Soroka Medical Center, Beer-Sheva, Israel Published in Chest 118(4):1018-1024, October, 2000 SUMMARY The authors undertook to compare the severity (as distinct from the frequency) of apneic events in two sleeping positions: supine (on the back) and lateral (on the side). They examined the results of sleep polysomnography in 30 patients with Obstructive Sleep Apnea (OSA) of severe degree, with an average Respiratory Distress Index (RDI= number of apneas per hour of sleep) equal to 70. Their OSA was Nonpositional, that is to say, the RDI in the supine position was less than twice the RDI in the lateral position. Each subject had to have at least 30 apneas in the supine position and 30 in the lateral position during stage 2 (light) sleep. The 30 patients found to meet these criteria came from examination of sleep study results of 92 consecutive patients with Nonpositional OSA. These 30 subjects were predominantly male (26 or 87%), middle-aged (average=58 years), and overweight (average Body Mass Index=32). In the supine position, their average RDI was 86; in the lateral position, it was 65.

    My name is Doug Linder. I'm just some guy with apnoea. It's easier to tell you what I am not:
    • Most importantly, I am NOT a doctor. I do not keep lists of doctors, specialists, equipment companys, or anything like that. I can't recommend doctors or brands of CPAP machines. Sorry. I don't have a folderful of secret "cures" for apnoea that I keep away from everyone else just out of spite. The minute I hear of a new potential treatment, I put it up here. I'm not highly connected to the field of sleep medicine. Yes, I have met a few nice folks in the industry, and some of them are indeed doctors. But they don't give free, anonymous, undiagnosed medical advice over the internet, either.

Sometimes, though, apnea or other sleep-related problems can be a cause for concern. Find out what it means when your child's doctor mentions apnea. Types of Apnea
    The word apnea comes from the Greek word meaning "without wind." Although it's perfectly normal for everyone to experience occasional pauses in breathing, apnea can be a problem when breathing stops for 20 seconds or longer. There are three types of apnea:
    • obstructive
    • central
    • mixed
    Obstructive Apnea
    A common type of apnea in children, obstructive apnea is caused by an obstruction of the airway (such as enlarged tonsils and adenoids ). This is most likely to happen during sleep because that's when the soft tissue at back of the throat is most relaxed. As many as 1% to 3% of otherwise healthy preschool-age children have obstructive apnea. Symptoms include:
    • snoring (the most common)
    • labored breathing while sleeping
    • gasping for air
    • very restless sleep and sleeping in unusual positions
    • changes in color
    Because obstructive sleep apnea may disturb sleep patterns, these children may also show continued sleepiness after awakening in the morning and tiredness and attention problems throughout the day.     There can be large falls in arterial oxygen saturation during sleep in patients with chronic obstructive pulmonary disease. In general, these falls can be predicted from daytime oxygen levels and are not due to classical OSA syndrome, but to a number of factors which combine to produce worsening hypoxaemia during sleep. These patients are often at the limits of their respiratory capacity, and the loss of the intercostal and accessory muscle contribution to breathing, and the increase in upper airways resistance associated with REM sleep, cause marked falls in ventilation. In addition, patients may be hypoxic while awake and small falls in ventilation and PaO2 lead to large falls in arterial oxygen saturation during sleep. The hypoxic episodes demonstrated by overnight oximetry in a patient with chronic obstructive pulmonary disease differ in character from those seen in patients with OSA syndrome (figure 10). There is some evidence that increases in upper airway narrowing during sleep, short of apnoeas and even snoring, may contribute to the development of CO2 retention in these patients. However classical sleep apnoea is not significantly more common in unselected COPD patients with raised CO2 levels, and excessive nocturnal hypoxaemia is not one of the predictors of long- term survival in patients with COPD.

    Episodes of both apnea and hypopnea usually occur in the same patient with OSA, and both lead to oxyhemoglobin desaturation. Many laboratories report the average number of apnea-hypopnea events per hour of sleep as the apnea-hypopnea index (AHI). OSA is associated with excessive daytime sleepiness and altered cardiopulmonary function. Significant morbidity and mortality are associated with OSA, even for those under 50. Epidemiologic studies estimate that the condition affects 4% of middle-aged men and 2% of middle-aged women. However, a relatively small percentage of cases in this group of adults has been diagnosed, because of insufficient awareness of this disorder among physicians and the general public. Pathogenesis Soft tissue and skeletal factors determine upper-airway size and patency during sleep. In obese patients, increased adipose tissue in the neck may predispose the airway to closure. MRI has documented fatty infiltration into the pharyngeal tissue of obese patients with sleep apnea. Fatty infiltration has also been observed in histologic oropharyngeal specimens from OSA patients, but not from controls. Also, superficial neck fat may compress the pharynx, thus narrowing the airway. Patients with normal body weight and sleep apnea may have tonsillar hypertrophy, macroglossia, or craniofacial skeletal abnormalities (e.g. retrognathia) or just craniofacial characteristics and bony configurations that predispose the airway to narrowing or closure in sleep.
