General Discussion Alternating hemiplegia of childhood (AHC) is a rare neurological disorder characterized by frequent, temporary episodes of paralysis on one side of the body (hemiplegia). Symptoms usually begin before the age of 18 months. This syndrome may be characterized by temporary (transient) hemiplegia of varying degrees; temporary paralysis of the muscles that control eye movement (transient ocular palsies); sudden, involuntary movements of limbs and facial muscles (choreoathetosis); and/or excessive sweating with changes in skin color and body temperature (autonomic nervous system dysfunction). Mental capacity may be affected. The exact cause of AHC is unknown. Some cases of AHC may be inherited as an autosomal dominant trait. Resources Alternating Hemiplegia of Childhood Foundation (AHC)

: My son, now 6 years old, was diagnosed with partial seizures at age 14 months. He did pretty well the next two years while on phenobarbital. Since that time, he has had increasing "seizure" activity which was treated with tegretol and depakote. His seizure activity continued. EEGs have never shown epileptic waveforms. In July, he had an EEG and developed nystagmus and mouth sucking during photic stimulation but no epileptic waveforms were noted. The next day
he had a video EEG with him having 3 episodes that we had been calling seizures. Again the EEG did not show epileptic waveforms. After his seizures, my son would often experience weakness on one side of his body. These episodes have escalated to sometimes as many as 10+ a day, although on a rare day, he will have none. He was then diagnosed with alternating hemiplegia of childhood. He pediatric neurologist would like a second opinion from someone who has treated a
child with this disorder. We were originally going to take him to Detroit Childrens Hospital but feel that the six hour drive would be too much for him. Since Cleveland is much closer to us, we are wondering if any of your staff have cared for a child with this disorder. He also is developmentally delayed. Thank you.

This 26 year old lady teacher presented a month ago with sudden onset of headache, LOC and inability to speak with left hemiplegia. She regained consciousness after 24 hours and when I saw her she was alert, comprehension was intact and she was obeying simple commands, but was totally unable to speak. She had a left UMN facial paresis and dense left hemiplegia with pyramidal signs. After a week she could communicate with signs and then by writing with her right hand.Hemiplegia and motor aphasia remained and gradually started improving after 3 weeks. She can now talk but not fluently and is ambulant with support. No fever, heart problems, coagulation defects or HT .. ESR normal. other blood tests are normal. Past medical history : She had a similar attack when she was 7 years old (1983) but had motor aphasia and RIGHT hemiplegia at that time.She recovered fully. She had a seizure (generalised) in 2000 and was put on anticonvulsants. She was advised a test which she did not have done due to reasons I cannot fully fathom. She became a teacher and was doing her job well and otherwise was a bright and active lady till this attack. I have put up her 1983 CT of the brain ( not a good picture). what is your comment? She was asked to do an imaging study after thais but was not done. I will post her current CT after 2 days.

Division of Neurology and the Department of Paediatrics, The Hospital for Sick Children, Toronto ABSTRACT. In this report eight patients are described who suffer an unusual symptom complex of intermittent, alternating hemiplegia. Symptoms have begun in early childhood and in one child at age 3 months. Typically, there is a brief vasomotor prodrome and then the abrupt onset of hemiplegia. Contralateral headache tends to develop soon after the onset of neurological symptoms. A 14-month-old boy with alternating hemiplegia of childhood, an idiopathic disorder of early childhood causing episodic hemibody tonic spasms and hemiplegia, showed repetitive jerks of abduction of the ipsilateral eye during the spells.
