Alternerende hemiplegi
Alternating hemiplegia of childhood (AHC)
AHC er et sjældent syndrom med kortvarige tilfælde med forbigående halvsidige lammelser, der varer fra minutter til dage. Betegnelsen alternerende hemiplegi betyder skiftende halvsidige lammelser. AHC begynder i småbarnsalderen 1-5 år. Der er ofte ledsagefænomener som ufrivillige bevægelser, balanceforstyrrelser og ukontrollerede øjenbevægelser. Desuden forekommer udviklingshæmning og anfaldslignende fænomener. De typiske anfald er halvsidige anfald af slappe eller stive halvsidige lammelser, som kan sprede sig til den anden side. Barnet er typisk vågent under anfald og anfaldet brydes typisk, hvis barnet falder i søvn. Der kan tillige ses choreatiske (vridende) bevægelser og anfald af dystoni (stivhed). Årsagen er ukendt, men familiære tilfælde kendes. Arvegangen er i familiære tilfælde formentlig autosomal dominant. Tilstanden betegnes som sjælden, og der er rapporteret under 100 tilfælde i litteraturen. Kilde: Omim nr. 104290

Sunday, March 20, 2005 By Bill Schlegl Herald-Whig Staff Writer When Michael Ryan was an infant, his mother noticed something about her baby that scared her. "At 2 months his eyes started jumping. I thought, 'This isn't right,' " Cindy Ryan said. She and her husband, Dave, who live in the Scott County town of Alsey, took Michael to a neurologist. The doctor diagnosed Michael with Alternating Hemiplegia of Childhood (AHC), a rare neurological disorder that causes temporary paralysis when the person gets too excited or is stressed. According to the Web site for the Alternating Hemiplegia of Childhood Foundation, the paralysis ranges from simple numbness in an extremity to full loss of feeling and movement. The attacks may last for minutes, hours or even days. The syndrome is so rare only 350 cases have been diagnosed worldwide, with 11 in Illinois, Cindy said. Michael, who is now 8, was late in every step of normal child development. It took him nine months to sit up, one year to crawl and two years to walk. "We were to the point we didn't know if he would ever walk. All of the sudden it just clicked. We're very thankful for that," Cindy said.

    prof.dr. R.R. Frants Korte samenvatting: Migraine is een complexe aandoening waar meerdere genen bij betrokken zijn en waarbij ook niet-erfelijke invloeden een rol spelen. De klinische verschijnselen lopen bovendien zeer uiteen. Om meer over het ontstaan van migraine te kunnen gaan begrijpen, nam Esther Kors een bepaalde vorm van de ziekte onder de loep, familiaire hemiplegische migraine (FHM). Het is een zeldzame erfelijke aandoening met een duidelijk overervingspatroon.
    Ze onderzocht een aantal families met verschillende mutaties in de twee genen en keek met welke verschijnselen die mutaties samengaan. Zelfs patiënten met eenzelfde mutatie, constateert ze onder meer, kunnen heel uiteenlopende klachten ontwikkelen. Samenvatting: Dit proefschrift beschrijft de resultaten van de klinische en moleculaire studies die zijn verricht in families met mutaties in de genen voor familiaire hemiplegische migraine, te weten het CACNA1A gen gelegen op chromosoom 19 (FHM1) en het ATP1A2 gen, gelegen op chromosoom 1 (FHM2). Tijdens de verschillende onderzoeken beschreven in het proefschrift is getracht een relatie te leggen tussen de klinische karakteristieken (fenotype) en de bevindingen op DNA niveau (genotype).
