MULTIPLEX CONGENITA Arthrogryposis refers to a joint that is in a fixed position. Arthrogryposis multiplex congenita is diagnosed when two or more joints in more than one limb are fixed from birth (Figure 151.1). Figure 151.1. — Arthrogryposis multiplex congenita. A joint becomes fixed in a given position because of unevenly impaired motility. Unevenly impaired joint motility results from segmental muscle weakness or hypotonia, or from a sustained asymmetrical posture. Segmental muscle weakness or hypotonia is due to an imbalance between agonistic and antagonistic muscles acting upon a joint. Sustained asymmetrical posture results from movement constraints that occur as a result of reduced uterine volume or cutaneous bands that restrict movements. MainMenu Back Next Index

The characteristics of arthrogryposis multiplex congenita that distinguish it from brachial plexus palsy are: (1) the presence of skin pits at the wrists and (2) the permanent nature of the arm position (Figure 167.1). In patients with brachial plexus palsy there is no skin pits and the abnormal position is only present when the neonate attempts to move the arm

DIFFERENTIAL DIAGNOSIS OF A NEONATE WITH ARTHROGRYPOSIS MULTIPLEX CONGENITA Neonates with proximal arthrogryposis are likely to have amyoplasia congenita . Neonates with distal arthrogryposis present a complex diagnostic problem. Most neonates with distal arthrogryposis can only be diagnosed by recognizing a constellation of signs leading to the diagnosis of a syndrome. Few neonates with distal arthrogryposis have specific laboratory findings. Zellweger syndrome courses with increased very-long-chain fatty acids in plasma.

    congenita = seen at birth.
At the heart of AMC is a failure to form a joint or to form it fully (arthrogryposis). Sometimes this defect is generalized (multiplex). Why someone stuck the really obvious congenita tag on an already overly lengthy name is anybody's guess as there are no acquired cases. It is always congenita. So just consider the first two terms as having significance. In the developing limbs, the central mesoderm tissue that will become bone and muscle begins to sort out with a long tube of bone prone tissue wrapped in muscle bound tissue. At certain intervals, small fluid lakes begin to break the length of this into segments. The muscle development is running well ahead of the bone and even begins to do some twitching as nerves have dived in and linked up. As the twitching moves the segments the spaces come alive with shape and detailed form steering
qualities called joints. However, absent that twitching, FOR ANY REASON, the joint formation fails. If the interference is local, just one joint fails. If the interference is late, then some shaping may be amiss or maybe even things go well. Joints that do not form are not joints at all. Often we hear of this person or that having "fused" bones from birth in the foot or whatever. No. They are not fused as that means that they were once separate and then joined. They never succeeded in separating in the first place. But who wants to use this name - arthrogryposis? Nobody. So the bones are called fused - wink wink.

Arthrogryposis
Definition
Arthrogryposis translated from the Greek literally means "curved or hooked joints." Hence, this term is used to describe multiple joint contractures present at birth. There are many causes for congenital multiple joint contractures, and could be due to abnormal nerve or muscle development in the womb. However, the commonest form of arthrogryposis, present in 40% of cases, is a condition called amyoplasia.
Symptoms
The child is born with multiple joint contractures in all limbs. In the commonest form of arthrogryposis, called amyoplasia, the typical deformities are often severe and symmetrical, as follows:
shoulder internal rotation deformity
elbow extension and pronation deformity
wrist volar and ulnar deformity
hand fingers in fixed flexion, and thumb-in-palm deformity
hip flexed, abducted and externally rotated, often dislocated
knee flexion deformity

The Center for Athrogryposis Treatment Imagine children unable to groom or dress themselves because their elbows are completely straight and cannot bend. Or children unable to walk with any efficiency because their hips and knees are not only deformed, but also are incapable of motion. This is the plight of children with arthrogryposis. Arthrogryposis, from the Greek terminology meaning hooked or curved joints, describes not a single diagnosis but a spectrum of congenital deformities. What each condition shares are the multiple joint contractures , poorly developed and contracted muscles, and limb deformities. There is a wide spectrum of involvement and severity, from children having only a deformed hand or foot, to those with both arms and legs completely affected. At the Center for Arthrogryposis Treatment, director Harold J.P. van Bosse, M.D. coordinates a team approach to these complex children. The children are carefully evaluated and followed by both the orthopaedic and pediatric staff. At their first visit, they are also referred to a geneticist and a neurologist to help establish which underlying condition, if any, may be present. Arthrogryposis is a term used to describe a number of rare, non-progressive conditions characterized by stiff joints and abnormal muscle development. It is also referred to as arthrogryposis multiplex congenita or amyloplasia. What causes arthrogryposis? The exact cause of arthrogryposis is unknown, though a number of different theories have been proposed. Some believe that arthrogryposis is caused by mechanical obstructions to intrauterine movement during pregnancy. Others believe that it may be a result of an early viral infection during development. Still others believe that arthrogryposis is the result of failure of the central nervous system and/or muscular system to develop appropriately. Arthrogryposis is not thought to be a genetic or hereditary condition. How common is arthrogryposis? Arthrogryposis is a rare condition, though the exact frequency with which it occurs is unknown. Previous studies have estimated that it affects 1 -3 of every 10,000 live births. How is arthrogryposis diagnosed? Arthrogryposis is diagnosed by treating physicians after a thorough medical history and careful physical examination. X-rays are often taken to confirm the diagnosis and to evaluate stiff or dislocated joints. Additional tests, including blood tests, muscle biopsies, and other imaging studies, are often used to further make the diagnosis. Patients with arthrogryposis often have involvement of the upper limbs. Typically, these patients will have shoulders that are internally rotated and adducted, elbows that are stiff in extension or flexion, and wrists and fingers that are flexed.

Very few centers are devoted to treating arthogryposis. By centralizing state-of-the-art techniques, the Arthrogryposis Center at the Center for Children provides an alternative for patients who travel to different locations for treatment. This multi-disciplinary clinic integrates a variety of services from the Hospital for Joint Diseases and the Rusk Institute of Rehabilitation Medicine. Through this clinic, each child is evaluated by a team consisting of a pediatrician, neurologist, orthopedic surgeon, geneticist, and physical and occupational therapist.
