Vascular and Interventional Radiology at NYU Medical Center Timothy Clark, M.D., Section Chief Interventional radiology is a specialty that combines state-of-the-art techniques with advanced surgical instrumentation in order to provide minimally invasive solutions to problems that in the past required extensive open surgery. This approach was first employed for arterial and venous disorders (endovascular surgery) in the 1960s, and it has since been expanded to treatment of a wide range of conditions involving the liver, kidney, and reproductive systems. One of the first services of its kind in the United States , the Interventional Radiology section at NYU is today one of the busiest centers in the country. Many techniques and devices currently used around the world were pioneered at NYU, including instrumentation used in angioplasty and stenting, endovascular grafting for abdominal aneurysm repair, and advanced embolization techniques used to treat many benign and malignant conditions. At NYU, a strong emphasis is placed on close collaboration between various medical and surgical specialists to provide the least invasive and most effective treatment for any given problem. While we offer the most advanced technology, the comfort and well-being of the individual patient always comes first.

Abdominal Aortic Aneurysm and Low Back Pain
Norman Engel, MS, DC, DABCO An aneurysm, by definition, is a sac formed by the dilatation of the wall of an artery, a vein or the heart. The chief signs of an arterial aneurysm are the formation of a pulsating tumor and often a bruit is heard over the swelling. Sometimes there are symptoms from pressure on contiguous parts. The aorta is the largest artery in the body. It arises from the left ventricle, ascending and bending over the heart, descending anteriorly to the thoracic and lumbar vertebrae. It splits at L-4 into the common iliacs, femorals, tibials, and dorsalis pedis arteries, supplying the lower extremities. The average diameter of aorta is 2.0-2.5 cm. More than 90 percent of abdominal aneurysms are associated with atherosclerosis and can cause leg pain (claudication), numbness or fatigue. An artery may be sclerosed (hardened) but if the lumenal diameter isn't altered, there is no aneurysm. Most abdominal aortic aneurysms do not produce symptoms, but some people feel a pulsating sensation in the abdomen. These silent (asymptomatic) aneurysms are often recognized by careful physical examination. Abdominal aortic aneurysm is most likely to occur in people older than 60 years, and it affects men more often than women.

    What is a Brain Aneurysm?
    A brain aneurysm, also called a cerebral or intracranial aneurysm, is an abnormal bulging outward of one of the arteries in the brain. It is estimated that up to one in 15 people in the United States will develop a brain aneurysm during their lifetime. Brain aneurysms are often discovered when they rupture, causing bleeding into the brain or the space closely surrounding the brain called the subarachnoid space, causing a subarachnoid hemorrhage. Subarachnoid hemorrhage from a ruptured brain aneurysm can lead to a hemorrhagic stroke, brain damage and death. The main goals of treatment once an aneurysm has ruptured are to stop the bleeding and potential permanent damage to the brain and to reduce the risk of recurrence.     Abdominal Aortic Aneurysm
    Bandolier has been asked by a GP whether there is enough evidence about the effectiveness of screening for abdominal aortic aneurysm (AAA) for it to be introduced into his practice. To try and answer this we searched MEDLINE from 1993 to the present to see if there were any new reports which helped. There was no single source of information which brought this subject together, but we did find some interesting papers.
    RCT for AAA
    A recently published English randomised trial in Chichester [1] has tried to examine the incidence of rupture following AAA screening. It was the most useful report and worth examining in some detail.
    The setting was 15,775 men and women aged 65-80 years identified from GP registers and FHSA lists. These people were then randomised by computer to control and screening groups. Those randomised to screening were invited for abdominal ultrasonographic scanning by letter from their family practice; one reminder was sent if there was no reply.
    Aneurysm was defined as maximum aortic diameter of 3 cm or more. Annual re-scanning took place if the diameter was 3 - 4.4 cm, and at three-monthly intervals if it was 4.5 - 5.9 cm. This protocol was continued until February 1994 or until the patient died, underwent surgery or declined further follow-up. Aortic diameters of 6 cm or more, and increase of diameter of 1 cm or more in a year, or developments of symptoms attributable to the aneurysm all constituted criteria for considering surgery.

They were the best of times. Then last year, while on a cruise, she received news that threatened to topple her happy existence: Her sister had suffered an aneurysm and was in a coma. Her twin sister. The aneurysm, a blister on a blood vessel on the brain, proved fatal for Ardith Eastlund's sister. Like 40 percent of people who suffer an aneurysm, her sister never awoke; she lingered for a week. A new procedure called "coiling" fills the aneurysm with tiny metal coils.

    The ballooning of the wall of the body's main artery that extends into the abdomen September 29, 1999 Web posted at: 2:20 p.m. EDT (1820 GMT) (CNN) People with abdominal aortic aneurysms, called AAAs, have until now had few treatment options besides a grueling surgery. But two new devices, approved by the Food and Drug Administration Tuesday, may offer a new way to repair this very dangerous bulge in an artery. An AAA is a weak area that develops in the wall of the main artery that brings blood flow into the abdomen. If the aneurysm breaks, it can cause almost instant fatal internal bleeding. Many people with AAAs have no symptoms before the aneurysm bursts, but each year roughly 200,000 Americans are diagnosed with the condition. And for that group, the FDA approved two repair patches doctors can insert through a small incision in the leg.
