An anal fissure is a cut or tear that occurs in the anus (the opening through which stool passes out of the body) and extends upwards into the anal canal. Fissures are a common condition of the anus and anal canal and are responsible for 6-15% of the visits to a colonic and rectal (colorectal) surgeon. They affect men and women equally and both the young and the old. Anal fissure is the most common cause of rectal bleeding in infancy. Fissures usually cause pain during bowel movements that often is severe. Anal fissures occur in the specialized tissue that lines the anus and anal canal, called anoderm. At a line just inside the anusreferred to as the anal verge or intersphincteric groovethe skin (dermis) of the inner buttocks changes to anoderm. Unlike skin, anoderm has no hairs, sweat glands, or sebaceous (oil) glands and contains a larger number of somatic sensory nerves that sense light touch and pain. (The abundance of nerves explains why anal fissures are so painful.) The hairless, gland-less, extremely sensitive anoderm continues for the entire length of the anal canal until it meets the demarcating line for the rectum, called the dentate line. (The rectum is the distal 15 cm of the colon that lies just above the anus and rectum and just below the

Anal Fissure, Abscess and Fistula The intestinal tract (or bowel) ends with the rectum. The last part of the rectum is a section about 1 1/2 inches long, known as the anal canal. It ends with the anus the opening to the outside of the body. There are several common problems, including hemorrhoids, that can occur in the area from the rectum to the anus. While almost everyone has heard of hemorrhoids, the other conditions are not so well known. Anal Fissure
Most people have experienced a tear or fissure at the corner of the mouth that can occur in cold weather or when yawning. Similarly, an anal fissure is a small tear in the lining of the anus, frequently caused by constipation. A hard, dry bowel movement results in a break in the tissue. However, fissures can also occur with severe bouts of diarrhea or inflammation. This results in the anus becoming dry and irritated, causing it to tear. Injury to the anal area during childbirth and abuse of laxatives may be other causes. A fissure can be quite painful during and immediately following bowel movements. This is because the anus and anal canal are ringed with muscles to control the passage of stool and to keep the anus tightly closed at other times. When those muscles expand, it stretches the fissure open. There may also be bleeding or itching with an anal fissure.

An anal fissure is a tear in the lining of the anal opening. Almost always the tear results from a combination of two factors:
  • irregular bowel patterns (either hard or liquid stools) and increased anal sphincter tone (usually associated with stress). When hard stools pass through a tight anal opening, a laceration may occur and that is referred to as a fissure. Fissures can also be caused by anal trauma, laxative abuse, childbirth trauma, or laceration by a foreign object. Fissures are not cancerous.
  • WHAT ARE THE SYMPTOMS OF AN ANAL FISSURE?
    Common symptoms include rectal pain that may be worse with bowel movements, rectal burning, bleeding (especially blood on the tissue paper), itching and/or a tearing sensation. HOW IS AN ANAL FISSURE DIAGNOSED?
    Fissures can often be identified through anal inspection and digital rectal examination. The anal examination includes the use of an anoscope to directly visualize the fissure. HOW IS A FISSURE USUALLY MANAGED OR TREATED?

    What is an anal fissure?
    An anal fissure is a tear in the lining of the lower rectum ( anus ) that causes pain during bowel movements. It is a common condition. Anal fissures do not lead to more serious conditions such as colon cancer. Most anal fissures heal with home treatment after a few days or weeks (acute anal fissures). Occasionally a fissure takes more than 6 weeks to heal (chronic anal fissure), in which case it usually requires medication to heal. Surgery may be necessary for fissures that don't heal with medication. Anal fissures affect people of all ages, particularly young and otherwise healthy people. They are equally common in men and women. Sometimes an anal fissure and a hemorrhoid develop at the same time.

These tears usually develop when anal tissue is damaged during a hard bowel movement or when higher-than-normal tension develops in the anal sphincters. Symptoms of an anal fissure include a sharp, stinging, or burning pain during a bowel movement. The pain, which can be severe, may last a few hours. You may also notice spots of bright red blood on toilet tissue. This blood is separate from the stool. Minor anal fissures can often be helped to heal by drinking more fluids, eating a high-fiber diet, allowing enough time for a bowel movement, and using stool softeners.

    Causes, incidence, and risk factors: Anal fissures are extremely common in young infants but may occur at any age. Studies suggest 80% of infants will have had an anal fissure by the end of the first year. Most fissures heal on their own and do not require treatment, aside from good diaper hygiene. However, some fissures may require medical treatment. The incidence of anal fissures decreases rapidly with age. Fissures are much less common among school-aged children than among infants. "sooorry guys, i gotta stay in, my aaaanal fisssure is acting up" would be the other end of that spectrum. and that's where i am- the other end. frankly this is an embarrassing illness, at least at first. you're sniggering at yourself, with your going on about ass-pain all the time. snigger, you say, about my butt! chortle. gee, you joke, someone really lit a fire under my.... after a while though, you stop making jokes. this is because you can't access the language centers of your brain anymore, which have been converted along with everything else into additional pain centers. i've become something of a connoisseur of pain in my time. it's been kidney stones, kidney infections, GERD , IBS, catheters of various stripes, a cone biopsy of the cervix without any anesthetic, childbirth without drugs, and two years of backfiled taxes- i feel as though i qualify as something of an amateur expert on the subject. if there was a job which only required sitting in a room and hurting i feel i would be eminently qualified to do so. it's not my kink or anything, i just know how to endure buttloads of senseless pain. let me assure you, anal fissures hurt like hell. you aren't a wimp, it's not in your head, other people aren't living with this all the time far more gracefully than you ever could. this shit hurts.

    How does it occur?
    Tears in the anal skin occur during bowel movements when hard, dry stools are passed. They may also be associated with:
    • straining at bowel movements when you are constipated

    What is an anal fissure?
    An anal fissure is a shallow tear or crack in the skin at the opening of the anus. More than 90% of teenagers with blood in their stools have an anal fissure. You may notice the following signs or symptoms:
    • The blood is bright red. The blood is only a few streaks or flecks. The blood is on the surface of the bowel movement (BM) or on the toilet tissue after wiping. You usually pass a large or hard bowel movement just before you notice the blood. Touching the tear causes mild pain.
