans, male pattern baldness (or androgenic alopecia) has been linked to a number of biological pathways. DHT is thought to promote hair loss in three ways: 1) Healthy follicles grow hair for a time, usually for 2-5 years, and then take a break before starting to grow hair again. DHT shortens the hair growth time and increases the follicle's hair growth holiday. This results in fewer new hairs and shorter ones at that. 2) Immediately before a healthy follicle stops hair growth, it shrinks and the hair it produces is thin and weak (vellus hair). DHT causes the follicle to shrink prematurely which is why bald people have peach fuzz on their heads. 3) Follicles need a blood supply to be nourished. Male Pattern Baldness (Androgentic Alopecia) is a genetic disorder that affects millions of men worldwide, causing them to lose their hair. In fact, Male Pattern Baldness is the leading cause of hair loss in men and it also affects women (on a much smaller scale).
What causes Male Pattern Baldness?
The causes of Male Pattern Baldness are complex, but we do know a few things about it:
  • It is hereditary. Typically, the gene that is responsible for it is passed down from the mother's side of the family.
  • Hormones such as testosterone and DHT play a major role in Male Pattern Baldness.
  • Genetically determined characteristics of the hair follicles cause them to get smaller with age, leading to the inability of the follicle to grow a hair.

Dateline follows five men on different hair treatments
By By Rob Stafford Dateline NBC They are Dateline's version of the '"Fab Five," five proud men at the pinnacle of their lives and life is good, except for one thing. They're all losing their hair. For men, going bald or having thinning hair can be an extremely emotional issue, bringing on feelings of lost virility, attractiveness, sexiness or simply getting old. But now our guys are going to try to get control of their ever-widening bald spots and ever-thinning hair. The five, all friends from the Miami area, have agreed to participate in an experiment for Dateline. For 12 months, they're each going to try a different drug, medical procedure or one of those products you've seen advertised on late night TV or the Internet.       We'll follow them over the next year to see how well these products and procedures work on these volunteers. Our five men range in age from their 30s to our oldest volunteer, who is 54.

Avodart: Recently, a dual inhibitor of both enzyme types (5ar-1 and 5ar-2) that convert testosterone to DHT has been approved for BPH. It's called Avodart (dutasteride)and is made by GlaxoSmithKline. Finasteride was shown to reduce DHT by up to approximately 70+% or so, where as Avodart reduced DHT by 90-96% depending on dose (0.5mg - 2.5mg). So, this drug would appear more effective for treating MPB, and Glaxo's data found 2.5mg of Avodart grew 1.5 times more hair compared to 5mg Finasteride over a 6 month period with approximately similar side effects as Finasteride.

Androgenic Alopecia I. Definition: Androgenic alopecia is the name for male and female pattern baldness. In androgenic alopecia the hair loss occurs slowly over years. It can start anytime after age twenty. There is usually a family history of hair loss. In women, the hair slowly becomes thin throughout the scalp and bald spots usually do not occur. Hair Loss
What is baldness?
Baldness is loss or lack of hair, usually from the top of the head. It may also occur on other parts of the body where hair normally grows. Baldness is usually part of the aging process. Some diseases and drugs may also cause baldness. Hair loss may be either temporary or permanent, depending on the cause.
How does it occur?
There are several types of baldness: male and female pattern, local, and general. Male-pattern baldness runs in families. This hereditary baldness usually happens above the forehead, causing a receding hairline. It also happens at the crown of the head. Over time, the bald spots increase in size until the entire top of the head is bald and there is hair only on the sides of the head. Sometimes female-pattern baldness occurs in women. Usually it causes the hair to thin in the front, on the crown, or on the sides.

For 95% of male baldness patients, hair loss is inherited. The hair follicles receive genetic coding during formation in the womb. These male baldness genes make certain hair follicles sensitive to the DHT hormone, or dihydrotestosterone. When this link is made, the hair follicle begins to shrink. It's first noticed as thinning hair, since the hair follicle begins producing only a very thin, short hair. Eventually the hair follicle dies and permanent male baldness and hair loss occurs. Male pattern baldness usually progresses to the familiar horseshoe-shaped fringe of hair. The degree of male baldness is unique and different, but doctors have classified male baldness categories and have accepted a scale that graphically represents different patterns of male baldness called the Norwood Classification. FOR PATIENTS
It is estimated that 35 million men in the United States are affected by male pattern baldness or androgenetic alopecia . "Andro" refers to the androgens (testosterone, dihydrotestosterone) necessary to produce male-pattern hair loss (MPHL). "Genetic" refers to the inherited gene necessary for MPHL to occur. In men who develop male pattern baldness the hair loss may begin any time after puberty when blood levels of androgens rise. The first change is usually recession in the temporal areas, which is seen in 96 percent of mature Caucasian males, including those men not destined to progress to further hair loss. Hamilton and later Norwood have classified the patterns of male pattern baldness (see illustration below). Although the density of hair in a given pattern of loss tends to diminish with age, there is no way to predict what pattern of hair loss a young man with early male pattern baldness will eventually assume. In general, those who begin losing hair in the second decade are those in whom the hair loss will be the most severe. In some men, initial male-pattern hair loss may be delayed until the late third to fourth decade. It is generally recognized that men in their 20’s have a 20 percent incidence of male pattern baldness, in their 30’s a 30 percent incidence of male pattern baldness, in their 40’s a 40 percent incidence of male pattern baldness, etc. Using these numbers one can see that a male in his 90’s has a 90 percent chance of having some degree of male pattern baldness.
