Brain-injured children who progress from coma to a sleeplike state of akinetic mutism are often misinterpreted by their parents as having improved. In children with akinetic mutism caused by extensive destruction of the cerebral gray matter, the computed tomographic scan may demonstrate the irreversible nature of the pathologic changes.

Diagnosis:
Akinetic mutism , resulting from anterior communicating artery aneurysm rupture, with subarachnoid hemorrhage and anterior cerebral artery distribution infarcts. HOSPITAL COURSE: The patient convalesced and underwent extensive physical and occupational therapy, but remained akinetic and mute at the time of discharge, four months after her presentation. She remained in a rehabilitation facility for several months, and was discharged to home. Subsequent outpatient follow up has documented little significant improvement.
DISCUSSION: Clinical Presentation Akinetic Mutism Apathy, which is derived from the Greek word pathos, can be defined as the absence of feeling, interest or concern. Abulia, derived from the Greek word boul (will), is usually defined as lack of will or an inability to decide. C. Miller Fisher, in an presentation at a national Neurosurgery meeting, suggested that akinetic mutism represented abulia in its most extreme form. The differential diagnosis of the volitional disorders of movement or speech is vast. They may be seen as part of a recognized personality disorder, such as avoidant or schizoid personality disorders. They may develop as a response to socio-environmental factors such as imprisonment, retirement, or a natural disaster. Apathy is frequently seen in affective and psychotic disorders such as major depression and schizophrenia.

    Akinetic mutism is a variety of stupor in which the patient is unable to talk or carry out purposeful behaviour but may lie with eyes open, seemingly unaware of what is going on around him. A syndrome characterized by a silent and inert state without voluntary motor activity despite preserved sensorimotor pathways and vigilance. It results from bilateral damage to the orbital surface of the frontal lobes. The patient appears awake and has normal ocular movement but does not speak, is incontinent, and has minimal motor response to painful stimulation. A state in which a person is unspeaking (mute) and unmoving (akinetic). A textbook on clinical neurology observes that a person with akinetic mutism has "sleep-waking cycles but, when apparently awake, with eyes open, lies mute. immobile and unresponsive." Akinetic mutism is often due to damage to the frontal lobes of the brain.

    Mutism is a psychiatric disorder most commonly found in children, characterized by a persistent failure to speak in select settings, which continues for more than 1 month. These children understand spoken language and have the ability to speak normally. In typical cases, they speak to their parents and a few selected others. Sometimes, they do not speak to certain individuals in the home. Most are unable to speak in school, and in other major social situations.
