BALANITIS
What is Balanitis ?
Boys have a hood of skin over the end of the penis called the foreskin. The area at the top of the penis underneath this is called the glans. In the middle of the glans is the end of the tube from the bladder called the urethra, through which urine comes. Balanitis refers to soreness of this glans area. The foreskin may also be sore in this condition. The penis may appear swollen and the child may complain of soreness. He may also complain of pain when passing urine.
How does Balanitis occur ?
In early childhood, the balanitis may just be an extension of nappy rash. Germs such as bacteria and yeast can multiply on the glans and cause the balanitis.
Why does Balanitis occur ?
Nappy rash or poor hygiene (inadequate washing of the penis) can be causes of balanitis. In diabetes, yeast germs such as candida (thrush) are more likely to grow, and balanitis can be the first sign of diabetes in children. A foreskin which is too tight can lead to soreness and infection. Balanitis is less common in children who have been circumcised.
Treatment Involved for Balanitis
Simple bathing may clear many cases. If possible the foreskin should be retracted and gentle cleaning of the glans should be carried out. The doctor may send off swabs from the skin to see what sort of germ is growing. Creams or antibiotics may be prescribed. The sort of creams that will be advised will be those which are active against candida yeast infections. Other antibiotics may depend on what germ is found. If the balanitis is due to a foreskin that is too tight, the doctor may recommend an operation to remove the foreskin (circumcision). This is a very safe procedure. It would not interfere with any aspects of the child's function in the future.

Balanitis ?
Dec 16, 1999 In May I noticed a dry rash about the size of a dime(with skin flaking) on the glans of my penis. The doctor diagnosed it as a candida related fungal infection and I treated with clotrimazole 1%. The rash subsided after apprxomately 4 week of treatment, but then returned. I re-treated, the rash went away then returned. I am still assuming this is a recurrent balanitis, but no test were done including Syphillis or ghonorrea to confirm diagnosis. Could this be considered an OI. My VL is ND and CD4 around 450. Are there any other tests which could verify what exactly this is and how to treat? Response from Dr. Feinberg

I live in Dublin, Ireland
I have always had problems with my foreskin/penis.About 5 years ago I was diagnosed as having herpes by a dermatologist.I was shocked to hear this as I never had blisters on my penis.He never conducted any examinations before coming to this conclusion e.g. no blood test, no swab, no urine sample.I went to another STD expert who conducted all the tests and told me that I was clear of all STDs and that I merely had balanitis.He told me to take salt baths and put Caldesene powder on the affected area.I tried this for two months solid but it didn't really work well because my foreskin still felt very sore.I was getting swollen foreskins and recurrent smelly discharges that would last for about two days and clear up once I kept washing my penis.
Anyway, I then got a torsion in my right testicle and had to have it removed.I have since had a heavy silicone implant inserted in it's place.This is probably not relevant to my main query.
Six months ago, I contacted a urologist about my recurrent balanitis and he suggested a circumcision.I had this done in January 05.I have healed totally from my circumcision BUT I still have a soreness around the area where the top of my foreskin used to be e.g. just under the hood of the glans.

Plasma cell balanitis/vulvitis
What is plasma cell balanitis/vulvitis?
This uncommon skin condition usually presents as red patches on the genitals. It is called plasma cell balanitis in men and plasma cell vulvitis in women because of the numerous plasma cells seen on skin biopsy. A plasma cell is a type of white blood cell found in some inflammatory conditions. Plasma cell balanitis affects the head of the penis (the glans) and is also known as balanitis circumscripta plasmacellularis or Zoon's balanitis. Plasma cell vulvitis is most commonly found on the inside of the vulva at the entrance to the vagina (vestibule). It is also known as vulvitis circumscripta plasmacellularis.
What are the signs and symptoms of plasma cell balanitis/vulvitis?
In males, the lesion is usually a single glistening red plaque. In women there are usually several patches. Patients may have no symptoms but they may complain of tenderness and/or slight itchiness.

    Causes, incidence, and risk factors: Balanitis is usually caused by poor hygiene in uncircumcised men. Bacterial, fungal, viral, or non-infectious inflammation may occur. Exposure to caustic soaps or failure to properly rinse soap off while bathing can also result in inflammation. Several other diseases, including Reiter's syndrome and lichen sclerosis et atrophicus can also produce balanitis.

    Signs and tests: Your dermatologist or urologist may be able to diagnosis the cause of your balanitis by examination alone. Additional tests, such as skin culture for viruses, fungi, or bacteria, are often needed, however. Occasionally, a skin biopsy is required.
    Treatment: Treatment depends on the cause of the balanitis. For example, infectious balanitis may be treated with topical or oral antibiotics. Some balanitis which occurs in association with skin diseases may respond to topical steroid creams. In severe cases, circumcision may be the best option.
    Expectations (prognosis): Most cases of balanitis can be controlled with topical medication and good hygiene. Surgery is not usually necessary. Outcomes are nearly always positive.
