Introduction
An anal fissure (AY-nul FISH-er) is a tear in the anus causing a painful linear ulcer at the margin of the anus. An anal fissure, also known as fissure-in-ano, may cause itching, pain or bleeding. Either extreme constipation or diarrhea , usually combined with nervous tension over a prolonged period of time, may produce anal abrasions, simple slit-like fissures, or acute ulcers at the anal verge. With constipation, this condition is usually caused by the passage of a hard dry stool that tears the anal lining upon deification. An anal fissure is a fairly common, painful condition in which the lining of the anal canal is torn. The anal canal is the last part of the rectum which ends at the anus. Stool passes through the anal canal and anus during a bowel movement. An anal fissure is caused by constipation or a forceful bowel movement, though a tight anus also may be a contributing factor. Once the skin is torn, each subsequent bowel movement can be painful, and the pain often is severe. There often is bleeding associated with the painful bowel movement. The amount of bleeding is small and may be noticed in the toilet bowl or on the toilet paper as bright red in color. The symptoms of an anal fissure are commonly mistaken for hemorrhoids , but hemorrhoids generally do not cause pain with bowel movements.

An anal fissure is a small split or tear in the anal mucosa that may cause painful bowel movements and bleeding . There may be blood on the outside of the stool or on the toilet tissue following a bowel movement.
Causes, incidence, and risk factors: Anal fissures are extremely common in young infants but may occur at any age. Studies suggest 80% of infants will have had an anal fissure by age one. Most fissures heal on their own and do not require treatment, aside from good diaper hygeine. However, some fissures may require medical treatment. The incidence of anal fissures decreases rapidly with age. Fissures are much less common among school-aged children than among infants. In adults, fissures may be caused by constipation , particularly when passing large, hard stools, or by prolonged diarrhea. In older adults, anal fissures may be caused by decreased blood flow to the area. Anal fissures are also common in women after childbirth and people with Crohn's disease

What is an anal fissure?
An anal fissure is a tear in the lining of the lower rectum ( anus ) that causes pain during bowel movements. It is a common condition. Anal fissures do not lead to more serious conditions such as colon cancer. Most anal fissures heal with home treatment after a few days or weeks (acute anal fissures). Occasionally a fissure takes more than 6 weeks to heal (chronic anal fissure), in which case it usually requires medication to heal. Surgery may be necessary for fissures that don't heal with medication. Anal fissures affect people of all ages, particularly young and otherwise healthy people. They are equally common in men and women. Sometimes an anal fissure and a hemorrhoid develop at the same time.
What causes an anal fissure?
Anal fissures are caused by vigorous stretching of the anal canal. A fissure may develop if you are constipated and try to pass a large, hard stool; if you don't relax your anal muscles (sphincter muscles) during a bowel movement; or sometimes after repeated diarrhea.

An anal fissure is a tear in the lining of the anal canal usually in the posterior portion, often the result of constipation and straining to have bowel movements. Fissures also can be caused by anal trauma (including anal sex) and laxative abuse, and can occur during childbirth. Less commonly they can result from inflammatory bowel disease, sexually-transmitted diseases, or scarring, especially after prior surgical procedures. Symptoms are sharp rectal pain and bleeding, usually during or following a bowel movement. The pain may be brief or can be a dull aching discomfort that lasts several hours. You may see blood on toilet paper or on the stool. Occasionally, anal itching is the major symptom, in which case a mucous discharge from the rectum usually is present.

Anal Fissure An anal fissure is a small tear or cut in the skin around the anus which can cause pain and/or bleeding. Due to the location of fissures, they become aggravated during and after bowel movements. Symptoms of an anal fissure include pain during defecation and red blood in the stool. Constipation or diarrhea can cause a tear in the anal lining resulting in a fissure. A chronic fissure may be due to poor bowel habits, tight or spastic anal sphincter muscles, scarring or an underlying medical problem. Historically chronic fissures were treated with surgical disruption or division of the internal anal sphincter muscle. New medications include topically applied Nitroglycerin and Nifedipine, both of which may be used to decrease anal muscle spasm. Botox A, which needs to be injected, also decreases anal muscle spasm and may reduce the need to treat fissures surgically. Over 90% of anal fissures heal without surgical intervention. Your physician may suggest a high fiber diet and/or stool softeners to avoid constipation so that passage of soft stools may occur. Sitz baths may also be recommended.

This is a very common condition, and is a small tear in the lining of the anal canal, which can cause pain, bleeding and itching. It is commonly caused by constipation, but often occurs after a bout of diarrhoea or for no obvious reason. Rarely anal fissures can be associated with more serious inflammatory bowel conditions. Probably about half of fissures will heal with little difficulty. Taking Ispaghula husk, which is a bulk forming laxative may aid the healing process, particularly if your diet is low in fibre. An anaesthetic cream is sometimes prescribed if there is a lot of pain, but sensitivity to these preparations may occur. Chronic fissures (a fissure that has not healed in about 6 weeks) may require a more active intervention. Traditionally this involves surgery usually with some form of stretching of the anal sphincter. More recently, Glyceryl trinitrate ointment (GTN) has been used with reasonable success. This is an unlicensed treatment in the UK, and you will need to discuss it's use with a medical practitioner.

