Medische redenen a) Phimosis Bij phimosis is de voorhuid zo vernauwd dat deze niet of nauwelijks teruggetrokken kan worden over de eikel. Phimosis kan aangeboren zijn, of veroorzaakt worden door herhaalde infecties van de voorhuid. Bij kinderen tot ongeveer een jaar of 5 is het normaal dat de voorhuid nog niet (volledig) teruggetrokken kan worden. Ten onrechte krijgen ouders nog vaak het advies de voorhuid te "oefenen". Dit kan beter achterwege blijven aangezien door het geforceerd terugtrekken van de voorhuid phimosis juist kan ontstaan. Phimosis kan allerlei problemen veroorzaken. Doordat de voorhuid niet teruggetrokken kan worden en de eikel eronder niet goed schoon te maken is, hoopt zich smegma op wat ontstekingen en zo verdere vernauwingen in de hand werkt. Ook kan meatus stenose, een vernauwing van de opening van de plasbuis ontstaan. Vaak blijkt bij mannen met kanker van de penis (wat gelukkig niet vaak voorkomt), dat ze een lange voorgeschiedenis van phimosis hebben. Een besnijdenis is aan te raden. De voorhuid kan niet teruggetrokken worden b) Paraphimosis Als de voorhuid teruggetrokken is, opgewollen is geraakt en niet meer teruggeschoven kan worden over de eikel, dan is er sprake van paraphimosis.
