An anal fissure is a tear in the delicate lining of the anus. They're relatively common and are most often caused by a hard, dry stool as it's being passed. Diarrhoea and any inflammation in the same area can cause the lining to tear as well. With an anal fissure, passing bowel motions can be very uncomfortable. Bright red fresh blood will often appear in the toilet bowl or on the toilet tissue when you clean yourself, and gives people a real fright. The pain then eases off until the next time you open your bowels when back it comes again. For the pain, an anaesthetic cream or gel (available from the pharmacist) works well. Taking a sitz, or hip, bath is also recommended if you have the time. This involves sitting in a bath of warm water that covers the hips and buttocks a few times a day. To protect the fissure from the outside forces of clothes rubbing against it, use a barrier cream, such as zinc and castor oil, to protect it. Plenty of fibre from fruit, vegetables, wholemeal bread, breakfast cereals, dried figs and prunes, and at least eight glasses of water a day will keep the bowel motions soft so that further damage doesn't occur. While you're at the pharmacist, you may wish to get a gentle stool-softening liquid.

What are Anal Fissures?
An anal fissure is a tear in the lining of the anus that is associated with pain (especially during bowel movements), bleeding, and/or itching. While the exact cause of anal fissures is unknown, there is accumulating evidence that increased muscle tone (hypertonicity) of the internal, but not the external, anal sphincter is a predisposing factor in the formation of anal fissures, and possibly hemorrhoids. Blood vessels normally provide an adequate blood supply to the site where fissures occur. When sphincter tone is increased, the blood supply to the affected area is decreased and even minimal trauma, such as a hard stool, may increase the susceptibility to a fissure. In the US, approximately 235,000 anal fissures are diagnosed each year. About 60% of these fissures are acute and heal with conservative treatment. Stool softeners, topical anesthetics, sitz baths, and increased intake of dietary fiber are recommended to relieve pain, and prevent recurrence of the fissures. Anal fissures that do not heal within several weeks are considered chronic. Historically, chronic anal fissures have been treated surgically with a lateral internal anal sphincterotomy, in which the internal anal sphincter (IAS) is partially cut. While severing the IAS has been shown to promote the healing of a large percentage of chronic fissures, it has also been found to create some form of fecal incontinence in up to 35% of patients receiving this surgical treatment.

Anal Fissures
An anal fissure is a longitudinal tear in the distal anal canal, usually in the posterior or anterior midline. Anal fissures may be associated with secondary changes such as a sentinel tag, hypertrophied anal papilla, induration of the edge of the fissure, and anal stenosis. A patient with multiple fissures, or whose fissure is not in the midline, is more likely to have Crohn's disease. Treatment High fiber foods, warm sitz baths, stool softeners (if necessary), and daily application of 1% hydrocortisone cream to the fissure Lateral partial internal sphincterotomy is indicated when 4 weeks of medical therapy fails. Definition of hemorrhoids:

Anal Fissures Several muscles encircle the anal canal and work together to control bowel movements. The inner muscle, just beneath the lining of the anal canal, is called the internal (involuntary) anal sphincter. This muscle is usually covered by skin. When a bowel movement occurs, the muscle relaxes to allow the stool to pass. Tears that occur along the anal canal are called anal fissures. When a tear occurs, pain makes the muscle fibers contract, stopping the stool from passing. The fissure pulls apart when the muscle contracts (anal spasm). In turn, this affects the small blood vessels that carry nutrients and oxygen to the torn tissues. As a result, healing is slowed. Symptoms The pain and discomfort of an anal fissure usually gets worse when a person has a bowel movement. The pain tends to linger a long time afterward. There may be bleeding from the tear as well. Constipation may also occur as the condition gets worse. Causes and Risk Factors A fissure can occur from:
  • Passing a hard stool or prolonged episode of diarrhea Lack of fiber in the diet and/or water with that fiber Food that creates a rough passage through the digestive system, such as popcorn, nuts or tortilla chips

New toxin treatment for anal fissures
This article has no abstract so we have provided the first paragraph of the full text. Current treatments for anal fissures focus on reducing the tone of the internal anal sphincter, whether by sphincterotomy, injection of botulinum toxin or the topical application of nitroglycerin or nifedipine. The surgical procedure carries a risk of causing fecal incontinence or other side effects, and reports vary as to the efficacy of the alternative treatments. In their recent paper, Garrido and colleagues from Chile describe the successful use of a new agent, gonyautoxin, in patients with acute and chronic anal fissures.     One problem with a papercut is that every time you use your hands, you tend to break open the small wound which delays healing causes more pain. The same holds true with an anal fissure. Every time you have a bowel movement, the anus is stretched and the fissure can possibly be reopened causing more symptoms. This makes some anal fissures chronic and difficult to heal. This problem is quite common and may cause considerable misery.
    What causes an anal fissure?
    The anal opening is only so big. To have a bowel movement the anal sphincter relaxes and open up as much as it can, but the stool must be still be soft enough to squeeze through the opening.     Classification of fissures
    Fissures are graded as superficial or deep. A superficial fissure does not extend full thickness through the skin; a deep fissure results in exposing underlying muscle fiber. Fissures are also graded as acute or chronic. An acute fissure is less than thirty days duration; a chronic fissure is present for greater than thirty days.

    ANAL FISSURE AND FISTULAS The anus is the opening of the digestive tract where bowel movements leave the body. An anal fissure is a tear or crack in the lining of the anus. An anal fistula is an abnormal opening in the skin near the anus. An anal fissure may be caused by: * passing large bowel movements * straining at childbirth * diarrhea or * inserting an object in the anus ANAL FISSURES are extremely painful. Pain is often most severe during bowel movements. They can cause painful spasms of the muscles around the anus. Spots of bright red blood in the stool may be noticed. There is often a hemorrhoid-like thickening of the skin at the lower end of the fissure. The fear of pain during a bowel movement can cause constipation. Anal fissures usually resolve if bowel movements are kept regular and soft. Stool softeners such as bran or psyllium seed preparations can help. Suppositories can be used if these measures do not work. Anesthetic ointment or solutions often help relieve the discomfort, but repeated use of pain killing or numbing ointments can cause irritation or inflammation around the anus. Warm sitz baths for 15 minutes after each bowel movement will give some relief. In extreme cases, the fissure must be repaired surgically. Most ANAL FISTULAS start from small pouches called crypts. An abscess may occur before or after a fistula develops. Symptoms around the anus are:

    November 2003 An anal fissure is a tear in the skin around the opening of the anus. It can cause sharp pain, especially when opening the bowels. Anal fissure is thought to be a common disorder for which many people do not seek medical advice. It can be easily confused with haemorrhoids (piles).
    Symptoms
    Sudden and severe pain in or around the anus is the main symptom of an anal fissure. The pain often occurs during or shortly after a bowel movement, but may also occur at other times. This pain can be severe - and is often described as like passing razor blades. As a result, a fissure sufferer is likely to dread opening their bowels. An anal fissure may cause a "sentinel pile". This is a little tag of skin that develops on the edge of the anus where the fissure is. It may leave a streak of bright red blood on the toilet paper or drops of blood in the toilet bowl. A sentinel pile is not a haemorrhoid.
