For weeks she desperately searched for someone who would accept the organs of her unborn baby for transplantation after it was born. But because of technicalities in the laws regarding organ donation and the definition of brain death, her efforts almost came up empty. yes no Anencephaly is a maldevelopment of the brain and skull which occurs in the first weeks of pregnancy. The upper part of the brain and the overlying skull cap are absent although the lower part of the brain and the base of the skull do develop to some extent. Female babies are more commonly affected than males. This abnormality occurs in about 1:1,000 pregnancies. Its incidence is highest in those areas where spina bifida and hydrocephalus are common and, in fact, anencephaly and spina bifida can be present in the same infant. If the pregnancy does go to full term the baby is either still born or dies shortly after birth. Understandably most parents opt for termination of pregnancy once the diagnosis has been made. During subsequent pregnancies scanning is carried out to make sure the abnormality has not reoccurred. Inheritance patterns
The risk of anencephaly and spina bifida occurring is 1:50 if there has been one previously affected pregnancy but the risk increases to 1:5 if there have been two previously affected pregnancies. It is therefore essential that the mother takes a folic acid supplement before conception and for the first twelve weeks of any subsequent pregnancy.

Anencephaly is due to failure of the anterior neural tube to close properly during very early intrauterine life.
Mothers with infants having neural tube defects have high levels of alpha feto-protein. Pathogenesis
Anencephaly has been related to low levels of folic acid, obesity, hypervitaminosis A and previous infants with neural tube defects.
The lesion is sporadic and has no chromosomal abnormality as far as is known., Epidemiology
Anencephaly is seen in the United States at a frequency of 1:1000 births.
However, if a woman has an infant with a neural tube defect, her chance of a second infant having the defect is 1:50, if she has 2 infants with the defect her chance of a third is 1:25 etc. General Gross Description
In anencephaly, the infant is born with no cerebral hemispheres, only a rudimentary brainstem and brainstem, and no calvarium (skull bone).
Often there are associated malformations such as cleft lip and palate, abnormalities of hands and feet and other internal malformations.
Usually associated with bulging, frog-like, eyes.

As with those presented in this issue, some will focus on a particular topic, while in other instances two or three interesting but utterly unrelated examples will be explored. The series of cases presented here center on issues related to neural tube defects. Let us know whether you think these case synopses are or are not of value to you. Case 1:
This infant was born at 30 weeks gestation following an uneventful pregnancy during which prenatal care was begun in the 25th week. Because of the lateness of onset of this care, no maternal serum screening was completed. When presenting in labor, intrauterine heart tones could not be detected. The female baby was stillborn with obvious malformation of the calvarium. Clinical examination showed a large cranial defect with exposed but underdeveloped neural tissue with an apparent membrane covering. No other defects were noted. Photographs also show shallow orbits and prominent globes as well as prominent, mildly malformed ears. Post mortem assessment confirmed that this baby had profound underdevelopment of the brain and no other relevant features. Chromosome evaluation demonstrated a normal, female karyotype (46, XX).
