Atrial fibrillation is the most common form of irregular heartbeat. The condition causes the heart to pump blood less efficiently, allowing blood to pool within its chambers and increasing the risk of heart attack, stroke, and heart failure.

What is atrial fibrillation?
Atrial fibrillation (AF) is the most common, abnormal rhythm of the heart.
The heart contracts (beats) and pumps blood with a regular rhythm, for example, at a rate of 60 beats per minute there is a beat every second. The heart may beat faster or slower with a shorter or longer interval between beats, but at any one rate the interval between beats is constant. This regular rhythm occurs as a result of regular electrical discharges (currents) that travel through the heart and cause the muscle of the heart to contract. In AF, the electrical discharges are irregular and rapid and, as a result, the heart beats irregularly and, usually, rapidly.

It usually involves a rapid heart rate , in which the upper heart chambers (atria) are stimulated to contract in a very disorganized and abnormal manner. Causes, incidence, and risk factors Return to top Arrhythmias are caused by a disruption of the normal functioning of the electrical conduction system of the heart. Normally, the atria and ventricles contract in a coordinated manner. In atrial fibrillation and flutter, the atria are stimulated to contract very quickly and differently from the normal activity originating from the sinoatrial node. This results in ineffective and uncoordinated contraction of the atria in atrial fibrillation, and in a peculiarly organized contraction pattern in atrial flutter. The condition can be caused by impulses which are transmitted to the ventricles in an irregular fashion or by some impulses failing to be transmitted. This makes the ventricles beat irregularly, which leads to an irregular (and usually fast) pulse in atrial fibrillation.

Unfortunately, there is evidence that individuals who have had hyperthyroidism in the past and have been cured and now have normal thyroid function may still have an increased risk for atrial fibrillation relative to the rest of the population. Thus, by age 70, 10% of individuals with a history of hyperthyroidism have atrial fibrillation while only 5% of those who lack a history of a thyroid problem have the condition. A combination of factors may explain these observations. Excess thyroid hormone stimulates the heart to go faster and may precipitate irregular heart rhythm disturbances, particularly in patients who are predisposed to this occurring.

Surgical procedure for atrial fibrillation

The latest interventional procedure to treat atrial fibrillation is a minimally invasive operation called the "Mini-Maze." The "Mini-Maze" was developed by Dr. RANDALL K. WOLF, Professor of Cardiothoracic Surgery and Director of the Center for Surgical Innovation, with the UNIVERSITY of CINCINNATI SURGEONS Inc., using instruments that he helped design.
Dr. Wolf's procedure combines an “ablation” method and the "maze" procedure, WITHOUT performing "Open heart surgery." Dr. Wolf developed this new minimally invasive procedure to cure atrial fibrillation without making a sternal incision, usually 7 inches long in the middle of the chest, and without having to use the heart-lung machine. The mini-maze procedure is a much less invasive procedure, in which he enters the heart through small incisions between the patient’s ribs. Dr. Wolf performs in fact two surgical procedures at one time. He uses a special "Bi-polar" radio frequency (RF) clamp to ablate and electrically isolate the pulmonary veins, where the triggers are located that activate atrial fibrillation. This ablation procedure is performed through two small “non-rib-spreading” mini-incisions, one on each side of the chest.

A steady, inconspicuous heartbeat is usually taken for granted so when an attack of atrial fibrillation strikes it is a very frightening experience. The heart beats wildly with a pulse rate as high as 200 beats/minute, dizziness and breathlessness and even fainting may follow and chest pain, extreme fatigue and the need for frequent urination are common symptoms. Fibrillation attack victims usually experience one or more of these symptoms, but a few patients are not aware of any symptoms at all until an electrocardiogram exposes their irregular heart rhythm(1-4). Atrial fibrillation is the most common cardiac arrhythmia and affects more than 1.5 million Americans. Its primary characteristic is a rapid and irregular heartbeat. The incidence of atrial fibrillation shows a significant increase beyond the age of 50 years and the condition is considerably more common among men than among women. Atrial fibrillation may be chronic or intermittent (paroxysmal) and may be triggered by an underlying heart disease such as mitral valve prolapse or stenosis, coronary artery disease, hypertensive heart disease, a heart attack or an inflammation of the membrane surrounding the heart (pericarditis). Atrial fibrillation is also a common complication of heart surgery(1-3,5-7).
