  • Overview . Appendicitis is a common cause of abdominal pain. However, the presentation is not always classic and a high index of suspicion is necessary. Affects any age group but is rare in infants, most common in adolescence and young adult years. Generally occurs from obstruction of the appendiceal lumen by lymphoid hyperplasia or a fecalith.
  • Clinical Presentation.
  • History . Classic history is that of periumbilical or epigastric pain that migrates to right lower quadrant. Pain may be felt in flank (retrocecal appendix, pregnancy), testicle (retroileal appendix), or bladder. Anorexia, nausea, and vomiting may occur after the onset of pain. Anorexia is less likely to be present in children. Presentation is more likely to be atypical in very young, very old, and pregnant patients. Maintain a high index of suspicion in any patient with abdominal pain. Physical exam. Fever is only found in 15%.

    Description of Appendicitis
    Most people mistakenly believe that appendicitis is a thing of the past, thanks to antibiotics. In fact, appendicitis is still a common problem that can only be treated surgically. Antibiotics may, however, help a patient recover from surgery with fewer complications. Located on the lower right side of the abdomen, the appendix - a small tube that extends from the large bowel - seems to serve no useful purpose in the body. Most people are not aware of this organ unless it becomes inflamed. Although waste matter from the bowel flows past it, the appendix's own mucous secretions normally keep it clean. The swollen appendix, filled with pus and fluid, may block off its supply of blood. With its blood supply cut off, the wall of the appendix weakens and sometimes ruptures. This is called a perforated appendix. The pus that breaks through may infect the abdominal cavity or cause an abdominal

    Definition: Appendicitis is a sudden inflammation of the appendix, a small, finger-shaped tube that branches off the large intestine.
    Causes, incidence, and risk factors: Appendicitis is one of the most common causes of emergency abdominal surgery in children. Approximately 4 appendectomies per 1,000 children are done annually in the United States. Appendicitis is more common in males than in females, and incidence peaks in the late teens and early 20s. The condition is uncommon among children younger than 2, but it can occur. Appendicitis generally follows obstruction of the appendix by feces (fecalith), a foreign body, or rarely, a tumor . Typically, the first symptom is crampy or "colicky" pain around the navel (periumbilical). There is usually a marked reduction in or total absence of appetite, often associated with nausea, and occasionally, vomiting and low grade fever. As the inflammation in the appendix increases, the pain tends to move downward and to the right (right lower quadrant) and localizes directly above the position of the appendix at a point called "

    Appendicitis
    Appendicitis is inflammation of the appendix, a small pouch attached to the cecum, the beginning of the colon, on the lower right side of the abdomen. The appendix is not necessary for life, but it can become diseased. If untreated, an inflamed appendix can burst, causing infection and even death. Appendicitis can affect people at any age, but is most common in people ages 10 to 30. The cause of appendicitis is usually unknown. It may occur after a viral infection in the digestive tract or when the opening connecting the large intestine and appendix is blocked. The inflammation can cause infection, a blood clot, or rupture of the appendix. Because of the risk of rupture, appendicitis is considered an emergency, so anyone with symptoms needs to see a doctor immediately.     Introduction: Acute Appendicitis
    Acute Appendicitis: The main symptom of acute appendicitis is abdominal pain or abdominal sensitivity. However, abdominal pain occurs with many conditions and only an estimated 5% of cases of abdominal pain are actually appendicitis. Although uncommon, appendicitis is very serious, and difficulty in diagnosing appendicitis in the emergency department makes appendicitis the 3rd leading cause of malpractice lawsuits. Misdiagnosis of appendicitis is particularly common in children and infants with abdominal pain with estimates of initial misdiagnosis rates from 28% to 57% for under age 2-12 and almost 100% misdiagnosis for appendicitis in infants. Acute Appendicitis: Appendicitis is inflammation of the appendix, a small portion of the large intestine that hangs down from the lower right side. Although the appendix does not seem to serve any purpose, it can still become diseased. If untreated, an inflamed appendix can burst, causing infection and even death. About 1 in 500 people has appendicitis each year.

    Transverse and sagittal sonograms demonstrate a thick-walled, noncompressible appendix ( arrows ). A hyperechoic appendicolith ( black arrows ) is visible within the lumen of the appendix and demonstrates acoustic shadowing ( white arrows ). The appendix measures 25 (transverse) x 21 (anteroposterior) x 20 (sagittal) mm.
    Differential Diagnosis
    Given the sonographic findings, acute appendicitis is the most likely diagnosis. Crohn's disease with ileocecal and appendiceal involvement, acute typhlitis, cecal diverticulitis, and appendiceal neoplasm are other possible, but less likely, diagnoses.

    Discussion
    Acute appendicitis commonly presents with periumbilical pain, nausea, and vomiting. Within days, the pain localizes to the right lower quadrant and is accompanied by peritoneal signs on physical examination. A clinical diagnosis is often difficult to make, as a variety of other disorders can have a similar clinical presentation. For example, gynecological disorders such as ovarian torsion, rupture of an ovarian cyst, ectopic pregnancy, or acute pelvic inflammatory disease may mimic appendicitis in women.

    Presentation
    A 29-year-old woman presented to the emergency room with nausea, vomiting and abdominal pain. The pain was initially in the periumbilical area and later localized to the right lower quadrant. On physical exam, her temperature was 38 degrees Celsius. She had guarding and mild rebound tenderness in the right lower quadrant of her abdomen. On pelvic examination, there is right adnexal tenderness. Her white blood cell count (WBC) is 12K/µl and her urine B-HCG is negative.

    Supine and upright radiographs of the abdomen
    Ultrasound

    Supine and upright plain radiographs of the abdomen show a relative paucity of bowel gas on the left side of the abdomen. There is mild irregularity and thickening of the folds in a loop of bowel in the right lower quadrant ( arrow ). Several gas-fluid levels are seen in this region ( arrows ). No appendicolith or free intraperitoneal air is seen. These findings are consistent with an inflammatory process in the right lower quadrant resulting in a localized small bowel ileus. The appendix is a small pouch like a finger connected to your main gut. It's use is unknown but sometimes it gets blocked by stool passing by. Once this happens bacteria in the stool start to multiply and cause an infection of the appendix. It's like having a river of stool passing by. If it stops it gets stagnant just like a real pool of water allowing bacteria to grow - which is why stagnant water starts to smell bad.
