Sleep Science Editor It is estimated that a large portion of sleep disorders have gone undiagnosed in the general population. Sleep disorders have a negative effect on physical and mental performance. The most common form of sleep disorder is sleep apnoea and may affect up to 10% of the general population. This condition is most prevalent in the population of men over the age of 30. Could you suffer from sleep apnea? Dr. Farhad Hakimi, a board certified maxillofacial prosthodontist, began to study respiratory obstructive sleep disorders when a colleague referred a patient to him who suffered from obstructive sleep apnea but did not want to undergo surgery. Dr. Hakimi recognized that the problem for most people suffering from these disorders was the tongue sliding backward in the mouth. Using expertise gained from 15 years as a Professor of Dentistry at New York University, he designed an innovative oral device called the Snore/Stress Relieving Device (SSRD). UCLA study scrutinizes loss of cells in brain's breathing command-post
Aim to grow old and die peacefully in your sleep? Be careful what you wish for. A new UCLA study suggests that some people die in their sleep because they stop breathing due to a cumulative loss of cells in the brain's breathing command-post. The online edition of Nature Neuroscience reports the findings on Aug. 7. "We wanted to reveal the mechanism behind central sleep apnea, which most commonly affects people after age 65," explained Jack Feldman, principal investigator and Distinguished Professor of Neurobiology at the David Geffen School of Medicine at UCLA. "Unlike obstructive sleep apnea – in which a person stops breathing when their airway collapses central sleep apnea is triggered by something going awry in the brain's breathing center." Feldman's team had earlier pinpointed a brainstem region they dubbed the preBötzinger complex (preBötC) as the command post for generating breathing in mammals, and identified a small group of preBötC neurons responsible for issuing the commands. This time, the researchers studied the role of the preBötC neurons in generating breathing during sleep, and what would happen if these brain cells were destroyed.

Sleep apnea is a disorder that commonly affects more than 12 million people in the United States. It takes its name from the Greek word apnea, which means "without breath." People with sleep apnea literally stop breathing repeatedly during their sleep, often for a minute or longer and as many as hundreds of times during a single night. Sleep apnea can be caused by either complete obstruction of the airway (obstructive apnea) or partial obstruction (obstructive hypopnea hypopnea is slow, shallow breathing), both of which can wake one up. There are three types of sleep apnea — obstructive, central, and mixed. Of these, obstructive sleep apnea (OSA) is the most common. OSA occurs in approximately 2 percent of women and 4 percent of men over the age of 35. Causes The exact cause of OSA remains unclear. The site of obstruction in most patients is the soft palate, extending to the region at the base of the tongue. There are no rigid structures, such as cartilage or bone, in this area to hold the airway open. During the day, muscles in the region keep the passage wide open. But as a person with OSA falls asleep, these muscles relax to a point where the airway collapses and becomes obstructed. When the airway closes, breathing stops, and the sleeper awakens to open the airway. The arousal from sleep usually lasts only a few seconds, but brief arousals disrupt continuous sleep and prevent the person from reaching the deep stages of slumber, such as rapid eye movement (REM) sleep, which the body needs in order to rest and replenish its strength. Once normal breathing is restored, the person falls asleep only to repeat the cycle throughout the night.

About the Doctor
Dr. Strauss, a graduate of Temple University School of Dentistry (1968) has been in private practice in Northern Virginia since 1973. Active in the dental management of sleep disorders since 1988, Dr. Strauss is a member of most major international, national, regional and local sleep disorders associations. He co-founded and maintained an active leadership within the Sleep Disorders Dental Society (SDDS) that is now the Academy of Dental Sleep Medicine (ADSM). As a past-president and Standards of Practice Committee chairperson, Dr. Strauss has maintained a leadership role in ongoing improvement of the quality of care.
Dr. Strauss developed the original Oral Appliance Therapy (OAT) information brochure and membership brochure for the SDDS and was a contributing author to their initial education slide program. He is certified in the use of all basic types of oral appliances used in the management of sleep disordered breathing and has developed several of his own appliance designs. He is credentialed by the Certifying Board - Academy of Dental Sleep Medicine. Dr. Strauss has been a contributing author to the medical text, "Snoring and Obstructive Sleep Apnea," by David N. F. Fairbanks, M.D. As a part-time staff member of Crozer-Chester Medical Center and consultant on OAT to Walter Reed Army Medical Center, Dr. Strauss has been involved in both research and in clinical practice. He provides OAT for patients working in conjunction with most of the sleep disorders centers and sleep specialists throughout the Washington, DC metropolitan and suburban area.
