(formerly ARIFA). Alopecia means hair loss. Alopecia areata is a particular disease affecting hair growth in the scalp and elsewhere. This loss is the cause of great concern to those affected, since the hair is the most important part of our "body image". WHO GETS ALOPECIA AREATA? : The condition can commence in children, and is most common in teenagers and young adults. It can occur for the first time in old age. Most studies show that males and females are equally affected. IS IT INFECTIOUS? : No. There is no way that a person with alopecia can transmit the problem to anyone else, and it is not "caught" by using someone else's comb or towel. IS ALOPECIA AREATA INHERITED? : This condition usually affects one member of a family. As will be discussed later, some related conditions can appear in other family members, and occurrence of alopecia in relatives would be possible but unlikely. IS IT COMMON? : No figures are available to answer this question - but 1-2% of new patients seen by dermatologists have alopecia areata. It is perhaps best to say it is not "uncommon". HOW DOES IT START?

Alopecia
is a set of disorders ranging from male and female pattern alopecia ( alopecia androgenetica ), to alopecia areata , which involves the loss of some of the hair from the head, alopecia totalis , which involves the loss of all head hair, to the most extreme form, alopecia universalis , which involves the loss of all hair from the head and the body. Treatment for alopecia has limited success. The more hair lost, the less successful the treatment will be. Many doctors are unsympathetic towards the psychological implications of alopecia; which include stress, anxiety and depression, and can in many cases involve issues relating to identity change, particularly when the eyebrows and eyelashes are also lost. Hair loss is sometimes the result of chemotherapy treatment for cancer sufferers.

Introduction Alopecia affects both men and women and may result from a variety of causes. The cosmetic consequences may have a substantial emotional impact on the patient. As might be expected, many patients are not in the least content with their hair loss and seek the advice of a physician in search of a remedy. Depending on the cause and extent of the problem, medical and surgical therapies exist which may offer assistance for the patient experiencing alopecia. These modalities may be used alone or in combination, and the particular regimen must be individualized to meet the particular needs of the patient. The physician is obligated to ensure that the patient has a clear understanding of his/her condition and expectations in order for a realistic and thoughtful treatment plan to be devised. This presentation will consider the etiology of alopecia and patient selection with an emphasis on the medical and surgical management of the disorder.

History
Anatomy
The hair itself, or the shaft, is composed of keratin, and is essentially a collection of dead cells which are the product of the hair follicle. The hair follicle describes an invagination of the epidermis into the underlying dermis. The proximal end of the follicle has an onion-shaped dilatation called the bulb, which is hollow at its base and envelopes the dermal papilla. The follicle has two layers, the dermal layer (fibrous tissue) and the epidermal layer. Into each follicle, the arrectores pilorum muscle inserts at the dermal layer below the entrance of the sebaceous gland duct. The arrectore pilorus acts to elevate the hair shaft and express sebum.

(For latest news about Paul's hair - follow this link Jane started teasing me about my bald spot in June 1998. I didn't take it seriously at first, but in July I noticed my hairline was receding, too, and by the end of July, the rate of hair-loss was starting to seem a bit unnatural, not least because I was also losing my eyebrows! In August I went to a doctor, who sent me to a specialist, but by then my hairdresser had already made a diagnosis about my increasingly patchy hair - and the specialist didn't really tell me anything more than I already knew. It was Alopecia. In October I shaved off what was left, and a few weeks later I was more or less totally bald - all over. It took about four months...
