ANTERIOR KNEE PAIN
'Anterior knee pain syndrome' means 'the group of symptoms which are frequently associated with pain in the front of the knee'. It is not a specific disease, but rather a group of conditions with similar symptoms. It is important that the clinician (usually the physiotherapist in the first instance) understands the various separate problems that can cause pain in the front of the knee, so that the right questions are asked and the rights tests done to determine the exact cause of this particular patient's pain.
Common complaint
This is one of the most common of knee complaints and probably the one where clinicians most frequently fail to understand or fully assess the problem prior to undertaking surgery.

Abstract
A chief complaint for many patients is anterior knee pain. This is more common in females; the incidence is two to three times than that in males. The challenge for the physician is to determine the etiology, the diagnosis, and the best treatment. Chondromalacia Patellae
What is Chondromalacia patellae? Chondromalacia literally means "softening of the cartilage", and Patellae means "the knee-cap". So Chondromalacia patellae means "softening of the articular cartilage of the knee-cap." The articular cartilage is the cartilage lining under the knee-cap that articulates with the knee joint. Under normal circumstances, it is smooth and shiny, so that it glides smoothly along the articular groove of the femur as the knee bends. When it "softens", it may break down, causing irregularities along the undersurface of the patella. What causes Chondromalacia patellae? Chondromalacia patellae occurs in two distinct age-groups:
  • It can happen in the older age-group (in the 40’s and beyond) when the articular cartilage breaks down as part of the wear-and-tear process that occurs with the rest of the body. The patella cartilage is one of the earliest places where cartilage breakdown occurs, and is slowly progressive, leading to degenerative arthritis (osteoarthritis) in the knee joint.
  • It can occur frequently in teenagers (especially girls) when the articular cartilage "softens" in response to excessive and uneven pressure on the cartilage, due to structural changes in the legs with rapid growth, and muscle imbalance around the knee. During periods of rapid growth, especially in girls, any knee valgus (knock-knees) is accentuated, thereby increasing the

    The Role of the Knee Brace in the Prevention of Anterior Knee Pain Syndrome
    Shlomo BenGal, MPH, Joseph Lowe, MD, Gideon Mann, MD, Alexander Finsterbush, MD, and Yoav Matan, MD Published in The American Journal of Sports Medicine , January/February 1997 A prospective study of 60 young athletes (army personnel), 27 in the brace group and 33 in the non-brace control group, underwent a vigorous physical training regimen to determine the efficacy of a knee brace with a silicone patellar support ring (Bauerfeind GenuTrain). Due to dropouts, the results were considered significant only among male brace users. Data was collected in the 1st and 8th week of the study by investigators who did not know the identity of the candidates. Data included the following: personal estimation of pain levels, orthopedic examination, and documentation of physical fitness levels. The incidence of anterior knee pain syndrome increased with the intensity of exertion as the study progressed.

    (Runners Knee / Patellofemoral Pain Syndrome is also called Patellofemoral Stress Syndrome or Anterior Knee Pain Syndrome) Runners Knee / Patellofemoral Pain Syndrome is one of the most common causes of knee pain in all age groups, including teens and young adults. It can be chronic or intermittent. It is common in runners, hence the term "runners knee", but also occurs in inactive people. Runners Knee / Patellofemoral Pain Syndrome is a condition characterized by pain between the kneecap (patella) and thighbone (femur). Poor kneecap tracking is believed to be the main cause this condition. The kneecap (patella) slides over a groove on the thighbone (femur) as your knee bends and straightens. If, for example, the front thigh muscles (quadriceps) are weak or imbalanced, the resulting muscle imbalance can pull the kneecap to the left or right of the groove, causing pressure, friction, and irritation to the cartilage on the undersurface of the kneecap when the knee is in motion.

    Patellofemoral Pain Syndrome: A Review and Guidelines for Treatment

    A patient information handout on patellofemoral pain syndrome, written by the author of this article, is provided on page 2019. Managing patellofemoral pain syndrome is a challenge, in part because of lack of consensus regarding its cause and treatment. Contributing factors include overuse and overload of the patellofemoral joint, biomechanical problems and muscular dysfunction. The initial treatment plan should include quadriceps strengthening and temporary activity modification. Additional exercises may be incorporated as dictated by the findings of the physical examination. Footwear should be closely evaluated for quality and fit, and the use of arch supports should be considered. (Am Fam Physician 1999;60:2012-22.) P atellofemoral pain syndrome can be defined as retropatellar or peripatellar pain resulting from physical and biochemical changes in the patellofemoral joint. It should be distinguished from chondromalacia, which is actual fraying and damage to the underlying patellar cartilage. Patients with patellofemoral pain syndrome have anterior knee pain that typically occurs with activity and often worsens when they are descending steps or hills. It can also be triggered by prolonged sitting. One or both knees can be affected. Consensus is lacking regarding the cause and treatment of the syndrome.
