Ellis, MD Nasolacrimal Duct Obstruction The development of the lacrimal drainage system begins at approximately 6 week's gestation. Canalization of the ectodermal cord begins during the third month in the area of the medial canthus. Canalization progresses toward the eyelid margin and toward the inferior meatus. The lacrimal puncta normally opens with eyelid separation during the seventh month of gestation. The distal lacrimal drainage system may not become patent until birth or shortly after birth. Blockage of the nasolacrimal system typically occurs in the distal portion where the tear duct opens into the nose beneath the inferior turbinate (Hasner valve). Because tears are not normally produced until a few weeks after birth, blockage of the lacrimal duct may not be recognized until several weeks after birth. Only about 1% to 6% of infants have symptomatic nasolacrimal duct obstruction. However, the majority of cases resolve by 1 year of age.

CORRECTION OF ANOPHTHALMOS WITH SILICON- AND HYDROXYAPATITE IMPLANTATIONS: A COMPARISON OF POSTOPERATIVE FINDINGSS A. G. Norda , J.-T. Vagt , J. Steinhauer , H.-W. Meyer-Rüsenberg In a retrospective clinical study, 49 anophthalmos patients were examined, who during the period between 1989 and 1995 had been provided after enucleation with either a sclera-coated silicon (n=26) or hydroxyapatite spheres (n=23). Aim of the investigation was to enable a postoperative comparison between intensity of enophthalmos, motility of artificial eye and implantation, local findings of the microscopic slit-lamp examination, and subjective and objective tolerance. The average observation time was 4,2 years for silicon and 1,8 years for hydroxyapatite implantation.
In contrast to the silicon implantation, there was significantly greater horizontal motility after hydroxyapatite implantation. Vertical motility and intensity of enophthalmos were not significantly divergent. Neither did the subjective satisfaction after questioning differ significantly in the statistics.
Complication-free healing processes were found in 88% of cases after hydroxyapatite implantation, and in 67% of the patient group with silicon prosthesis implantation. A post-enucleation socket syndrome occurred in 2% of silicon implantations, but in no case of hydroxyapatite implantation.

Anophthalmos (Loss of one eye)

It is very rare to be born with an eye missing, but an eye can be lost at any time in life due to trauma, infection or disease. When surgery is performed to remove a damaged eye the operation is called enucleation. Once the eye has been removed the surgeon inserts a prosthetic implant into the eye socket to create as normal an appearance as possible.
Eye implants
Eye implants can be made from several different materials. Common materials currently used include:
  • Hydroxyapatite, a calcium phosphate hydroxide compound which is derived from marine coral. This material is also found in bone. It is porous and allows blood vessels to grow around it, so that the implant essentially becomes part of the eye socket. Silicone Glass Grafts of fat from elsewhere in the body

We examined a newborn child with unilateral right-sided clinical anophthalmos born to a mother with unilateral left-sided anophthalmos. Although rare, isolated nonsyndromic heritable unilateral anophthalmia and microphthalmia have been reported in the literature. We briefly review the genetics of such anomalies and discuss the importance of a full clinical genetics evaluation. Treatment of this patient's anophthalmic socket consists of progressive conformer expansion to be followed by placement of a self-inflating polymer expander.