An anal fissure is a tear or crack in the lining of the anus. An anal fistula is an abnormal passage like a tube. One opening is in the anal canal and the other usually in the skin near the anus. A fissure may be caused by:
* passage of large stools,
* strain of childbirth,
* diarrhea, or
* insertion of an object. A long-term form of fissure is a type of ulcer formed by scarring from a local infection. Fissures are extremely painful. Pain is often most severe during bowel movements. They can cause painful spasms of the muscles around the anus. One may notice spots of bright red blood in the stool. There is often a hemorrhoid-like thickening of the mucous membrane at the lower end of the fissure. The fear of pain during a bowel movement can cause constipation. Fissures usually clear if bowel movements are kept regular and soft. Stool softeners such as bran or psyllium seed preparations can help. Suppositories are used if these measures do not work. Anesthetic ointment or solutions that arrest bleeding often help. Warm sitz baths for 15 minutes after each bowel movement will give some relief. In extreme cases when these measures fail, the fissure is repaired surgically. Most fistulas start from small pouches called crypts. Symptoms of a fistula are:

Introduction
An anal fissure, or split in the skin of the distal anal canal, is a common problem that causes substantial morbidity in people who are otherwise healthy. The incidence of anal fissure is similar in men and women. The majority of fissures occur in the posterior midline of the anal canal and are usually the result of tearing of the skin from hard stool or other physical irritants. Multiple fissures or lateral fissures may have other causes, such as Crohn's disease, ulcerative colitis, tuberculosis,infection with human immunodeficiency virus (HIV) or syphilis. Spasm of the anal sphincter has been noted in association with anal fissure, and for many years treatment has focused on alleviating spasm of the sphincter. Since 1951, the most common treatment for chronic anal fissure in the United States and Europe has been lateral internal sphincterotomy. This involves cutting the internal sphincter on the right or left mid-lateral area to break the circular anatomy of the muscle permanently. Although the technique is simple and effective, the fundamental drawback of this surgery is its potential to cause minor but sometimes permanent alterations in the control of gas, mucus and, occasionally, stool. This can sometimes become a major problem, particularly in elderly patients or those with diarrhea, irritable bowel syndrome, diabetes or recurrent fissure after previous surgery.

Cochrane reviews are regularly checked and updated if necessary. Background: Because of the disability associated with surgery for anal fissure and the risk of incontinence, medical alternatives for surgery have been sought. Most recently, pharmacologic methods that relax the anal smooth muscle, to accomplish reversibly what occurs in surgery, have been used to obtain fissure healing. Objectives: To assess the efficacy and morbidity of various medical therapies for anal fissure. Search strategy: Search terms include "anal fissure randomized". Selection criteria: Studies in which participants were randomized to a non-surgical therapy for anal fissure. Comparison groups may include an operative procedure, an alternate medical therapy or placebo. Chronic fissure, acute fissure and fissure in children are included in the review. Atypical fissures associated with inflammatory bowel disease or cancer or anal infection are excluded. Data collection and analysis: Data were abstracted from published reports and meeting abstracts, assessing method of randomization, blinding, "intention to treat" and drop-outs, therapies, supportive measures (applied to both groups), dosing and frequency and cross-overs. Dichotomous outcome measures included Non-healing of the fissure (a combination of persistence and recurrence), and Adverse events (including incontinence, headache, infection, anaphylaxis). Continuous outcome measures included measures of pain relief and anorectal manometry.

After lurking about in the wings the required 2 months I have felt the need to tell you about my anal fissure Bob.
It all started about two years ago in Thailand. I had just fired a round of green chile liquishit down the hole that the Asians call "toilet" when I noticed an odd sensation just inside the rim of my sphincter accompanied by a blasting spray of rich red blood.
After living in Asia for six months I thought that I had experienced nearly every digestive tract malady known to man. Worms, burning and colonic liquidity on a huge scale. Butt (hehe) this was something completely different.
It was a singularly unique feeling that I know now to have been the actual tearing of my rectum. It was Bob making himself known to me.
At first Bob wasn't so bad. Occasional itch and discomfort. Nothing that I couldn't handle. A mint flavored suppository now and again seemed to do the trick. But then about a year ago my cruel master Bob began requiring more and more from me. Itching on a scale that can only be desribed as "hellish" was the order of the day. I had a permanent brown stain on my index finger from trying to scratch the inside of my colon through my troubled anus.
