Clinical Features
Epidemiology: (Study of the rate of occurrence of a particular condition.) According to a recent study from the Mayo Clinic, the lifetime incidence rate of alopecia areata is 1.7% which makes it almost as common as psoriasis. Sixty percent of patients usually present with their first patch under the age of 20. Twenty percent present with their first patch over the age of 40 and twelve percent present with their first patch past the age of 50.
Presentation:
One can classify alopecia areata based on extent of disease or pattern of hair loss. Alopecia areata usually refers to varying amounts of patchy hair loss to larger areas of little or no hair: this kind of hair loss generally takes place on the scalp, but any hair-bearing surface can be affected. The term alopecia means hair loss. In the form of alopecia known as alopecia areata , round bald patches appear suddenly, most often affecting the scalp. What causes alopecia areata?
Alopecia areata is considered to be one of the autoimmune disorders - lymphocytes aound the hair follicles release chemical messengers (called cytokines) that reject the hair for unknown reasons. Alopecia areata may occur in more than one member of the family, and such families may develop other autoimmune diseases such as pernicious anaemia and vitiligo . It is also more common in patients with chromosomal disorders such as Down's syndrome. Alopecia areata is not contagious and is not caused by any food. Like many other health problems, it sometimes starts after a stressful event, but this is not usually the case.

What is male pattern hair loss (androgenetic alopecia)?
Hair loss resulting in thinning is known as alopecia . When it is related to hormones (androgens) and genetics, it is known as androgenetic alopecia , or more commonly just balding . Male pattern alopecia is characterised by a receding hairline and/or hair loss on the top of the head. A similar type of hair loss in women, female pattern alopecia , results in thinning hair on the vertex (top) of the scalp but is generally less severe than occurs in males.

What causes pattern balding?
Male pattern hair loss is an inherited condition, caused by a genetically determined sensitivity to the effects of dihydrotestosterone, or DHT. DHT is believed to shorten the growth, or anagen, phase of the hair cycle, causing miniaturisation of the follicles, and producing progressively finer hairs. The production of DHT is regulated by an enzyme called 5-alpha reductase.

A lthough alopecia can occur anywhere on the body, it is most distressing when it affects the scalp. Hair loss can range from a small bare patch that is easily masked by hairstyling to a more diffuse and obvious pattern. Alopecia in women has been found to have significantly deleterious effects on self-esteem, psychologic well-being, and body image. See page 907 for definitions of strength-of-evidence levels. Pathophysiology Every hair follicle continually goes through three phases: anagen (growth), catagen (involution, or a brief transition between growth and resting), and telogen (resting). Disorders of alopecia can be divided into those in which the hair follicle is normal but the cycling of hair growth is abnormal (e.g., telogen effluvium) and those in which the hair follicle is damaged (e.g., cicatricial alopecia).

What is alopecia areata? Alopecia areata is a hair loss condition which usually affects the scalp. It can, however, sometimes affect other areas of the body. Hair loss tends to be rather rapid and often involves one side of the head more than the other. Alopecia areata affects both males and females. This type of hair loss is different than male pattern baldness, an inherited condition. What causes alopecia areata Current evidence suggests that alopecia areata is caused by an abnormality in the immune system. This particular abnormality leads to autoimmunity. As a result, the immune system attacks particular tissues of the body. In alopecia areata, for unknown reasons, the body's own immune system attacks the hair follicles and disrupts normal hair formation. Biopsies of affected skin show immune cells inside of the hair follicles where they are not normally present. What causes this is unknown. Alopecia areata is sometimes associated with other autoimmune conditions such as allergic disorders, thyroid disease, vitiligo , lupus, rheumatoid arthritis , and ulcerative colitis. Sometimes, alopecia areata occurs within family members, suggesting a role of genes and heredity.

There are many kinds of hair loss, including what we call:
  • alopecia areata (patches of baldness that usually grow back);
  • telogen effluvium (rapid shedding after childbirth, fever, or sudden weight loss); and
  • traction alopecia (thinning from tight braids or ponytails).
All of these types of hair loss can be alarming but, fortunately, they are most often mild and temporary or reversible. The majority of people who lose their hair, however, do it gradually and all through the scalp, with some emphasis on the frontal part. Doctors refer to this process as "androgenetic alopecia" which implies that a combination of hormones
(andro-) and heredity (genetics) is needed to develop the condition.

Alopecia Areata I. Definition: Alopecia areata is the name for a condition in which round patches of hair loss appear suddenly. The hair-growing tissue is attacked by the patient's own immune cells for unknown reasons. There are three stages: first, there is sudden hair loss, then the patches of hair loss enlarge, and last, new hair grows back. This process takes months, sometimes more than a year, but rarely does the hair never grow back. II. Causes:
  • We do not know why the immune cells attack the hair-growing tissue. Alopecia areata is not contagious, not caused by foods, and is not the result of nervousness. Alopecia areata sometimes runs in families.
III. Treatment:
  • Cortisone injections often stimulate hair regrowth. Twenty to thirty injections per patch are required once a month. The injections are uncomfortable. Cortisone injections only work in the areas that have been injected; the injections do not prevent new areas of hair loss. It generally takes one to two months after the injection before hair growth is visible. Some patients do not respond to any treatment. We cannot predict which patients will respond to treatment.

Androgenic Alopecia I. Definition: Androgenic alopecia is the name for male and female pattern baldness. In androgenic alopecia the hair loss occurs slowly over years. It can start anytime after age twenty. There is usually a family history of hair loss. In women, the hair slowly becomes thin throughout the scalp and bald spots usually do not occur. Men first develop hair loss at the temples, followed by an enlarging bald spot on top of the head. II. Causes:
  • Androgenic alopecia is thought to be due to the hair growing tissue's sensitivity to hormones; this sensitivity is due to genetic factors. Not contagious, not caused by foods, not the result of nervousness. Androgenic alopecia often runs in families.
III. Treatment:
  • Rogaine (minoxidil) is a medicine used to treat high blood pressure which when applied to the skin causes hair growth in some individuals. One out of ten people who use Rogaine will grow hair. Rogaine will slow the rate of hair loss in five out of ten patients who use it. The other four out of ten are wasting their money. Hair transplants are another option for treating androgenic alopecia. Plugs of hair are removed surgically from areas with plenty of hair and implanted in the areas with little hair.
