What is Anosmia?
In February 2004, users of zinc nasal sprays made by Zicam and Cold-Eeze began to complain they lost their sense of smell. The medical term for the loss of smell is called anosmia, a disorder that can be devastating and permanent. Zinc lozenges and oral sprays have been shown, in some studies, to shorten the duration of colds by a few days, though research on the effectiveness of nasal zinc products is mixed. Some studies show zinc nasal sprays can shorten the common cold, while others show no effect. Still, sales of the zinc nasal sprays continue to boom, even after the line in the Cold-Eeze products was discontinued in September 2004. Manufacturer Quigley Corporation said the discontinuation was because of financial reasons, but at the time, the FDA was reviewing anosmia reports and the company had become the target of lawsuits. There are over 200 medical conditions and many medications associated with olfactory changes and loss, so the anosmia could be coincidental to zinc nasal spray use, but they could also be the cause. Since there are no studies on whether zinc nasal spray users have a higher risk of anosmia, the debate can continue but lacks definitive evidence.

The law allows the sale of herbal products and homeopathic remedies unless the FDA proves them harmful. Just a few quality controls and labeling rules exist for these products, yet they can be found on the shelves among over-the-counter drugs that have been tested, approved and regulated by the FDA for safety and efficacy. Both Cold-Eeze and Zicam are popular homeopathic remedies that offer zinc gluconate products. In some studies, zinc lozenges and oral sprays have been shown to shorten the duration of colds by a few days, but research on the effectiveness of nasal zinc products is mixed. The exact number of people who suffer from a condition called anosmia, the loss of smell, is not known, but the National Institutes of Health believes about 2.5 million people suffer from partial or complete loss of smell. Some of these anosmia sufferers, in recent years, have alleged zinc nasal sprays were the cause of their permanent loss of smell. As of February 2004, the FDA was accepting complaints regarding zinc nasal sprays through its hot line from people suffering from anosmia. At the time, both companies said their products were safe and that they had no indication in any studies that their zinc nasal sprays had caused anosmia, but there were no rigorous studies allowing experts to make a scientific conclusion.

Consumers using Zicam Cold Remedy have reported anosmia, or a loss of smell. Although Zicam has an entire line of cold remedy products, the only one that has so far been linked to anosmia is the nasal gel or swabs, which are applied directly to nose. Zicam's maker continues to deny a link between its cold remedy product and anosmia, and issued a statement reaffirming the safety of intranasal Zicam Cold Remedy. According to reports, some consumers have experienced anosmia after using Zicam just once. The cases are all similar in description and the majority of the anosmia sufferers have not regained function. In the 1930s, zinc compounds were used in attempts to prevent polio. In around 10% of the people that had received the zinc compounds, anosmia was suffered permanently. While Zicam manufacturer Matrixx Initiatives says there is a significant difference in that Zicam contains zinc gluconate and not zinc sulfate as in the polio uses, consumers and lawyers are arguing differently. So far, four lawsuits have been filed against Zicam's maker because of anosmia complaints. The FDA is investigating the consumer anosmia complaints and has a hot line to accept any more complaints regarding Zicam.

This is associated with heaviness of the head, stuffiness of the nose and lack of concentration. On X- Ray (OM view ) there is no deviation of the septum...PNS are clear except a mild ethmoiditis.
I had prescribed antihistaminics, nasal decongestants and a nasal steroid spray with a hope to relieve the possible edema of the olfactory region. However even after two months although the nasal stuffiness is better, the sense of smell has not improved. The patient has, however, developed tinnitus for which I have prescribed cinnarizine.
Please advise me regarding the possible cause and the future course of action that I must take.

Normally, the only thing about anosmics that smells is their feet. However, joking apart, if you have a diminished and partial sense of smell then you suffer from a similar condition known as hyposmia. Numerous things can cause anosmia such as damage to the nose via chemical fumes, vapours or hot smoke. Other possible causes are head injuries, disease, or viruses. People can also be born with it, while others simply develop it over the course of their life.
