Appendicitis

Appendicitis is a condition characterised by inflammation of the appendix . While mild cases may resolve without treatment, many require laparotomy with removal of the inflamed appendix. Untreated, mortality is high, mainly due to peritonitis and shock when the inflamed appendix ruptures.

Causes
It is possible for fecal matter to become lodged in the appendix, leading to bacterial infection , which can cause appendicitis. If the condition is untreated, the appendix can turn gangrenous and can eventually burst, leading to peritonitis septicemia and eventually death edit
Signs, symptoms and findings
The pain of appendicitis usually starts centrally (periumbilical) before localising to the right iliac fossa (the lower right side of the abdomen). There is usually associated anorexia (loss of appetite).

What is appendicitis?
Appendicitis is an inflammation of the appendix, a small worm-like pouch attached to the large bowel. It can happen at any age but most cases are between 8 and 25 years of age. It is rarely seen in children aged under two years.
For young people, appendicitis is probably the most common cause of stomach pain requiring emergency surgery.
What causes appendicitis?
In most cases, the specific reason for the inflammation is not known but it is sometimes caused by small pieces of hardened stool (faecaliths) that get stuck in the appendix.

Appendicitis is an inflammation of the appendix , a small tubular piece of tissue about the size of a finger that connects to the large intestine, usually at the lower right side of the abdomen. The inside of the appendix forms a pouch that opens into the large intestine. With appendicitis, the appendix becomes inflamed and infected by bacteria because the inside of the appendix gets blocked - by a foreign body (such as a seed in food) that was swallowed or because of swelling in the intestine from an infection. If the infected appendix is not removed, it can eventually burst or rupture from the buildup of pressure. This may happen as soon as 48 to 72 hours after symptoms start. The infection from a ruptured appendix is very serious - it can form an abscess (a walled-off infection of pus) or spread throughout the abdomen.

Acute Appendicitis: Review and Update

Appendicitis is common, with a lifetime occurrence of 7 percent. Abdominal pain and anorexia are the predominant symptoms. The most important physical examination finding is right lower quadrant tenderness to palpation. A complete blood count and urinalysis are sometimes helpful in determining the diagnosis and supporting the presence or absence of appendicitis, while appendiceal computed tomographic scans and ultrasonography can be helpful in equivocal cases. Delay in diagnosing appendicitis increases the risk of perforation and complications. Complication and mortality rates are much higher in children and the elderly. (Am Fam Physician 1999;60:2027-34.) A ppendicitis is the most common acute surgical condition of the abdomen. Approximately 7 percent of the population will have appendicitis in their lifetime, with the peak incidence occurring between the ages of 10 and 30 years.

A ppendicitis remains the most common acute surgical condition of the abdomen. In 1997, more than 260,000 new cases occurred in the United States. The overall lifetime occurrence is approximately 12 percent in men and 25 percent in women. The case-fatality rate of appendicitis jumps from less than 1 percent to 5 percent or higher when perforation occurs. Because abdominal pain is a common presenting complaint in the outpatient setting, family physicians serve an important role in the rapid diagnosis of acute appendicitis. Accurate and timely diagnosis of acute appendicitis is essential to minimize morbidity. Prompt surgical treatment may reduce the risk of appendix perforation. The case-fatality rate of appendicitis jumps from less than 1 percent in nonperforated cases to 5 percent or higher when perforation occurs. The diagnosis of appendicitis traditionally has been based on clinical features found primarily in the patient's history and physical examination.

Etiology and Incidence
Although acute appendicitis is the most common disease of the appendix, other potential pathologic conditions affecting the appendix include swallowed foreign bodies, pinworms, fecaliths, carcinoids, cancer, villous adenomas, and diverticula. The appendix may also be involved in idiopathic ulcerative colitis or the ileocolitis of Crohn's disease. Except for hernia, acute appendicitis is the most common cause in the USA of an attack of severe, acute abdominal pain that requires abdominal operation. Because symptoms and signs vary widely and because delay before operation is so hazardous, it is accepted that nearly 15% of operations for acute appendicitis lead to other findings at laparotomy or even to findings of no disease.
Symptoms and Signs
Classic right lower quadrant tenderness is located at McBurney's point (junction of the middle and outer thirds of the line joining the umbilicus to the anterior superior spine). Rovsing's sign (pain felt in the right lower quadrant resulting from palpation of the left lower quadrant) suggests the possibility of appendicitis. The psoas sign (an increase in pain from passive extension of the right hip joint, which stretches the iliopsoas muscle) or adductor pain (pain produced by passive internal rotation of the flexed thigh) may suggest both the anatomic location of the appendix and progression of the inflammatory process.
