    Unstable angina accounts for more than 1 million hospital admissions annually ; 6 to 8 percent of patients with this condition have nonfatal myocardial infarction or die within the first year after diagnosis.     To the Editor: Yeghiazarians and colleagues (Jan. 13 issue) reductase inhibitors (statins) in their interesting review of unstable angina pectoris. Evidence from a secondary-prevention 9000 patients, 36 percent of whom had been given a diagnosis of unstable angina between 3 and 36 months before study entry, showed that pravastatin significantly lowered the rate of hospitalization for unstable angina pectoris, with a relative reduction in risk of 12 percent (95 percent confidence interval, 4 to 19 percent; P=0.005).     Angina pectoris
    Angina pectoris er den medisinske betegnelse på en smerte som utløses fra hjertet på grunn av surstoffmangel i en del av hjertemuskulaturen. Angina pectoris betyr direkte oversatt "tranghet i brystet". På norsk bruker vi betegnelsen "hjertekrampe".
    Forekomst - hyppighet
    Angina pectoris hører til den gruppen åreforkalknings-sykdommer som er mest utbredt i de vestlige land. Norge har ca. 50.000 personer med angina pectoris bare i aldersgruppen 45 til 69 år. Hvert år rammes ca. 6.000 nye av sykdommen. Hyppigheten tiltar med økende alder, og den er større hos menn enn hos kvinner. Denne forskjellen reduseres etter hvert som årene går. Norge hører til de land i verden hvor sykdommen er mest utbredt.
    Årsaker
    Det er ikke mulig å peke på en enkelt årsak til åreforkalknings-sykdommene, og dermed angina pectoris. Det er mange grunner til at pulsårene i hjertemuskelen forsnevres. Noen av årsakene er kjent, noen er en i tvil om og mange er sikkert ennå ukjente.

    ¡¡¡¡stagnancy of Qi and blood, or obstruction of the heart Orifice due to blood stasis or turbid phlegm. Based on TCM differentiation, the author divided angina pectoris of coronary heart disease into the following types, namely stagnation of Qi and blood, deficiency of Yin , insufficiency of Yang , deficiency of both Yin and Yang , and stagnation of Qi and phlegm-dampness. Using acupuncture treatment to relieve the symptoms, the author gained quite a good short-term therapeutic effect. Some examples are introduced in the following. Stagnation of Qi and Blood

Physiological Considerations The immediate, apparent physiological cause of the condition we know of as angina pectoris is in nearly all cases an imbalance between the central or deep circulation and the superficial circulation. The latter is the supply of blood to the extremities and the superficial portions of the body, while the central or deep circulation is that which is supplied to the internal organs. In the case of angina the blood supply to the liver, to the lungs, and to the heart are closely related. The control and the balance achieved between these two circulations are located in the sympathetic ganglia in the upper dorsal region. The control of the blood flow through the liver, the lungs, and the heart lies in a nerve plexus in the aorta. This certainly is also under a higher control from the sympathetic ganglia. It can be seen that anything which would disturb the balance which is normally achieved between the cerebrospinal and the autonomic nervous systems would in turn cause a disturbance of the circulation if this imbalance had occurred in the upper dorsal area. The disturbance in the case of angina would cause an excess of blood flow to the heart, liver, and lungs particularly with a cessation or a diminished blood flow to the extremities of the body, thus often causing the feet to be cool or cold.

    Herzbeschwerden können harmlos sein, wie z. B. durch Aufregung, Kaffee oder Stress hervorgerufenes Herzklopfen; sie können aber auch sehr ernsthaft oder sogar lebensbedrohend sein. Koronare Herzkrankheit (KHK) und Angina pectoris Die häufigste Erkrankung des Herzens ist die sogenannte koronare Herzkrankheit, die in den Industrieländern zu den häufigsten Todesursachen zählt. Sie wird durch eine Verengung der Herzkranzgefäße verursacht. Durch den unzureichenden Blutfluss wird der Herzmuskel nicht mehr ausreichend mit Sauerstoff versorgt (siehe auch Arterienverkalkung ). Typische Beschwerden sind Atembeklemmung, Engegefühl und Schmerzen in der Brust. Manchmal strahlen diese Schmerzen auch in den linken Arm, die Schulter, den Hals oder in den Bauch aus und werden dann nicht sofort mit dem Herzen in Verbindung gebracht. Man nennt diesen Beschwerdekomplex, der häufig bei körperlicher oder psychischer Belastung auftritt und nach einer Ruhepause wieder verschwindet, Angina pectoris (wörtlich: Brustenge).
