Postgrad Med 107(7):81-90 A lopecia areata, a dermatologic disorder that has been recognized for more than 2,000 years, is a common problem in primary care practice. The hair loss is believed to have an autoimmune basis and typically presents as patchy areas of hairlessness on various regions of the body. Severity ranges from localized alopecia areata, which accounts for most cases, to total loss of scalp hair (alopecia totalis) or loss of all body hair (alopecia universalis). Although other autoimmune disorders sometimes occur with alopecia areata, most patients are healthy. Autoimmune thyroid disease and vitiligo generally are found in less than 10% of patients with alopecia, and other autoimmune disorders are much less common (1).

Anxiety about balding prompts many people to seek solutions from their primary care physician. Unfortunately, physicians do not have a lot to offer except emotional support. In this article, Dr Rubin offers some plain talk about the bare realities of hair loss and the available treatment options. H air loss is universal and inevitable among all humans. Although the process is virtually the physiologic norm, hair has such a powerful role in a person's psychosexual self-image that anxiety about its loss may prompt him or her to seek medical attention. Obviously, hair loss must be evaluated during the patient's relatively brief visit, without the benefit of lengthy observation. Careful attention should be given to history taking, and worried patients should be allowed to express anxiety about the expected outcome (ie, rapid hair loss leading to total baldness). A thorough history usually helps focus on possible causes of alopecia, and the absence of clues to specific disease encourages patients to accept the diagnosis of androgenetic alopecia.
Genetic factors
There is no question that genetic factors have a role in the development of androgenetic alopecia, but specific hereditary patterns are a poor basis for predicting outcome in individual patients. Androgenetic alopecia is a multifactorial genetic condition whose phenotypic expression is so variable that accurate forecasting is impossible. Certainly, a 25-year-old man from a family in which all other men were totally bald by age 40 would be expected to have a greater chance of advanced alopecia than his age-matched counterpart with no family history of baldness. However, prediction of outcome in patients with a less obvious hereditary pattern is unreliable, and errors are likely. In fact, it is better to tell patients that, at this time, their family history can be considered only a general indication of what may be in store for them.

Alopecia areata, simply stated, is a bald patch of abrupt onset (see photos). This stands in stark contrast to typical "male pattern balding," which characteristically is a slow thinning of hair over the temples and/or the back of the head. In alopecia areata the extent of hair loss varies from a few to many centimeters. Ten percent of cases involve hair-bearing areas other than the scalp, such as the beard or the eyebrow. The scalp in alopecia areata is very smooth, non-scarred, and non-inflamed. Sometimes the area tingles or might be slightly tender. If the bald patch is red or scaly, alternative diagnoses should be sought. Cause
The cause of alopecia areata is unknown. However, it is felt to be related to abnormal T-cell function of the immune system. From time to time it is associated with other immune disorders, such as thyroid disease. Treatment
Spontaneous regrowth of hair in mild alopecia areata is likely. More than 50% of people with this disorder will experience regrowth of hair within one year. Therefore, for many, the treatment is "tincture of time." If the extent of hair loss is particularly disturbing, then certain medications may be of value: steroid or cortisone-like salves and injections may promote early hair growth. Also, topical minoxidil (generic or brand Rogaine) may be beneficial in about 30% of cases.

The human hair varies widely in its color and texture. Also thickness and the amount of body hair a person has depends on age, sex, race and their genetic makeup. Hair grows everywhere on the body except the palms of our hands and soles of our feet. Many hairs are so fine they are vitually invisible, while others are so course that we relax, bleach, dye or remove them to achieve a smoother look. Whether it is course, fine, thick, wavy, curly or straight, it is the hair on our heads that causes us the most concern. When we start to lose it there, it is considered alopecia. Basically hair is made up of a protein called keratin, the hair you can see above the skin level is a string of dead keratin cells. The average adult loses up to 100 of them a day. This is normal and even losing up to 300 a day can be alright, if the person has thick hair. A gradual thinning of the hair is natural in both men and women with aging. This is known as involutional alopecia. The hair follicles go into a telogenic or resting stage. The remaining hairs become shorter and fewer in number. Androgenic alopecia is a genetically predisposed condition that affects both men and women. Men can begin losing their hair as early as their teens or early 20's. A receeding hairline and gradual disappearance of hair from the crown is called male pattern baldness. Most women don't have noticable thinning hair until their 40's or later. In women, female pattern baldness is typically a general thinning over the entire scalp, with the most hair loss at the crown.

Alopecia is the medical term for baldness. According to the American Medical Association, 95% of cases involve androgenetic alopecia, also known as common baldness or, in men, male pattern baldness. Androgenetic alopecia is an inherited condition that affects about 25% of men before the age of 30 and two-thirds of all men before the age of 60. The condition is less common and less extreme in women. It can develop in older adults, resulting in an overall thinning of all the scalp hair rather than complete baldness. Alopecia areata and androgenetic alopecia are types of nonscarring alopecia, that is temporary hair loss that does not result from damaged hair follicles. Scarring alopecia is permanent hair loss that results from damaged hair follicles. There are other forms of scarring and nonscarring alopecia caused by skin diseases and underlying illness. Hair Hair is made up of keratin, a protein found in the nails and outer layer of the skin. Each hair is rooted in a hair follicle, a tiny indentation in the outer layer of the skin (epidermis). Hair follicles are made up of cortical, or matrix, cells that proliferate and keratinize into the hair.
